HISTORY AND ADMISSION FINDINGS: We report on four patients treated for typical hemolytic uremic syndrome (HUS) at our hospital during the outbreak in Germany in May 2011. Three out of four patients had previously travelled to northern Germany, one patient was exposed within the family.
INVESTIGATIONS: All patients were tested positive for enterohemorrhagic E.coli of the type O104:H4 in stool cultures and presented with signs of thrombotic microangiopathy and fragmented erythrocytes.
DIAGNOSIS, TREATMENT AND COURSE: Due to strong hemolytic activity (LDH >600 U/l), thrombocytopenia (< 60,000/µ

AIMS: In 1936 the Finnish Anti-Tuberculosis Association founded the first nursery, "Joulumerkkikoti", into which infants born into tuberculous families were admitted and given BCG vaccination to reduce the risk of tuberculosis. This prophylactic regimen was effective in reducing infant mortality and morbidity of tuberculosis. We investigated the mortality of these children later in childhood and adulthood.
METHODS: The index cohort consisted of 3,020 subjects born between 1945 and 1965 in Finland and isolated from their family immediately after birth. The average separation time was 218 days.

The title novel phosphonium-based manganese(II) hybrid crystal, {(C24H26O2P)2[MnCl4]}n or (TBTP)2[MnCl4], was synthesized via slow evaporation. Single-crystal X-ray diffraction reveals that it crystallizes in the monoclinic space group P21/n, featuring discrete slightly distorted tetrahedral [MnCl4]2- anions and bulky (TBTP)+ cations [(TBTP)+ is (tert-butoxycarbonyl)methyltriphenylphosphonium] with weak C-H...Cl interactions. The material exhibits green luminescence centred at 515 nm with a millisecond-scale lifetime (2.83 ms), which is unequivocally assigned to the long-lived spin-forbidden M

BACKGROUND: Slit diaphragm is a specialized adhesion junction between the opposing podocytes, establishing the final filtration barrier to urinary protein loss. At the cytoplasmic insertion site of each slit diaphragm there is an electron-dense and protein-rich cellular compartment that is essential for slit diaphragm integrity and signal transduction. Mutations in genes that encode components of this membrane-less compartment have been associated with glomerular diseases. However, the molecular mechanism governing formation of compartmentalized slit diaphragm assembly remains elusive.

A decade of genetic counseling of frontotemporal dementia (FTD) affected families has generated two important observations. First, the uptake rate for presymptomatic testing for FTD is low in our department of Clinical Genetics at the Erasmus Medical Center in the Netherlands. Second, FTD at-risk counselees reported substantial familial opposition to genetic testing, which is distinct from the attitude in Huntington Disease affected families. We hypothesize that the low acceptance for FTD genetic counseling is consequential to the familial opposition and explain this within the theoretical fra

In my work with a migrant family, I explored the youngest daughter's symptomatology and relationship with her mother; analysis offered an approach to her traumatic history, as well as her private pain, which I was initially able to experience through the complex psychological fabric woven between both mother and daughter. The ideas contributed in this paper are drawn from the clinical case, where the central focus on traumatism, pain and migration were the ostensible grounds with which the family sought assistance; however, other issues arose, such as the daughter's entry into adolescence, psy

BACKGROUND: The purpose of this study was to examine the health and social circumstances, knowledge of and access to health and social services, and success at getting work among urban day laborers.
METHODS: We conducted an interview survey of 217 men waiting for work at several sites in San Francisco.
RESULTS: Day laborers were generally unsuccessful at obtaining work and had less than optimal housing, but supported a number of family members. Over half reported fair or poor health, associated with longer time as a day laborer, poor English proficiency, and financially supporting three or m

BACKGROUND: Few studies examine risk to offspring who experience both parental alcohol problems and parental separation and still fewer consider gender of the affected parent. We examined interactive effects of maternal versus paternal alcohol problems and parental separation on timing of first alcoholic drink in daughters.
METHODS: Data were drawn from a sample of 3,539 European (or other) ancestry (EA) and 611 African ancestry (AA) female twins born between 1975 and 1985, median age 15 at first assessment. Cox proportional hazards regression models were estimated predicting age at first ful

BACKGROUND: A review of the studies examining the genetic etiology of panic disorder shows the familial nature of the disorder and demonstrates that the etiology is greatly influenced by genetic factors. Strong evidence for vertical transmission in family studies led to molecular genetic studies, of which association designs appear promising, particularly when based on trait markers.
DATA SOURCES: The MEDLINE and PsycLIT databases were searched for all reports published between 1966 and 2000 containing the keywords panic, genetic, twin, adoption, linkage, association, and QTL.
CONCLUSION: We

Although juvenile separation anxiety disorder is maintained to be a predisposing factor to adult panic disorder in DSM-III-R, past research has failed to clarify (a) whether it is separation anxiety per se or school refusal that is the pathogenic risk factor and (b) whether affected youngsters are specifically at risk of developing panic disorder rather than symptoms of general anxiety or phobias in later life. The present study of 74 adults who responded to media publicity found that a measure of early separation anxiety but not a history of school refusal was associated with risk of adult pa

PURPOSE: In this commentary we describe our experience developing a "gentle cesarean" program at a community hospital housing a family medicine residency program. The gentle cesarean technique has been popularized in recent obstetrics literature as a viable option to enhance the experience and outcomes of women and families undergoing cesarean delivery.
METHODS: Skin-to-skin placement of the infant in the operating room with no separation of mother and infant, reduction of extraneous noise, and initiation of breastfeeding in the operating room distinguish this technique from traditional cesar

BACKGROUND: Family social support, as a form of social capital, contributes to social health disparities at different age of life. In a life-course epidemiological perspective, the aims of our study were to examine the association between self-reported family social environment during childhood and self-reported health in young adulthood and to assess the role of family functioning during childhood as a potential mediating factor in explaining the association between family breakup in childhood and self-reported health in young adulthood.
METHODS: We analyzed data from the first wave of the H

PURPOSE: Family structure transitions affect adolescents' mental health, yet uncertainty remains about their type, frequency, and timing effects. This longitudinal study examines these transitions and their association with early adolescents' internalizing and externalizing problems.
METHOD: Using data from the French EDEN cohort, a Sequence Analysis was performed to cluster participating mothers (n = 1,915) according to changes in family structure over time, defined as transitions between living with the child's biological father, alone, with a new partner or with other adults. Among adolesc

BACKGROUND: While homesickness in refugees is a recurring theme in clinical practice, respective research in this population is scarce. The Utrecht Homesickness Scale (UHS) allows distinguishing between certain aspects of homesickness, namely genuine separation distress like missing family and friends or yearning for home on the one side and problems regarding adjustment to the new situation on the other; so far, the instrument was applied mainly in samples of university students, and never in refugees.
OBJECTIVE: We aimed to explore homesickness in a refugee population and its association wi

The myriad of negative effects brought about by the incarceration of a family member have consistently been demonstrated in research. However, previous works have tended to focus on the perspectives of family members separately, rather than exploring the dynamic relationships within the family as an entire unit. Moreover, such research is still limited in the Chinese cultural context. Thus, the current study aimed to examine the applicability of the Family Process Theory on a small sample of Chinese fathers who were imprisoned and have returned to their communities, conceptualizing the challen

BACKGROUND/PURPOSE: From 1978 to 2000, 10 pairs of conjoined twins were successfully separated with 14 surviving twins. Six were separated emergently because of the death or impending death of their respective twins. All of the remaining 4 sets required at least one emergency operation on at least one twin before subsequent separations.
METHODS: A 6-item questionnaire was developed to interview this unique cohort. The question design was open ended to maximize opportunity for patient/family response. Families were administered the same questionnaire in face-to-face interviews. The physician-a

Animal studies have reported associations of early maternal separation with altered μ-opioid receptor function but data on humans are scarce. We now investigated whether childhood family environment is related to μ-opioid receptor availability in the human brain in adulthood. Healthy participants (n = 37-39 in the analyses) were recruited from the prospective population-based Young Finns Study (YFS) that started in 1980. Childhood family environment was evaluated in 1980, including scores for stress-prone life events, disadvantageous emotional family atmosphere, and adverse socioeconomic envir

A review of family and twin studies using specified diagnostic criteria shows the highly familial nature of panic disorder and suggests evidence for a genetic etiology. The population-based lifetime rates of panic disorder cross-nationally range between 1.2/100 and 2.4/100, whereas, the lifetime rates in first-degree relatives of panic probands range between 7.7/100 and 20.5/100. There is evidence from family and twin studies for the separation of panic disorder and generalized anxiety disorder. While there is a substantial comorbidity in individuals between panic disorder and major depression
