[Cerebrovascular accidents in relation to drug consumption or drug abuse].

Many drugs may cause cerebral infarction or hemorrhage. The authors describe the epidemiology and the physiopathological aspects of stroke in patients using anticoagulant therapy, oral contraception or ergot alkaloids. Cerebrovascular complications are also noticed in abusers of cocaine or other stimulants of the central nervous system: amphetamine, phenylpropanolamine, xanthines.

Highly specific quantification of ergotamine in urine, blood, and hair samples by liquid chromatography-tandem mass spectrometry.

Ergotamine has been used for therapeutic purposes since the 1950s, usually to treat vascular headache. It is highly toxic and in large, repe … n trap mass spectrometer. The method was validated and successfully applied to a case of iatrogenic ergotism resulting from the intake of ergotamine tartrate for treating headache. For the first time, ergotamine was identified and quantified in hair. The ergotamine concentrations measured were 320 pg/mL in blood, 100 pg/mL in urine, 24 pg/mg in proximal ha …

Neurogenic inflammation in the pathophysiology and treatment of migraine.

The trigeminal nerve transmits headache pain from blood vessels of the pia mater and dura mater. Triggers for this pain are not well understood, but probably are multiple and largely chemical and develop within the brain parenchyma, the blood vessel wall, and the blood itself. These unknown triggers stimulate the trigeminovascular axons, causing pain and releasing vasoactive neuropeptides from perivascular axons. Released neuropeptides activate endothelial cells, mast cells, and platelets to then increase extracellular levels of amines, arachidonate metabolites, peptides, and ions. Hyperalgesi

Analysis of 113 Mycotoxins and Plant Toxins in Wheat, Soy and Pea Protein Concentrates from the German Market Reveals, i.a., a High Prevalence of Alterperylenol in Wheat Gluten.

Wheat, soy and pea protein isolates used for the production of plant-based meat alternatives were analyzed for 87 mycotoxins and 26 plant toxins by means of a validated QuEChERS-based LC-MS/MS multi-method. For wheat gluten, the results revealed the presence of Alternaria toxins, deoxynivalenol, enniatins, ergot alkaloids and ochratoxin A (OTA). For soy protein, beauvericin, enniatin B and OTA were the most frequently detected mycotoxins, while for pea protein, OTA was the most frequently detected mycotoxin. From plant toxins, the tropane alkaloids atropine and scopolamine were detected in whe

Medical therapy of prolactinomas.

Bromocriptine therapy normalizes prolactin levels and restores fertility in 82 to 90% of patients with prolactinoma. Tumour diameter is reduced (> 25%) in 60 to 79% of patients within 3 to 12 months. Symptoms return in the majority of cases, even after 1.5 to 10 years of treatment, if the drug is withdrawn; thus, in macroprolactinoma, life-long dopamine agonist treatment is indicated. A depot formulation of bromocriptine has been introduced which allows 50 to 100 mg of bromocriptine to be administered at monthly intervals. Treatment resulted in normalization of prolactin levels in 72% of cases

Pituitary genomic expression profiles of steers are altered by grazing of high vs. low endophyte-infected tall fescue forages.

Consumption of ergot alkaloid-containing tall fescue grass impairs several metabolic, vascular, growth, and reproductive processes in cattle, collectively producing a clinical condition known as "fescue toxicosis." Despite the apparent association between pituitary function and these physiological parameters, including depressed serum prolactin; no reports describe the effect of fescue toxicosis on pituitary genomic expression profiles. To identify candidate regulatory mechanisms, we compared the global and selected targeted mRNA expression patterns of pituitaries collected from beef steers th

Treatment of Parkinson's disease with pergolide and relation to restrictive valvular heart disease.

BACKGROUND: Restrictive valvular heart disease has been reported in patients with Parkinson's disease treated with pergolide. However, few data are available on frequency, severity, dose dependency, and reversibility of pergolide-induced disease, nor on the pulmonary pressures of these patients. We aimed to clarify these characteristics in a large group of patients. METHODS: 78 patients with Parkinson's disease treated with pergolide and 18 never treated with an ergot-derived dopamine agonist (controls) were evaluated by echocardiography. A valvular scoring system was used, ranging from 1 (pr

Medical therapy in idiopathic retroperitoneal fibrosis.

Except for its linkage with the ergot derivatives retroperitoneal fibrosis is a disease of unknown etiology. Two cases in which the traditional surgical approach was used but with the subsequent use of steroids are presented. The third patient was treated with steroids alone. The use of steroids alone is suggested for a limited number of poor risk patients when certain criteria are met.

[Clinical case of the month. Triple valvular dysfunction associated with long-term ergotamine therapy].

We report the case of a 48-year-old woman who present a tricuspid, mitral and aortic regurgitation after long term ingestion of ergot alkaloids. Tricuspid and mitral fibrosis associated with retraction of the aortic leaflets are the causes of heart failure. Replacement of the three valves will be necessary. This is one, not the least complication of long-term ergotamine therapy.

Bromocriptine and psychosis: a literature review.

Bromocriptine is an ergot-derived dopamine agonist. Its current uses include the treatment of Parkinson's disease, postpartum ablaction, prolactinomas, acromegaly, and amenorrhea and galactorrhea secondary to neuroleptic use. It is often reported to produce psychiatric side effects such as confusion, hallucinations, and delusions. The literature is reviewed and supports a strong anecdotal relationship between bromocriptine use and psychosis.

Inadvertent Methylergonovine Administration to a Neonate.

BACKGROUND Methylergonovine is an ergot alkaloid used to treat post-partum hemorrhage secondary to uterine atony. Mistaking methylergonovine for vitamin K with accidental administration to the neonate is a rare iatrogenic illness occurring almost exclusively in the delivery room setting. Complications of ergot alkaloids in neonates include respiratory depression, seizures, and death. CASE REPORT A term infant was inadvertently given 0.1 mg of methylergonovine intramuscularly in the right thigh. The error was only noted when the vial of medication was scanned, after administration, identifying

[Two cases of patients with Parkinson's disease developing valvular heart disease while taking cabergoline].

We experienced 2 patients of valvular heart disease in Parkinson's patients taking cabergoline. Patient 1 was a 79-year-old woman who began taking 4 mg cabergoline daily after being diagnosed with Parkinson's disease (PD) in June 2003. She presented with dyspnea in November 2005. The patient had cardiomegaly, pulmonary congestion, and pleural effusion, and an echocardiogram showed valvular heart disease in the form of aortic regurgitation (AR) (grade I), tricuspid regurgitation (TR) (grade I), and mitral regurgitation (MR) (grade III). Cabergoline was thought to have caused these phenomena, so

Role of serotoninergic pathways in drug-induced valvular heart disease and diagnostic features by echocardiography.

Serotonin plays a significant role in the development of carcinoid heart disease, which primarily leads to fibrosis and contraction of right-sided heart valves. Recently, strong evidence has emerged that the use of specific drug classes, such as ergot alkaloids (for migraine headaches), 5-hydroxytryptamine (5-HT or serotonin) uptake regulators or inhibitors (for weight reduction), and ergot-derived dopamine agonists (for Parkinson's disease), can result in left-sided heart valve damage that resembles carcinoid heart disease. Recent studies have suggested that both right-sided and left-sided dr

A study of excessive daytime sleepiness and its clinical significance in three groups of Parkinson's disease patients taking pramipexole, cabergoline and levodopa mono and combination therapy.

OBJECTIVE: To determine if therapy with an ergot and a non-ergot dopamine agonist and levodopa confers an increased risk of excessive daytime sleepiness and secondary "sleep attacks" in Parkinson's disease (PD). METHODS: Comparative study of three clinical groups taking, pramipexole (Group 1, n = 19, 8 monotherapy), cabergoline (Group 2, n = 22, 10 monotherapy), and levodopa monotherapy (Group 3, n = 14). Clinical and demographic characteristics, occurrence of "sleep attacks", and assessment of daytime sleepiness [using the Epworth Sleepiness Scale (ESS)], recorded. RESULTS: No patients repo

Dopamine receptor affinities in vitro and stereotypic activities in vivo of cabergoline in rats.

An ergot alkaloid derivative, cabergoline, and its metabolites were investigated for their affinities for dopamine D1 and D2 receptors in rat striatum in vitro in comparison with those of bromocriptine and pergolide. The affinity for D1 receptors was in the following order: pergolide > des-dimethylaminopropyl cabergoline (FCE21904) > cabergoline > or = bromocriptine > or = des-methyl cabergoline (FCE27395) > or = des-ethylcarbamoyl cabergoline (FCE21590). From the effects of GTP on these affinities for the D1 receptor, cabergoline, some of its metabolites, and pergolide were characterized as a

The endocrine disruptive effects of ergopeptine alkaloids on pregnant mares.

During equine gestation, ergopeptine alkaloid exposure is not uncommon, and pregnant mares are particularly sensitive to the endocrine disruptive effects of these compounds on lactogenesis and steroidogenesis. Agalactia, prolonged gestation, abortion, dystocia, and placental and fetal abnormalities are all clinical manifestations of changes in the endocrine milieu induced by the ingestion of ergopeptine alkaloid-contaminated feedstuffs by mares during late gestation. An understanding of the endocrine disruptive effects of gestational exposure to ergopeptine alkaloids is necessary for the diagn

[Vasospastic angina pectoris following abortion induced by prostaglandin analogue].

A case of vasospastic angina pectoris with loss of consciousness, bradycardia and seizures induced by medical abortion following administration of mifepristone and gemeprost is reported. The patient had a history of smoking and migraine, and former treatment with ergot alkaloids or serotonin agonists had also resulted in chest pain and lipothymia. The case underlines the importance of obtaining a detailed history of vasospastic disorders in women referred for medical abortion.
