Comparative study of intertrochanteric fracture fixation using proximal femoral nail with and without distal interlocking screws.

BACKGROUND: Intertrochanteric (IT) fracture is one of the most common fractures seen in an orthopaedic practice. Proximal femoral nailing (PFN) is a common modality of fixing IT femur fracture. We retrospectively studied whether a PFN with two proximal lag screws can be done without distal interlocking screws in the 31-A1 and 31-A2 fracture patterns according to the Arbeitsgemeinschaft für Osteosynthesefragen/Orthopaedic Trauma Association (AO/OTA) guidelines for IT femur fractures. AIM: To compare the outcomes of IT fractures (AO/OTA 31-A1 and 31-A2) treated by PFN with and without distal in

Salter-Harris Type III and Type IV Combined Fracture of the Distal Femoral Epiphysis: A Case Report.

Distal femoral physeal fractures are not common but have a high rate of complications. They generally follow one of the patterns described in the Salter-Harris classification. We present a case of combination of Salter-Harris type III and type IV injury. Our case was a 15-year-old boy who had a motor vehicle accident. There was swelling, ecchymosis, severe pain, and valgus deformity, because of medial proximal fracture fragment, on the left knee. We deemed that Salter-Harris type III and type IV combination fracture in our case has not been previously reported. We prepared this paper in consid

A custom coupling device of total knee and ipsilateral total hip arthroplasties after distal femoral fracture.

The treatment of periprosthetic femoral fractures around long-stemmed hip implants is an orthopedic challenge, which may be complicated by the presence or need for a total knee arthroplasty. Treatment of this fracture when the proximal implant is well fixed and a distal implant is required poses ongoing challenges. Traditional plating and allograft struts have produced variable results, particularly in osteoporotic bone, where incidence of nonunion is greater. Thus, we report a custom interlocking device, which couples the stem of the fixed proximal implant to a new stemmed total knee prosthes

Biomechanical comparison of distal femoral fracture fixation: Polyaxial vs. uniaxial locking screws with different blocking screw configurations in retrograde femoral nailing.

INTRODUCTION: Clinical studies have indicated reduced reliance on blocking screws with the introduction of polyaxial locking nails, yet the biomechanical superiority of polyaxial locking over blocking screws remains unverified, specifically for distal dia-metaphyseal femur fractures. HYPOTHESIS: We hypothesized that the combination of multiaxial locking using three screws and parallel locking reinforced by two blocking screws would result in superior outcomes compared to utilizing only parallel locking with two screws and one blocking screw. MATERIALS AND METHODS: Third-generation custom-mad

Periprosthetic fracture around a stable femoral stem treated with locking plate osteosynthesis: distal femoral locking plate alone versus with cerclage cable.

To promote rapid bone healing, an adequate stable fixation implant with a percutaneous reduction instrument should be used for Vancouver type B1 or C fractures. The objective of this study was to describe radiographic and clinical outcomes of patients with periprosthetic fracture (PPF) around a stable femoral stem, treated with a distal femoral locking plate alone or with a cerclage cable. A total of 21 patients with PPF amenable to either a reverse distal femoral locking plate (LCP DF®) alone or with a cerclage cable, with a mean age of 75.7 years, were included. In these patients, ten fractu

A surgical algorithm for the management of recalcitrant distal femur nonunions based on distal femoral bone stock, fracture alignment, medial void, and stability of fixation.

BACKGROUND: Recalcitrant distal femur nonunions (RDFN) are a challenge in management due to factors including poor bone stock, multiple surgeries, metaphyseal bone loss, and joint contractures. There are no specific guidelines in the management of cases of RDFN. Based on our experience, we devised an algorithm and we present the results of 62 cases of RDFN managed following it. MATERIALS AND METHODS: Our algorithm was formulated after analyzing 34 cases of RDFN and it involved four factors which were hypothesized to influence outcomes namely: distal femoral bone stock, extent of medial void,

Nail-Plate Combination Fixation for Periprosthetic Distal Femoral Fractures Allows Early Weight-Bearing With Low Reoperation and Revision Rates.

BACKGROUND: A combination retrograde femoral intramedullary nail (IMN) and locking plate for periprosthetic distal femoral fractures (PDFFs) may offer improved construct strength and early weight-bearing versus locked plating constructs or IMN fixation. This study compared clinical and radiographic outcomes in PDFF treatment with nail-plate combination (NPC), locked plating, or IMN. METHODS: Patients aged ≥ 18 years undergoing PDFF surgery between January 1, 2015, and April 1, 2024, at three institutions were retrospectively identified. Patients were included with NPC, locked plate only (i.e.

Low risk of nonunion with lateral locked plating of distal femoral fractures-A retrospective study of 191 consecutive patients.

INTRODUCTION: The reported rate of nonunion of distal femoral fractures varies in the literature. Several risk factors for nonunion following lateral locked plating (LLP) have been described. We aimed to study the rate of nonunion, and risk factors thereof, in a Swedish population where fragility fractures are common. A secondary aim was to study risk factors for reoperation for any cause. PATIENTS AND METHODS: We retrospectively reviewed the hospital files and radiographs of all adult patients admitted to our institution with a distal femoral fracture, from 2004 through 2013. Risk Factors for Distal Femoral Periprosthetic Fractures after Total Knee Arthroplasty: A Systematic Review of Patient and Implant Variables.

The number of total knee arthroplasty (TKA) procedures performed continues to rise annually. As projections show a continued increase in these surgeries, it is important to identify patients at risk for certain complications. Distal femur periprosthetic fractures around TKA can pose notable morbidity to patients, while also imparting a marked burden on the health system. This systematic review aims to identify patient and implant characteristics that increase the risk of distal femur periprosthetic fractures. Three common literature databases were searched for "distal femur periprosthetic frac

Distal Femoral Replacement for Fragility and Periprosthetic Fractures in a District General Hospital: A Five-Year Retrospective Cohort Study.

Background Distal femur fractures in elderly or comorbid patients pose a significant challenge due to poor bone stock, complex fracture morphology, and limited tolerance for prolonged immobility. Distal femoral replacement (DFR) is increasingly utilised as an alternative to internal fixation, but evidence for its use outside tertiary centres remains limited. Methods We retrospectively reviewed all DFR procedures for trauma indications at a district general hospital (DGH) over a five-year period (2019-2024). Patient demographics, perioperative variables, infection rates, transfusion requirement

Rates of interlock screw back-out are high with the retrograde femoral nailing advanced system for distal femur fractures.

PURPOSE: The retrograde femoral nailing advanced (RFNA) system (DePuy synthes) is a commonly used implant for the fixation of low distal femur and periprosthetic fractures. There is concern that the rate of distal interlock screw back-out may be higher for the RFNA compared to other nails (ON). The purpose of this study was to evaluate the incidence of interlock screw back-out and associated screw removal for RFNA versus ON, along with associated risk factors. METHODS: A retrospective comparative study of patients who underwent retrograde nailing for a distal femur fracture at an academic lev

Early Full Weight-Bearing Exercises to Improve Gait Function in Older Adult Patients Following Surgery for Distal Femoral Fractures.

INTRODUCTION: Postoperative rehabilitation for distal femur fracture generally requires gait exercises with weight-bearing restrictions. We aimed to determine whether full weight-bearing gait exercises immediately postoperatively effectively maintain walking ability in older adult patients with distal femur fractures. METHODS: Patients diagnosed with distal femur fractures between April 2017 and March 2024 were retrospectively evaluated. Patients aged ≥75 years who had undergone surgical treatment were classified into full weight-bearing (FWB) and non-weight-bearing (NWB) groups, with FWB and
