Department of Defense positions on alternative uses of hyperbaric oxygen therapy.

Hyperbaric oxygen therapy is an existing and approved treatment to address multiple medical conditions, including decompression sickness, air or gas embolism, carbon monoxide poisoning, and profound blood loss when transfusion cannot be accomplished. However, recent efforts have emerged to promote hyperbaric oxygen therapy for other purposes. The most controversial applications have been utilizing this therapy as a treatment for mild traumatic brain injury and post-traumatic stress disorder. As evidence accumulates and the debate continues about whether published studies have satisfied the thr

A look at state-level risk assessment in the United States: making decisions in the absence of federal risk values.

State environmental agencies in the United States are charged with making risk management decisions that protect public health and the envir … . Within this context, this article describes the decision-making process and resources used by the federal government and individual U.S. states. The risk management of trichloroethylene (TCE) in the United States is presented as a case study to demonstrate the need for a collaborative approach among U.S. states toward identification and selection of chemical risk …

Creating a science of homelessness during the Reagan era.

UNLABELLED: POLICY POINTS: A retrospective analysis of federally funded homeless research in the 1980s serves as a case study of how politics can influence social and behavioral science research agendas today in the United States. These studies of homeless populations, the first funded by the National Institute of Mental Health, demonstrated that only about a third of the homeless population was mentally ill and that a diverse group of people experienced homelessness. This groundbreaking research program set the mold for a generation of research and policy characterizing homelessness as primar

Examining the Potential Role of Opioid Settlement Funds in the Face of Impending Federal Budget Reductions for Substance Use Disorders.

OBJECTIVE: To assess the potential of opioid settlement dollars disbursed to state and local governments to replace or supplement federal substance use disorder (SUD) funding. STUDY SETTING AND DESIGN: For each state, we calculated the percentage of Substance Use and Mental Health Services Administration (SAMHSA) SUD funding for each state that can be offset by settlement funds. DATA SOURCES AND ANALYTIC SAMPLE: We estimated annual opioid settlement distributions (2022-2038) from KFF Health News and SAMHSA-funded SUD awards (2024) from SAMHSA's website and the Tracking Accountability in Gove

Take-home Naloxone at Opioid Treatment Programs: A Lifesaver.

Opioid-related overdose deaths have increased almost 30% in the US since the COVID-19 pandemic began. Tragically, many of these deaths could be prevented with widespread availability of naloxone. One innocuous harm-reduction strategy would be the federal government mandating the provision of take-home naloxone and brief overdose education to patients at opioid treatment programs. Take-home naloxone, for instance, may be used by a friend or a family member to save the life of the patient receiving treatment for opioid use disorder. Importantly, many studies demonstrate that patients receiving t…

Charting A Path Forward For Medicare Advantage: Expert Consensus On Policy Priorities And Trade-Offs.

More than half of Medicare beneficiaries are enrolled in Medicare Advantage (MA), yet concerns persist about overpayments to MA plans, inadequate plan oversight, and limits on beneficiaries' access to care. To identify MA reform options, we conducted a two-round e-Delphi panel with thirty-nine experts, including researchers, industry representatives, patient advocates, and former policy makers. Panelists rated fifteen policy reform ideas on their impact on beneficiaries, the federal government, and MA plans. Experts agreed that six policy ideas would benefit beneficiaries, three would benefit …

Implementing Family on Alert Program to Address Drug Abuse in Malaysia: A Qualitative Exploratory Study.

Drug misuse causes serious health problems, including the increased risk of addiction, particularly among adolescents. Hence, a family-based drug prevention program is designed to reduce problems among youth and encourage positive development at the family level. This study aims to explore the effectiveness of the Family on Alert (FoA) program implemented by the National Anti-Drug Agency in Malaysia. A qualitative method was used employing Focus Group Discussion (FGD) with 18 informants from NADA's state officers of Kuala Lumpur Federal Territory, Selangor, Johor, Kedah, and Sabah. Thematic an

Industries in the United States with airborne beryllium exposure and estimates of the number of current workers potentially exposed.

Estimates of the number of workers in the United States occupationally exposed to beryllium were published in the 1970s and 1980s and ranged from 21,200 to 800,000. We obtained information from several sources to identify specific industries with beryllium exposure and to estimate the number of current workers potentially exposed to beryllium. We spoke with representatives from the primary beryllium industry and government agencies about the number of exposed workers in their facilities. To identify industries in the private sector but outside the primary industry, we used data from the Integr

Helping smokers quit improves health and budgets.

(1) Treatment of smoking-related illnesses accounts for about 11 percent of Medicaid expenditures. (2) As of 2014, seven states provide comprehensive tobacco-cessation coverage to Medicaid enrollees, including individual counseling, group counseling, and seven types of U.S. Food and Drug Administration-approved medications. (3) Without assistance from medication or other forms of help, around 4 percent to 7 percent of quit attempts are successful; with tobacco-cessation medication use, the success rate is 25 percent.

The first pharmacoepidemiologic investigations: national drug safety policy in the United States, 1901-1902.

The pharmaceutical industry developed in the late 19th century as a consequence of both scientific and commercial innovations, such as the development of diphtheria antitoxin and the commercialization of smallpox vaccine. Two tetanus outbreaks in 1901 -- from contaminated diphtheria antitoxin in St. Louis, Missouri, and contaminated smallpox vaccine in Camden, New Jersey -- raised public concern about pharmaceutical safety. In St. Louis, errant manufacturing processes were found to be the source of the outbreak. In Camden, investigation identified contaminated vaccine from one manufacturer as

Medicaid Inmate Exclusion Policy and Infectious Diseases Care for Justice-Involved Populations.

The Medicaid Inmate Exclusion Policy (MIEP) prohibits using federal funds for ambulatory care services and medications (including for infectious diseases) for incarcerated persons. More than one quarter of states, including California and Massachusetts, have asked the federal government for authority to waive the MIEP. To improve health outcomes and continuation of care, those states seek to cover transitional care services provided to persons in the period before release from incarceration. The Massachusetts Sheriffs' Association, Massachusetts Department of Correction, Executive Office of He…

Spina bifida advocacy--a case study in public policy success.

Advocacy. Public policy. Politics. Congress and the Administration. Given recent events, these words often cause people to groan, roll their eyes, tune-out, or launch into a litany of complaints about the government. While each of these responses is understandable, the Spina Bifida Association (SBA) maintains a steadfast commitment to working with individuals, families, physicians, nurses, and other caregivers to channel their sense of frustration with government, public policy, and regulations into positive change for the Spina Bifida community. SBA engages in health policy advocacy to help f

Current Reproductive Health Policy and Outcomes in the United States.

Regulation of abortion, contraception, and other facets of reproductive health care are historically intertwined into our nation's approach to controlling reproduction. Government interference into reproductive health care is associated with worse maternal and infant health outcomes. In the following review, we discuss the history of these restrictions and detail the ways in which federal and state-level policies restricting abortion, contraception, care for minors, and funding for reproductive health services impact reproductive health outcomes, including maternal and infant morbidity and mor

The federal smallpox vaccination program: where do we go from here?

Despite the underwhelming response to the federal government's initiative to inoculate health care workers against smallpox, the Bush administration has indicated that the program will continue and that it could be expanded to include other health care providers, emergency service workers, and others deemed essential for continuity of government. We discuss the reasons for the program's failure to date, outline recent advisories questioning the extent of the vaccination effort, and suggest suspension of further vaccination efforts until certain questions and issues are resolved.
