Predictors of stress in parents of children with fetal alcohol spectrum disorders.

This study examined child characteristics and family factors as predictors of stress in the biological or adoptive parents of 6- to 12-year-old children with fetal alcohol spectrum disorders (FASDs). Impaired executive functioning, poorer adaptive functioning, externalizing and internalizing behavior problems, and adoptive parent status all made significant and independent contributions to the prediction of higher levels of child domain stress, as reported by parents on the Parenting Stress Index. Biological parent status and fewer family resources were associated with higher levels of parent …

A pilot community intervention for young women with fetal alcohol spectrum disorders.

Fetal Alcohol Syndrome, a permanent birth defect caused by maternal alcohol use during pregnancy, is a leading preventable cause of mental retardation. Neuropsychological deficits have been well documented, however interventions developed have not been evaluated. We describe a successful 12-month community pilot intervention with 19 young women with Fetal Alcohol Spectrum Disorders (FASD). Improved outcomes (including decreased alcohol and drug use, increased use of contraceptives and medical and mental health care services, and stable housing) were obtained by implementing a community interve…

Cognitive and transcriptomic effects of Epigallocatechin gallate in fetal alcohol spectrum disorders.

Prenatal alcohol exposure impacts fetal development, leading to Fetal Alcohol Spectrum Disorders (FASD) characterized by cognitive, behavioral, and physical impairments. This pre-post, open-label, non-randomized pilot study explores Epigallocatechin-3-Gallate (EGCG), a potent antioxidant and neuronal plasticity modulator, as a therapeutic intervention for FASD improvement. In a 12-month pilot study, patients with 40 FASD (mean age 10 ± 3 years) received 9 mg/kg/day of EGCG, with outcomes assessed through RNA sequencing and neurocognitive evaluations (WISC-IV, CBCL 6-18, and NEPSY-II). Reduced …

Executive and Social Functioning in Fetal Alcohol Spectrum Disorders: Comparison to Autism.

Executive function is an area of challenge for both children with fetal alcohol spectrum disorders (FASD) and children with autism spectrum disorder (ASD). Parent ratings of everyday executive function relate to a range of outcomes, including social functioning in ASD. Comparisons between FASD and ASD have revealed both overlapping and distinct skills, but have not addressed executive function or its relation to social function. Utilizing parent report, the current study addressed relative strengths and weaknesses across scales of everyday executive function, as well as group differences betwe…

Gut Microbiome, Neuroinflammation, and Fetal Alcohol Spectrum Disorders: Insights from Rodent Models.

Fetal alcohol spectrum disorder (FASD) is a significant public health issue that affects children. It results from ethanol exposure during pregnancy, leading to considerable physical, neurodevelopmental, behavioral, and cognitive deficits. The exact mechanism is not well understood. Recently, considerable attention has been focused on the influence of gut microbiome on brain development through the gut-brain axis. Changes in the gut microbiome resulting from ethanol exposure may contribute to the pathology of FASD, potentially involving neuroinflammation. This literature review summarizes the

Neuroprotective Role of DING protein in Fetal Alcohol Spectrum Disorders and Depression.

INTRODUCTION: An estimated 20% of women consume alcohol during pregnancy, and 10% of women receive antidepressants during their pregnancy. Women with depression are more likely to use alcohol (EtOH) in early pregnancy and are more likely to have a child with one of the fetal alcohol spectrum disorders (FASD). Previously, we provided evidence in rat neurons in vitro that DING phosphatase (p38SJ, a member of the DINGGG family of proteins that has neuroprotective effects under conditions of cellular stress) was neuroprotective against EtOH-mediated toxicity. Now, we examine the effects of DING, a…

Reduction of APOE accounts for neurobehavioral deficits in fetal alcohol spectrum disorders.

A hallmark of fetal alcohol spectrum disorders (FASD) is neurobehavioral deficits that still do not have effective treatment. Here, we present that reduction of Apolipoprotein E (APOE) is critically involved in neurobehavioral deficits in FASD. We show that prenatal alcohol exposure (PAE) changes chromatin accessibility of Apoe locus, and causes reduction of APOE levels in both the brain and peripheral blood in postnatal mice. Of note, postnatal administration of an APOE receptor agonist (APOE-RA) mitigates motor learning deficits and anxiety in those mice. Several molecular and electrophysiol…

Machine learning algorithms to the early diagnosis of fetal alcohol spectrum disorders.

INTRODUCTION: Fetal alcohol spectrum disorders include a variety of physical and neurocognitive disorders caused by prenatal alcohol exposure. Although their overall prevalence is around 0.77%, FASD remains underdiagnosed and little known, partly due to the complexity of their diagnosis, which shares some symptoms with other pathologies such as autism spectrum, depression or hyperactivity disorders. METHODS: This study included 73 control and 158 patients diagnosed with FASD. Variables selected were based on IOM classification from 2016, including sociodemographic, clinical, and psychological…

Italian Guidelines for the diagnosis and treatment of Fetal Alcohol Spectrum Disorders.

Drinking alcohol during pregnancy can cause congenital disabilities. J. Roquette, P. Lemoine and K.L. Jones were the first to describe these effects. In 1973, Jones and Smith coined the term Fetal Alcohol Syndrome to describe children with facial anomalies, poor growth, and learning difficulties. The caution against drinking during pregnancy has existed for centuries, including in The Bible (Judges 13:3-4). Maternal alcohol consumption is linked to congenital disabilities. To ensure safety, it is advised to abstain from alcohol during pregnancy. Fetal Alcohol Spectrum Disorder (FASD) was obser

Brain Volume in Fetal Alcohol Spectrum Disorders Over a 20-Year Span.

IMPORTANCE: Anomalous brain development and mental health problems are prevalent in fetal alcohol spectrum disorders (FASD), but there is a paucity of longitudinal brain imaging research into adulthood. This study presents long-term follow-up of brain volumetrics in a cohort of participants with FASD. OBJECTIVE: To test whether brain tissue declines faster with aging in individuals with FASD compared with control participants. DESIGN, SETTING, AND PARTICIPANTS: This cohort study used magnetic resonance imaging (MRI) data collected from individuals with FASD and control individuals (age 13-37…

[Fetal Alcohol Spectrum Disorders in Adults - Results from a Diagnostic Outpatient Clinic].

AIM OF THE STUDY: Fetal alcohol spectrum disorders (FASD) are quite common and, due to the risk of psychiatric comorbidities, highly relevant until adulthood. Diagnostic clarification in adulthood is a prerequisite for targeted treatment and needs-based support. METHODS: In a German metropolitan region, 80 people with suspicion of FASD were assessed from May 2015 to July 2020. The results of this interdisciplinary diagnostic assessment were systematically evaluated and the clinical characteristics of the persons with or without FASD were analysed. RESULTS: Approximately 70% of the population…

The Strengths and Positive Influences of Children With Fetal Alcohol Spectrum Disorders.

People with disabilities have not been adequately represented in strengths-based research. This study is the first to examine strengths and positive influences of young children with fetal alcohol spectrum disorder (FASD). Thirty adoptive and relative caregivers of children with FASD reported their children's strengths and positive influences and completed measures on family functioning. Using a conversion mixed design, we described themes in strengths and influences, degree of caregiver positivity and relationships with child and family functioning. Caregivers reported wide-ranging strengths

Murine Models for the Study of Fetal Alcohol Spectrum Disorders: An Overview.

Prenatal alcohol exposure is associated to different physical, behavioral, cognitive, and neurological impairments collectively known as fetal alcohol spectrum disorder. The underlying mechanisms of ethanol toxicity are not completely understood. Experimental studies during human pregnancy to identify new diagnostic biomarkers are difficult to carry out beyond genetic or epigenetic analyses in biological matrices. Therefore, animal models are a useful tool to study the teratogenic effects of alcohol on the central nervous system and analyze the benefits of promising therapies. Animal models of

The Prevalence of Fetal Alcohol Spectrum Disorders in An American Indian Community.

The prevalence of fetal alcohol spectrum disorders (FASD) differs among populations and is largely unknown among minority populations. Prevalence and characterization of FASD is necessary for prevention efforts and allocation of resources for treatment and support. However, prevalence data are lacking, including among many minority populations. The aim of this study was to obtain an FASD prevalence estimate in a Southern California American Indian community employing active case-ascertainment. In 2016, American Indian children aged 5-7 years and their caregivers were recruited in collaboration…

Fetal alcohol spectrum disorder: neurodevelopmentally and behaviorally indistinguishable from other neurodevelopmental disorders.

BACKGROUND: The lack of universally accepted diagnostic criteria and the high rate of psychiatric comorbidity make it difficult to diagnose Fetal Alcohol Spectrum Disorder (FASD). In an effort to improve the diagnosis of FASD, the current study aimed to identify a neurodevelopmental profile that is both sensitive and specific to FASD. METHODS: A secondary analysis was conducted on data obtained from the Canadian component of the World Health Organization International Study on the Prevalence of FASD. Data on neurodevelopmental status and behavior were derived from a battery of standardized te

Review and gap analysis: molecular pathways leading to fetal alcohol spectrum disorders.

Alcohol exposure during pregnancy affects the development of the fetus in various ways and may lead to Fetal Alcohol Spectrum Disorders (FASD). FASD is one of the leading preventable forms of neurodevelopmental disorders. In the light of prevention and early intervention, knowledge on how ethanol exposure induces fetal damage is urgently needed. Besides direct ethanol and acetaldehyde toxicity, alcohol increases oxidative stress, and subsequent general effects (e.g., epigenetic imprinting, gene expression, and metabolite levels). The current review provides an overview of the existing knowledg…
