Atopic and seborrheic dermatitis: practical management.

Atopic dermatitis is common and causes sleep loss, a disfiguring appearance, an unpleasant odour, teasing, short stature and restriction of career choice in severe cases. There are no drugs which control the scratching; distraction, keeping the nails short and smooth, and the use of mittens at night are all helpful. The sedative action of the older H1 antihistamines makes them useful if scratching prevents a child from falling asleep. Emollients are useful for the associated skin dryness. The least potent topical steroids should be used sparingly to avoid the main hazards of skin atrophy, syst…

[Keratoderma palmoplantaris varians (striata et areata), a form of chronic idiopathic acrokeratosis described by Degos].

We report a case of a 15-year-old boy with hyperkeratotic lesions that were linear or striated on the palms and nummular on the soles. He was the only family member known to be affected, suggesting that the condition could be attributed to a de novo mutation or the recessive form of keratoderma palmoplantaris striata, described by Degos as chronic idiopathic acrokeratosis. The lesions did not improve with topical treatments (keratolytic agents, emollients, or corticosteroids) or oral retinoids. We observed that scratching of the affected areas was the main reason for deterioration of the lesio…

Shoe Dermatitis.

Shoe dermatitis is a type of contact dermatitis precipitated by allergens or irritants found in shoes. Potassium dichromate, commonly used in leather processing, is one of the most prevalent agents responsible for shoe dermatitis; however, it is not the only one. Shoe dermatitis caused by an allergen or an irritant may affect a person of any age, sex, or ethnicity. Numerous treatments exist for shoe dermatitis, the most simple yet important being avoidance of causative agents. Pharmaceutical agents commonly used are emollients, humectants, and topical corticosteroids. In more severe cases, top…

Prevention and treatment of diaper dermatitis.

Diaper dermatitis (DD) is one of the most common skin conditions that infants suffer from and their caregivers manage in the first months post-birth. As such, questions of effective prevention and treatment of the condition often arise. Nonmedical skincare practices that support healthy skin barrier function can prevent DD manifestation or alleviate the condition in many cases. The usage of barrier emollients and improved diaper technology contributes to keeping moisture and irritants away from an infant's delicate skin. This paper addresses facts behind commonly asked questions from caregiver…

Management of Atopic Hand Dermatitis.

This article provides an overview of clinical aspects of hand eczema in patients with atopic dermatitis. Hand eczema can be a part of atopic dermatitis itself or a comorbidity, for example, as irritant or allergic contact dermatitis. When managing hand eczema, it is important to first categorize the subtype and identify potential culprit allergens or irritants. First-line therapy should be a combination of emollients and topical corticosteroids; possible alternatives include topical calcineurin inhibitors or coal tar. Second-line therapy includes UV therapy and systemic therapy, including azat…

Nappy rash: a pharmaceutical approach.

Before dealing with nappy rash, first examine the cause. Simple precautions such as frequent changing of the nappy, cleaning the skin and applying a water-repellent barrier cream can prevent some types of nappy rash from occurring. Creams and ointments for nappy rash contain a variety of ingredients such as emollients, water-repellents, antiseptics, astringents and anti-inflammatory agents. Nappy rash caused by Candida (thrush) needs to be treated with specific antifungals, eg clotrimazole, nystatin, miconazole. Rashes which do not settle or which recur frequently should be referred to a docto…

Topical salicylic acid interferes with UVB therapy for psoriasis.

Salicylic acid has been widely used in the topical treatment of psoriasis. Chemically it is closely related to paraaminobenzoic acid. Following in vitro studies indicating that salicylic acid might exhibit relevant UVB absorption, we found that salicylic acid had a clinically pronounced filter effect when applied prior to UVB exposure. The duration of photoprotection after application was more than 12 h, sometimes exceeding 24 h. In a prospective, randomized, double-blind, left-right comparison study in patients with psoriasis between emollients with and without salicylic acid, salicylic acid …

Early-onset Pachyonychia Congenita with Oral and Cutaneous Manifestations.

An 11-year-old girl presented with painful plantar hyperkeratosis and nail dystrophy, evident since infancy. Examination revealed hyperkeratotic plaques on the soles, wedge-shaped nail hypertrophy, follicular hyperkeratosis over the extremities, and oral leukokeratosis. A positive maternal history suggested autosomal dominant inheritance with variable expressivity, consistent with pachyonychia congenita. Fungal infection was excluded via KOH mount; genetic testing was unavailable due to financial constraints. Despite prior treatments with keratolytics and emollients, improvement was minimal. T…

Current clinical practice in atopic dermatitis.

Atopic dermatitis affects both children and adults, and often presents in early life as itchy, dry skin primarily located on the face, trunk and limbs. There is usually a history of family or personal atopic disease and an increased blood level of the allergic antibody immunoglobulin E. The mainstays of treatment are emollients and corticosteroid ointments and/or creams. Other therapies include immunomodulators and management of any associated infections. Caring for a patient with atopic dermatitis requires skilled assessment, effective education and the ability to involve and support patients…

Male skin and ingredients relevant to male skin care.

Male skin care needs are heavily influenced by the need to remove facial hair on a regular basis. Facial skin issues associated with poor hair removal approaches are common and include razor burn and irritation. This paper evaluates current research on shaving technology and how careful ingredient selection can contribute to male skin health. The importance of maintaining hair softness during the shave and restoring facial hydration post-shave is discussed. Data are presented on how post-shave moisturizers containing glycerine and emollients can create an environment for improved barrier funct…

Pityriasis alba.

Pityriasis alba (PA) is a common benign condition in children that has no definitive treatment. Its etiology and pathogenesis are still poorly understood. Recent studies have found direct correlations between the incidence of PA and atopy, amount of sun exposure, lack of sunscreen use, and frequency of bathing. It is often an incidental finding on physical examination because it is usually asymptomatic. Although treatment with emollients and mild topical corticosteroids may accelerate the repigmentation, they have limited efficacy. Without intervention, the lesions normally resolve within mont…

Treatment of scabies with ivermectin.

The authors report six new cases of patients suffering from severe infestation with the mite sarcoptes scabiei, treated with ivermectin, currently the only oral therapy available for this disease. Each patient received 200 mug/kg of ivermectin, taken as single dose. No topical therapy was undertaken, except for topical treatment with emollient, as needed. The drug was very effective in all cases, easy to use, safe, and particularly useful in those patients with secondary eczematisation and escoriations, for whom the topical treatments are irritant and less well tolerated.

Dapsone-induced erythroderma with Beau's lines.

A 35-year-old female with borderline lepromatous (BL) leprosy who suffered from dapsone-induced erythroderma is reported. Sudden onset of erythroderma gave rise to a temporary arrest of the function of nail matrix with the resultant Beau's lines. She rapidly recovered with omission of dapsone and therapy with systemic corticosteroids and a topical emollient. In view of the potentially fatal hypersensitivity reaction, we suggest that any patient on multidrug therapy for leprosy needs an urgent referral to a dermatologist if the patient develops a skin rash during the first two months of treatme

Atopic and Contact Dermatitis.

Atopic dermatitis and contact dermatitis are among the most common dermatologic conditions encountered by the primary care clinician. Their presentation varies and can mimic infectious and other noninfectious conditions, making a thorough history essential, with particular emphasis on exposure to environmental and occupational irritants, as well as recognition of commonly affected sites. For all of these conditions, control of symptoms is often the goal of treatment, starting with general supportive care measures like frequent emollient use and often relying on topical steroidal and nonsteroid
