[Therapeutic effect of a mineralocorticoid in patients with hyponatremia of central origin].

Patients with hyponatremia of central origin were treated with a mineralocorticoid, and the therapeutic effect assessed. The subjects of this study were 14 patients (5 with subarachnoid hemorrhage, 2 with hypertensive intracerebral hemorrhage and 7 with head injury) who developed hyponatremia as a complication during their hospital stay for treatment of their intracranial lesions between March 1993 and April 1994. Hyponatremia of central origin was defined as serum Na+ levels below 135 mEq/l for at least two consecutive days with no other discernible etiology. The mineralocorticoid used was fl

[Current opinion on primary aldosteronism].

In recent years, a greater interest has been focused on primary aldosteronism (PA), which shows a higher prevalence rate than previously thought. The consequences of PA are life threatening such as a refractory hypertension with serious cardiovascular damages. The evaluation of a suspected PA should follow a step-by-step approach (screening test, then confirmatory test and, in some cases, adrenal venous sampling). This protocol may seem tedious, but it allows an accurate etiologic diagnosis that leads to an appropriate therapy with better blood pressure control, improvement of quality of life, a

Relationships among urinary kallikrein, mineralocorticoids and human hypertensive disease.

Urinary kallikrein excretion is reduced in patients with hypertension of unknown etiology. In addition, the excretion of this renal, kinin-forming enzyme was found to be elevated in hypertensive patients with primary aldosteronism. Aldosterone regulates kallikrein excretion, as normal subjects show increased kallikrein excretion in response to a low sodium intake, high potassium intake, or the synthetic mineralocorticoid, fludrocortisone, whereas kallikrein excretion falls during treatment with spironolactone. The relationship between kallikrein excretion and aldosterone activity may directly

Successful Use of Octreotide for Refractory Postural Hypotension in a Patient With Parkinson's Disease and Heart Failure With Preserved Ejection Fraction.

Postural hypotension, particularly in patients with autonomic dysfunction secondary to Parkinson's disease, presents a complex therapeutic challenge, especially when standard treatments fail. We report the case of an elderly patient admitted to a UK district general hospital with worsening diarrhoea and profound postural hypotension. The patient's background included Parkinson's disease, heart failure with preserved ejection fraction (HFpEF), ulcerative colitis, type 2 diabetes mellitus, and other chronic comorbidities. Initial management involved standard pharmacological therapy with fludroco

Endocrine effects of long-term calcineurin inhibitor use in solid organ transplant recipients.

BACKGROUND: The calcineurin inhibitors (CNIs) ciclosporin and tacrolimus are cornerstone immunosuppressants in solid organ transplantation, yet calcineurin blockade in endocrine tissues produces characteristic metabolic sequelae. This review synthesizes evidence on CNI-related disturbances in glucose and lipid metabolism, mineral balance, bone, and neuroendocrine axes. RESULTS: Calcineurin inhibitors precipitate post-transplant diabetes mellitus by blunting β-cell insulin release and augmenting insulin resistance; tacrolimus is consistently more diabetogenic than ciclosporin. Weight gain and

Cardiac tamponade in a patient with autoimmune polyglandular syndrome type 2.

UNLABELLED: We describe a case of a 40-year-old woman who was admitted to the intensive care unit with a rapid onset of dyspnea and orthopnea. She presented progressive weakness, weight loss and secondary amenorrhea during last year, while intermittent fever was present for the last two months. Initial biochemical evaluation showed anemia, hyponatremia and increased C-reactive protein levels. Clinical and echocardiographic evaluation revealed cardiac tamponade, which was treated with pericardiocentesis. Pleural fluid samples were negative for malignancy, tuberculosis or bacterial infection. Ho

Treatment of orthostatic hypotension.

Orthostatic hypotension (OH) is a fall in blood pressure after assuming an upright position. Whereas asymptomatic patients usually need no treatment, the majority of symptomatic patients can be cured by avoidance of trigger mechanisms and the use of physical countermaneuvers and non-pharmacological interventions. Several pharmacological therapies are available and generally fludrocortisone and midodrine are the drugs of first choice. Recently, highly individualized therapy with ambulatory norepinephrine therapy was able to mobilize otherwise immobile patients.

L-DOPS therapy for refractory orthostatic hypotension in autoimmune autonomic neuropathy.

The authors report a 46-year-old woman with antibodies to the nicotinic acetylcholine receptor (NiAchR) of the autonomic ganglia. She presented with severe orthostatic intolerance refractory to treatment with midodrine, fludrocortisone, erythropoietin, vasopressin, salt, and fluid loading. Addition of L-threo-3,4-dihidroxyphenylserine (L-DOPS) substantially improved blood pressure and orthostatic tolerance. L-DOPS may benefit patients with severe orthostatic intolerance and be particularly effective in patients with ganglionic NiAchR antibodies.

Clinical autonomic neuropharmacology.

Autonomic failure can be divided into peripheral and central dysfunction on the basis of pharmacologic characteristics of the disorders. Low plasma NE levels, impaired neuronal uptake, and adrenergic receptor supersensitivity distinguish patients with PAF from those with MSA, who have defective baroreflex modulation of blood pressure and CNS neurotransmitter dysfunction. An increased understanding of neurotransmitter metabolism and function in patients with chronic autonomic failure has led to a variety of rational therapeutic approaches. Much progress has been achieved in the management of or

Addison's disease.

Addison's disease is an uncommon endocrine condition manifested by a variety of nonspecific symptoms, such as malaise, anorexia and nausea. Symptoms usually do not occur until most of the adrenal gland has been destroyed. Autoimmune disease has surpassed tuberculosis as the primary cause of Addison's disease. Nevertheless, tuberculosis still accounts for a significant proportion of cases. The rapid adrenocorticotropic hormone (ACTH) stimulation test is useful for identifying adrenal insufficiency. Maintenance therapy consists of hydrocortisone and fludrocortisone.

Time-dependent Potentiation of False Context Fear Memory Through Glucocorticoid Receptor Activation and Mineralocorticoid Receptor Inactivation.

BACKGROUND: Hypothalamic corticotropin-releasing factor (CRF) has been implicated in the formation of false contextual fear memory. Here, we examined the involvement of glucocorticoid (GR) and mineralocorticoid receptors (MR) in this process. METHODS: Adult male C57BL/6J mice were exposed to Context B, similar but distinct from Context A, 3 h (B-3 h) after electric foot shock (FS) in Context A, and re-exposed to Context B either 24 h (B-24 h) or 9 days (B-9 d) after FS in Context A. To assess the effect of B-3 h exposure on the specificity of original memory, freezing levels were also measure

Suspected Diphtheria Toxoid and Tetanus Toxoid (dTdap) Booster Vaccine-Induced Postural Orthostatic Tachycardia Syndrome (POTS).

We describe a case of a 40-year-old South Asian woman who presented with symptoms suggestive of postural orthostatic tachycardia syndrome (POTS) following a diphtheria toxoid and tetanus toxoid (dTdap) booster vaccination administered one week prior. The patient's POTS responded favorably to treatment with low-dose fludrocortisone and ivabradine. Clinicians should maintain a high index of suspicion for POTS as a possible vaccine adverse event (VAE) post-dTdap booster inoculation and be aware of appropriate management strategies.
