OBJECTIVE: Type II endoleak (T2EL) is the most common complication following endovascular aneurysm repair (EVAR) of abdominal aortic aneurysms (AAA). T2EL may lead to aneurysm sac expansion and rupture. Identifying high-risk patients is crucial for prophylaxis and early intervention.
METHODS: This single-center retrospective study included 332 patients who underwent EVAR for infrarenal AAA. Demographic, clinical, anatomical, and medication-related data were collected. A nomogram was developed based on significant predictors. Its performance was assessed by receiver operating characteristic (R…

BACKGROUND: Endovascular abdominal aortic aneurysm repair (EVAR) is a preferred surgery to prevent aneurysm sac enlargement and minimize the risk of life-threatening rupture in patients with AAA. Serious complications of type II endoleaks following EVAR can cause sac expansion and increase rupture risk. This study focused on evaluating clinical and blood characteristics in patients with type II endoleaks to refine our understanding of systemic fluctuations associated with unsuccessful EVAR.
METHODS: This retrospective study included 146 patients with AAA who underwent primary elective endovas…

INTRODUCTION: Endoleaks are more common after fenestrated/branched endovascular aneurysm repair (F/B-EVAR) than infrarenal EVAR secondary to the length of aortic coverage and number of component junctions. Although reports have focused on type I and III endoleaks, less is known regarding type II endoleaks after F/B-EVAR. We hypothesized that type II endoleaks would be common and often complex (associated with additional endoleak types), given the potential for multiple inflow and outflow sources. We sought to describe the incidence and complexity of type II endoleaks after F/B-EVAR.

An aneurysm sac enlargement caused by type II endoleak (T2EL) following endovascular aneurysm repair (EVAR) for abdominal aortic aneurysms may cause serious complications such as rupture. Consequently, methods that preoperatively prevent or postoperatively treat T2EL have been employed. When significant aneurysm enlargement occurs due to persistent T2EL, embolization is first performed through several access points. However, although these endovascular reinterventions have a high technical success rate and are safe, their effectiveness remains questionable. When such endovascular procedures fa…

Endoleaks are a common complication of endovascular aortic aneurysm repair and often lead to aneurysm rupture. We report a complex case of a Type IIIA endoleak in a patient with suspected cholangitis. Immediate surgical measures to manage both the cholangitis and endoleak were performed while minimizing the risk of graft infection.

OBJECTIVE: Presence of an endoleak can compromise aneurysm exclusion after endovascular abdominal aortic aneurysm repair (EVAR). Type II endoleaks (T2Es) are most common and may cause sac expansion. We report outcomes of translumbar embolization (TLE) of T2Es following EVAR.
METHODS: We conducted a retrospective chart review of patients with T2E after EVAR treated with TLE from 2011 to 2018 at a single academic institution. Treatment indications were the presence of persistent T2E and aneurysm growth ≥5 mm. Sac stabilization was defined as growth ≤5 mm throughout the follow-up period.

BACKGROUND: Complications after thoracic endovascular aortic repair (TEVAR) are common. Even after a successful TEVAR, a late endoleak (>30 days) can occur. The objective of this study is to summarize the current evidence and, if lacking, the need of evidence regarding the incidence and predictive factors for type 1b endoleak in patients with aortic aneurysm treated with TEVAR.
METHODS: A systematic review of the literature was performed on endoleak type 1b, in patients with aortic aneurysm, after TEVAR. The PubMed and Embase databases were systematically searched for articles regarding endol…

BACKGROUND: The number of ruptured abdominal aortic aneurysm (r-AAA) patients who are treated by endovascular means is increasing as ruptured endovascular aneurysm repair (r-EVAR) enters the mainstream. However, even today, data on the incidence and behavior of endoleaks after r-EVAR are scarce. This study analyzed whether endoleaks behave differently after EVAR for rupture vs elective AAA repair.
METHODS: From 2002 to 2013, there were 2052 patients who underwent EVAR for treatment of rupture (n = 166 [8.1%]) and elective repair (n = 1886 [91.9%]) of infrarenal AAA. Follow-up included compute…

Type 2 endoleaks (EL2) are the most commonly encountered endoleaks following EVAR. Despite two decades of experience, there remains considerable variation in the management of EL2 with controversies ranging from if to treat, when to treat and how to treat. Here, we summarise the available evidence, describe the treatment techniques available and offer guidelines for management.

OBJECTIVE: The aim of the study was to determine whether type II endoleak (T2E) after endovascular repair of abdominal aorta (EVAR) is a benign condition (ie, not associated with growth, reintervention, rupture, or death).
METHODS: Data from patients who underwent EVAR for atherosclerotic infrarenal aortic aneurysms between June 1995 and May 2010 in the Vascular Surgery Department of Henri Mondor Hospital were prospectively collected. Data from patients presenting with at least one T2E on computed tomography scan during their follow-up were compared with those with no T2E. Three subcategories…

A case of late type IA endoleak is reported. The endoleak occurred approximately 1 year after successful treatment of a type IB endoleak with iliac branch device implantation in a patient who had undergone endovascular aneurysm repair 10 years earlier for an abdominal aortic aneurysm with severe neck angulation. The patient underwent secondary endovascular revision, which proved ineffective, with persistence of the endoleak and progressive aneurysm sac enlargement. A minimally invasive open surgical approach with endograft preservation, consisting of external proximal aortic neck banding, was …

OBJECTIVE: Both standard and low-profile endografts have been used for physician-modified endografts (PMEGs) to treat complex aortic aneurysms; however, recent data from a 2025 multi-institutional analysis suggest that low-profile devices are associated with type IIIc endoleak rates as high as 15% at the 20-month follow-up. Early demonstrations of PMEG modifications, including fenestration reinforcement with polytetrafluoroethylene (PTFE) cuffs, have proposed a possible remedy to these elevated endoleak rates. This analysis evaluates a single center's experience with PTFE cuff reinforcement fo…
