Management and treatment of elbow and forearm injuries.

Orthopedic injuries to the upper extremity are frequently seen in the emergency department (ED). The emergency medicine practitioner must be proficient in recognizing these injuries and their associated complications, and be able to provide appropriate orthopedic management. This article highlights the most frequent forearm and elbow injuries seen in the ED.

Infected nonunions of diaphyseal fractures of the forearm.

BACKGROUND: There are no reported series that specifically deal with repair of infected nonunions of the diaphysis of the forearm bones. We sought to determine whether a standardized treatment protocol we have utilized for 15 patients from 1989 to 2005 results in a high union rate, resolution of infection, and a good functional outcome. METHODS: The study cohort included nine male and six female patients who presented to a University hospital setting with an infected nonunion of the diaphysis of the radius or ulna. Every patient had a minimum of 2-year follow-up. The average patient age was 4…

Ultrasound examination of peripheral nerves in the forearm.

BACKGROUND AND OBJECTIVES: We examined in a volunteer population whether nerves in the forearm could be seen consistently using ultrasound imaging and whether this new information could have implications for the way we perform regional anesthesia of the median, radial, and ulnar nerves. METHODS: Eleven volunteers underwent ultrasound examination of both forearms. The median, ulnar, and radial nerves were followed and images were obtained at the elbow, proximal forearm, mid forearm, distal forearm and wrist levels. In addition the radial nerve was followed proximally to a point 5 cm above the …

Giant lipomatous tumours of the hand and forearm.

This study examines the presentation, management and outcomes of a series of 10 patients with giant lipomatous tumours (defined as greater than 5 cm diameter) of the hand and forearm who presented to our orthopaedic oncology service. All patients underwent local staging and were discussed at our multidisciplinary tumour meeting prior to definitive surgery. In all cases, neurovascular structures required mobilization in order to excise the tumour. Seven of the tumours were benign lipomas and one was a neural fibrolipoma. The other two were well differentiated lipoma-like liposarcomas/atypical l…

Diagnosis and treatment of longitudinal instability of the forearm.

Radio-ulnar dissociation can result from high-injury trauma that the compressive forces traverse the wrist forearm and elbow. This injury can be thought of as an "unhappy triad" of radial head fracture, triangular fibrocartilage complex failure, and a tear of the interosseous membrane. The radius is the primary stabilizer of the forearm with the forearm interosseous membrane enabling load sharing between the radius and the ulna. The central one-third of the interosseous membrane is 3 times stronger than the membranous portion and approaches the strength of the anterior cruciate ligament for de…

[Nerve entrapment syndrome of the elbow and forearm].

In this study we will discuss entrapment of the median, ulnair, radial and lateral antebrachial nerves of the elbow and the forearm. Compression of the nerves may occur when they traverse a tunnel and an incompatibility exists between the diameter of the tunnel and its contents (e.g. nerves, tendons,...). However, at the elbow and the forearm the nerves are also exposed to particularly dynamic compressions. This is due to anatomical relationships changing between the nerve and its surrounding muscles, tendons and aponevroses during the motion of flexion-extension of the elbow and the prono-sup…

Congenital vascular malformations in the hand and forearm.

PURPOSE: To review a single-center experience in the management of symptomatic congenital vascular malformations of the hand and forearm with special attention to embolotherapy. METHODS: A retrospective chart review was performed to identify patients with vascular malformations referred for arteriography and possible intervention between 1983 and 1998. Arteriography and venography were performed in all patients to differentiate between true high-flow arteriovenous malformations (AVM) and low-flow primary venous malformations (PVM). The clinical and radiological data, procedural results, and f…

[Trauma to the forearm with damage to the major arteries].

The authors made an analysis of surgical methods of treatment of 63 patients and long-term results in the period from 1 to 6 years in 23 of them. The restoration of all the injured anatomical structures of the forearm and, in particular, the restoration (suture, autovenous plasty) of the major arteries and primary suture of the injured nerves are thought to be necessary for good results.

Neurovascular problems in the forearm, wrist, and hand.

This article attempted to summarize the most common neurovascular injuries of the upper extremity, particularly the forearm, wrist, and hand. Although these injuries are rarely encountered in athletes, their pathology and treatment must be understood by the treating physician. Failure to recognize these injuries in a timely manner can lead to delay in diagnosis and weeks or months of lost participation by the athlete. The sports medicine physician must be aware of the potential risk for injury to the neurovascular structures, particularly in the athlete exposed to repetitive use or impact of t…

Intramedullary fixation of forearm fractures in adults.

The authors present a retrospective study of acute fractures of the diaphysis of the radius or ulna, or both, in adults treated by intramedullary nailing. Seventy diaphyseal fractures in 38 patients (30 men and 8 women) were treated by intramedullary fixation. The mean age of the patients was 31.5 years. Union occurred in 66 fractures (94%). The average union-time was 73 days. Compared with the results published by other authors, using the same evaluation criteria, union-time with the intramedullary technique was shorter than with other techniques. Union-rates and functional results were simil

A thinned forearm flap transfer to the nose.

We employed a forearm flap that had been thinned through primary defatting for nasal covering in three cases to reduce the need for secondary revision. Partial flap loss did occur in one case due to subcutaneous vascular plexus injury, but acceptable results were obtained in the remaining two cases. Donor site concave deformities were substantially reduced by removal of fat from around the flap margin. Great care must be taken during surgery to avoid damage to the small vascular network around the pedicle to prevent flap necrosis.

Musculo-tendinous transfers of the hand and forearm.

The concept of providing a more equitable distribution of forces acting upon a disabled limb is the basis of tendon transfers. The musculo-t … e extent of functional loss balanced against the muscle force available for transfer. Essential for forearm and hand function are wrist extension and finger flexion as well as thumb and intrinsic function. The loss of these functions may be compensated for by a variety of motors. [Fractures of a single bone of the forearm in children].

Primary cilia are sensory membrane protrusions whose dysfunction causes ciliopathies. INPP5E is a ciliary phosphoinositide phosphatase mutated in ciliopathies like Joubert syndrome. INPP5E regulates numerous ciliary functions, but how it accumulates in cilia remains poorly understood. Herein, we show INPP5E ciliary targeting requires its folded catalytic domain and is controlled by four conserved ciliary localization signals (CLSs): LLxPIR motif (CLS1), W383 (CLS2), FDRxLYL motif (CLS3) and CaaX box (CLS4). We answer two long-standing questions in the field. First, partial CLS1-CLS4 redundancy

Forearm flap in orthopaedic and hand surgery.

This study was undertaken in order to evaluate the usefulness of the forearm flap in reconstruction of severe injuries of the upper and lower extremities. A total of 34 patients with extensive skin defects of the upper and lower extremities were treated using radial forearm flaps during the last 4 years. Twenty-four patients had lower extremity injuries, while the remaining ten had upper extremity reconstruction. In two patients, the radial forearm flap was used as an island flap in retrograde direction for coverage of skin defects of the dorsum of the hand. In two patients, the palmaris longu…

Subclinical injuries in lacerations to the forearm and hand.

This report describes the incidence and severity of subclinical injuries to underlying structures in lacerations to the hand and forearm. One hundred consecutive hand and forearm lacerations that penetrated the full thickness of subcutaneous tissue were studied prospectively. Lacerations were explored under either biceps or forearm tourniquets. Injuries, treatment, tourniquet time, causative agent and complications were recorded. In all, 97 patients sustained 100 lacerations. A total of 49 deep injuries were discovered, none of which was detected clinically before exploration. Of these, 33 wer…

[Late results of forearm fractures in childhood].

At the Department of Pediatric Surgery, University of Munich, 1143 children up to 14 years of age were treated for forearm fractures between 1976 and 1985. Two-thirds of distal metaphyseal fractures affect only the radius, but diaphyseal fractures involve both bones in two-thirds to three-quarters of cases. In 97.8% of the cases, conservative therapy was administered. Follow-up examination was performed 3-13.5 years after treatment in 144 patients (including all with complications resulting from the fracture itself, the therapy or the healing). There were 32 who reported functional restriction…
