Long-term (>10-year) clinical follow-up after young embolic stroke/TIA of undetermined source.

BACKGROUND: To date, the pathophysiology of first-ever and recurrent stroke/TIA still remains unclear in young patients with embolic stroke/TIA of undetermined source (ESUS). Clinical studies with long-term follow-up in young ESUS patients are necessary to investigate the underlying pathophysiology of first-ever and recurrent stroke/TIA in this patient population, in particular the role of new-onset atrial fibrillation. AIMS: Our aim was to study the long-term (>10-year) clinical outcome of young patients (<50 years) with ESUS. METHODS: This cohort study included all patients aged ≤ 50 years…

Association Between Left Ventricular Ejection Fraction, Wall Motion Abnormality, and Embolic Stroke of Undetermined Source.

Background It is uncertain whether there is an association between left ventricular (LV) ejection fraction ( LVEF ) or LV wall motion abnormality and embolic stroke of undetermined source ( ESUS ). Methods and Results We performed a retrospective, cross-sectional study of patients with acute ischemic stroke enrolled in the CAESAR (Cornell Acute Stroke Academic Registry) from 2011 to 2016. We restricted this study to patients with ESUS and, as controls, those with small- and large-artery ischemic strokes. LVEF had to be above 35% to be considered ESUS . In a secondary analysis, we excluded pati…

Potential Role of 2-Phase Cardiac CT in Patients With Embolic Stroke of Undetermined Source.

BACKGROUND: Recent trials failed to demonstrate that anticoagulation was superior to antiplatelet agents in preventing recurrent stroke after embolic stroke of undetermined source (ESUS), which may be as a result of the heterogeneity of etiology. We thus investigated whether the use of 2-phase cardiac computed tomography (CT) could help to evaluate the etiology in patients with ESUS. METHODS: We performed 2-phase cardiac CT in patients with ESUS and evaluated whether there was positive cardiac findings. RESULTS: A total of 74 patients with ESUS were enrolled. Positive cardiac findings were f…

Embolic stroke of undetermined source and patent foramen ovale closure: Practice insights from meta-analysis?

Closure of patent foramen ovale (PFO) in embolic stroke of undetermined source (ESUS) reduces recurrent stroke compared with antiplatelet therapy in select patients but increases risk of atrial fibrillation. Device closure is an attractive option for young patients with an anatomically higher risk PFO and imaging proven ESUS. Benefit of PFO closure in ESUS compared with oral anticoagulation remains undefined.

Free fatty acid as a determinant of ischemic lesion volume in nonarterial-origin embolic stroke.

GOALS: This study aimed to determine whether the plasma levels of free fatty acid (FFA) are associated with ischemic lesion characteristics in nonarterial-origin embolic stroke. MATERIALS AND METHODS: We prospectively recruited 254 patients with acute cerebral infarction caused by cardioembolic stroke (CES, n=121) or with embolic stroke of undetermined source (ESUS, n=133). Plasma levels of FFA were measured during the acute stage (median of 2days after stroke onset). Acute ischemic lesions on diffusion-weighted imaging were measured in terms of size, composition, and pattern. Transthoracic e…

Detection of Left Ventricular Thrombus by Cardiac Magnetic Resonance in Embolic Stroke of Undetermined Source.

BACKGROUND AND PURPOSE: We aimed to use contrast-enhanced cardiac magnetic resonance (CE-CMR) imaging to elucidate the prevalence of left ventricular (LV) thrombus in patients suspected of embolic stroke of undetermined source (ESUS) with previous myocardial infarction or LV dysfunction (LV ejection fraction [LVEF] <50%). METHODS: We prospectively investigated 797 consecutive patients who presented to our hospital with acute ischemic stroke between 2014 and 2015. Patients with myocardial infarction or LVEF<50% underwent CE-CMR imaging. ESUS was diagnosed according to proposal criteria based o…

Embolic middle cerebral artery occlusion (MCAO) for ischemic stroke with homologous blood clots in rats.

Clinically, thrombolytic therapy with use of recombinant tissue plasminogen activator (tPA) remains the most effective treatment for acute ischemic stroke. However, the use of tPA is limited by its narrow therapeutic window and by increased risk of hemorrhagic transformation. There is an urgent need to develop suitable stroke models to study new thrombolytic agents and strategies for treatment of ischemic stroke. At present, two major types of ischemic stroke models have been developed in rats and mice: intraluminal suture MCAO and embolic MCAO. Although MCAO models via the intraluminal suture

Pilot study of cardiac magnetic resonance imaging for detection of embolic source after ischemic stroke.

BACKGROUND: Transesophageal echocardiography (TEE) is the standard for evaluating cardioembolic sources of stroke, although many strokes remain cryptogenic after TEE. Cardiac magnetic resonance (CMR) imaging may have advantages over TEE. We performed a prospective pilot study comparing CMR to TEE after stroke to assist in planning future definitive studies. METHODS: Individuals with nonlacunar stroke within 90 days of undergoing clinical TEE were prospectively identified and underwent a 1.5 Tesla research CMR scan. Exclusion criteria included >50% relevant cervical vessel stenosis and inabili

Circuitous embolic hemorrhagic stroke: carotid pseudoaneurysm to fetal posterior cerebral artery conduit: a case report.

INTRODUCTION: The cervical internal carotid artery (ICA) is susceptible to injury through various mechanisms, including dissection, which can lead to pseudoaneurysm formation. Pathological processes affecting the ICA, in association with an ipsilateral fetal posterior cerebral artery (PCA), resulting in parieto-occipital strokes are rarely reported. CASE PRESENTATION: We present a patient with a left PCA territory, presumably embolic, stroke with early hemorrhagic transformation. The identified nidus of the embolus was a carotid artery pseudoaneurysm. Manifestations included right homonymous

Neuroprotective effect of hyperbaric oxygen therapy monitored by MR-imaging after embolic stroke in rats.

The potential neuroprotective effects of hyperbaric oxygen (HBO) were tested in an embolic model of focal cerebral ischemia with partially spontaneous reperfusion. Rats (n = 10) were subjected to embolic middle cerebral artery occlusion (MCAO) and diffusion weighted MRI (DWI) was performed at baseline, 1, 3, and 6 h after MCAO to determine the ADC viability threshold yielding the lesion volumes that best approximated the 2,3,5-triphenyltetrazolium chloride (TTC) infarct volumes at 24 h (experiment 1). For assessment of neuroprotective effects, rats were treated with 100% oxygen at 2.5 atmosphe

Delayed treatment with a t-PA analogue and streptokinase in a rabbit embolic stroke model.

Fibrinolytic therapy may be effective in the treatment of ischemic stroke, and clinical trials are under way. We evaluated two fibrinolytic agents, an analogue of tissue plasminogen activator (Fb-Fb-CF, the catalytic fragment of the tissue plasminogen activator molecule with a prolonged serum half-life, n = 10) and streptokinase (n = 7), in a rabbit model of embolic stroke. Both agents were given 3 hours after stroke onset, a time relevant to the clinical setting. Fb-Fb-CF was significantly better (p less than 0.04) than saline (n = 7) in restoring blood flow to previously occluded intracrania…

Stroke and systemic embolism following atrial fibrillation ablation: the EMBOL-AF Global Registry.

BACKGROUND AND AIMS: Stroke and systemic embolism are recognized but poorly characterized complications of atrial fibrillation (AF) and left atrial flutter (AFL) catheter ablation. Their incidence, outcomes, and procedural associations remain insufficiently defined. METHODS: Retrospective global registry was initiated by the European Heart Rhythm Association Scientific Committee. A secure database captured data from ablation centres worldwide on general AF and left AFL ablation activity, as well as detailed information on patients with symptomatic embolic events between 2017 and 2024. Paradoxical embolism: A case of thromboembolic stroke secondary to pulmonary embolism and deep vein thrombosis.

Paradoxical embolism refers to thromboemboli originating from the venous system or right heart that traverse an intracardiac or pulmonary shunt into the systemic circulation, causing end-organ embolization. It is hypothesized to occur when the right atrial pressure exceeds the left atrial pressure. To date, patent foramen ovale is the most common intracardiac shunt associated with paradoxical embolism. During acute pulmonary embolism, acutely elevated right atrial pressure can reopen a functionally closed patent foramen ovale, facilitating paradoxical embolism into the systemic circulation. Th

Direct Oral Anticoagulants Versus Aspirin for Stroke Prevention After Embolic Stroke of Undetermined Source: An Updated Meta-Analysis of Randomized Controlled Trials.

Background: Four randomized controlled trials (RCTs) did not show a benefit of direct oral anticoagulant (DOAC) treatment compared with antiplatelet therapy for the prevention of recurrent strokes in patients with embolic stroke of undetermined source (ESUS). However, the balance between efficacy and safety in subgroups needs to be better defined. We aimed to assess the relative benefits of DOACs in key subgroups of adult patients with ESUS. Methods: We searched major databases (PubMed, Embase, CENTRAL, and Web of Science) for RCTs published from inception to 16 June 2024. The primary outcome …

Prevalence of Left Atrial Septal Pouch Among Patients with Embolic Stroke of Undetermined Source Or Stroke of Known Etiology: a Retrospective Study.

BACKGROUND AND OBJECTIVES: Cryptogenic strokes can be defined by the criteria established for an embolic stroke of undetermined source (ESUS). Some embolic events might be caused by a left atrial septal pouch (LASP), due to the potential of thrombus formation. In this study we aimed to determine if LASP is a risk factor for ESUS when compared to a population of strokes of known origin, the LASP screening rate in our institution and if LASP dimensions influences the risk of ESUS. METHODS: We retrospectively analyzed transesophageal echocardiograms (TEEs) in a large cohort of patients that had …

Ischemic Stroke Following Transradial Uterine Fibroid Embolization.

Uterine fibroid embolization is generally considered a standard treatment option for uterine fibroids. Thromboembolic complications, such as stroke, have been reported, but the incidence is rare. A 41-year-old woman presented with refractory menorrhagia due to a uterine fibroid and underwent uterine fibroid embolization via transradial access. On the second day after uterine fibroid embolization, she developed acute neurological deficits, including left-sided weakness, blurred vision, and ataxia. Magnetic resonance imaging revealed multiple posterior circulation infarcts without large vessel o

Mitral Valve Lymphoma: An Unusual Cause of Embolic Strokes.

An 80-year-old woman with a history of non-Hodgkin lymphoma and branch retinal artery occlusion presented with acute visual changes and received a diagnosis of multifocal embolic cerebral infarcts. Cardiac imaging revealed a mobile left atrial mass on the mitral valve. Surgical excision was undertaken because of the high risk of recurrent embolization and strokes. Histopathologic examination demonstrated chronic lymphocytic leukemia/small lymphocytic lymphoma within a fibrinous and calcified matrix. This case illustrates a rare intracardiac manifestation of lymphoma serving as a cardioembolic

Posterior circulation stroke after bronchial artery embolization.

We report on a 66-year-old woman with a posterior circulation stroke that occurred after bronchial artery embolization (BAE) due to post-tuberculous bronchiectasis. Stroke is a rare complication of BAE and is usually thought to be caused by inadvertent embolization via a bronchial artery-pulmonary vein shunt. However, the possibility of thromboembolic stroke should be considered, because of the patient's possible underlying anatomical variations or atherothrombosis.
