Imaging modalities in wounds and superficial skin infections.

Open wounds and lacerations are the most commonly encountered problems in emergency medicine. Detection and removal of a foreign body is essential to avoid the many complications of a retained foreign body, which may include infection, inflammation, allergic reaction, and disability. Currently, there are several imaging modalities that the emergency medicine provider may use to aid in foreign body detection and wound management. Techniques and appropriate use of these imaging modalities for foreign bodies and soft tissue infections are discussed in this article.

Late-Onset Infection of Resorbable Plates After Multiple Facial Fractures.

In recent years, a variety of resorbable plates have been used for craniofacial fractures. The authors report a case of plate infection that occurred more than 1 year after surgery and was difficult to distinguish from a foreign body reaction. A 19-year-old male suffered fractures of the right zygomatic bone, orbital floor, and left maxilla in a motorcycle accident. Reduction was performed using resorbable plates at 7 days after injury. The postoperative course was good. However, the patient presented 396 days after surgery with redness/swelling of the right upper eyelid and right cheek pain.

Clinical biocompatibility of biodegradable orthopaedic implants for internal fixation: a review.

When the polymeric material reaches the final stages of its degradation process, biodegradable orthopaedic fixation devices elicit a local foreign-body reaction. In most cases, the symptoms of this tissue response are subclinical and pass unnoticed, but in some patients a clinically manifest inflammatory foreign-body reaction ensues. Mild clinical reactions consist of a painful erythematous papule, those of medium severity show a sinus discharging polymeric debris for up to 6 months, and in the patients affected by a severe reaction, extensive osteolytic lesions may develop at the implant trac…

Chronic otorrhea caused by myospherulosis in the middle ear after tympanoplasty.

OBJECTIVES: Myospherulosis is a foreign body reaction induced by the application of oil-based ointments. Myospherulosis in the ear is extremely rare. Only 4 cases have been described, all of which occurred after (repeated) mastoid surgery. METHODS: We present a case of persistent otorrhea and conductive hearing loss caused by myospherulosis in the middle ear following tympanoplasty. RESULTS: The patient underwent revision middle ear surgery with removal of abnormally thick, pale tissue in the middle ear. Histology showed a foreign body reaction with signs of myospherulosis. CONCLUSIONS: Myo

Exploring the Overlooked Roles and Mechanisms of Fibroblasts in the Foreign Body Response.

Significance: Foreign body response (FBR), wherein a fibrotic capsule forms around an implanted structure, is a common surgical complication that often leads to pain, discomfort, and eventual revision surgeries. Although believed to have some mechanistic overlap with normal wound healing, much remains to be discovered about the specific mechanism by which this occurs. Recent Advances: Current understanding of FBR has focused on the roles of the immune system and the biomaterial, both major contributors to FBR. However, another key player, the fibroblast, is often overlooked. This review summar

Lip augmentation dermal filler reactions, histopathologic features.

Instances of perioral and labial foreign body reactions to a variety of injectable dermal fillers were selected from the oral and maxillofacial pathology and dermatopathology archives at Pacific Pathology Laboratory of San Diego with the objective being to engender a compilation of histopathologic characteristics that allow the pathologist to identify the inciting materials. All cases of foreign body reactions located in the lips and perioral regions were reviewed by four pathologists, retaining those cases with a history of injection lip augmentation as well as those with histologic features

Migration of a wooden foreign body into the lens: report of a case.

A penetrating ocular injury caused by a tree branch resulted in a small corneoscleral laceration that healed after primary repair. Post-traumatic inflammation persisted and a vitreous mass developed. The mass finally protruded through the pupil and touched the posterior cornea. Histologic examination of the enucleated eye showed that the lens had been replaced by a wooden foreign body that filled the pupillary space and was surrounded by lens capsule. No intact lens fibers remained.

Case report: late complication of a dry socket treatment.

Dry socket is often treated in dentistry with intra-alveolar dressings; the use of them remains controversial and has been related to some side effects such as neuritis, foreign body reactions, and myospherulosis. We present a case of an intra-alveolar dressing (zinc-oxide eugenol paste) that mimicked a trigeminal neuralgia for 3 years and caused a right maxillary chronic osteomyelitis and foreign body reaction in a zone corresponding to the alveolus of the maxillary first molar. This long-term complication was successfully managed by complete removal of the foreign body and curettage of the a

F-18 FDG PET of foreign body granuloma: pathologic correlation with imaging features in 3 cases.

PURPOSE: Retained textiles with foreign body granulomatous reaction can form mass-mimicking soft tissue tumors, which may cause diagnostic, clinical, and medicolegal problems. In this study, we present 3 cases of foreign body granuloma by retained textiles, and demonstrate FDG PET findings with detailed pathologic correlation. We also provide a review of previous literatures. MATERIALS AND METHODS: Three patients (all females; age range, 48-61 years) with histopathologic diagnosis of mass-forming foreign body granuloma caused by retained textiles were retrospectively studied. They underwent a

Brass intraocular foreign body: early intervention and combined surgical approach.

The rapid development of suppurative reaction in response to a copper-containing intraocular foreign body disposes the eye to a poor prognosis. An intraocular brass foreign body case involving cornea, iris, lens, vitreous and retina was satisfactorily managed by early foreign body removal and corneal transplantation followed by scleral buckling for retinal detachment. Early intervention is advocated as alternative management with expectation of improvement in the prognosis for recovery of useful visual function.

Normal and pathologic tissue reactions to soft tissue gel fillers.

BACKGROUND: All gels act as foreign bodies. Host response ranges from a few macrophages to an intense foreign-body reaction with fibrosis, depending on gel type. For polymer gels the filling effect stems from their volume. For combination gels it stems from the intended host foreign-body reaction to the microparticles. OBJECTIVE: This review explains how host tissue reacts with different gel types and how adverse reactions differ depending on gel type. METHODS AND MATERIALS: This study is based on observations during the past 6 1/2 years from experimental/clinical studies and biopsies from a…

Removal of a metallic foreign body embedded in the external nose via open rhinoplasty approach.

Most metallic foreign bodies are inert, but they can cause chronic inflammatory reactions and be a source of infection. Identification and removal of foreign bodies from wounds is often necessary. The present report describes two cases of a foreign body embedded in the external nose. Each case was successfully treated by an open rhinoplasty approach. This approach is an effective and safe method for removal of foreign bodies in the external nose. It provides a good surgical field and a better cosmetic outcome than a conventional incision.

Foreign body granuloma mimicking pituitary adenoma: Case report and literature review.

BACKGROUND: Foreign body granuloma of the pituitary gland is an exceptionally rare inflammatory lesion that may mimic recurrent adenoma on neuroimaging. CASE DESCRIPTION: We report the case of a 44-year-old woman who previously underwent endonasal endoscopic surgery for a nonfunctioning pituitary adenoma and later developed a sellar mass secondary to a foreign body granulomatous reaction. Magnetic resonance imaging suggested tumor recurrence; however, gadolinium-enhanced imaging was not performed because of the patient's known gadolinium allergy. Histopathological examination revealed xanthog

A comparative histologic study on the healing process after tissue transection. I. Carbon dioxide laser and electromicrosurgery.

The present study was undertaken to evaluate and compare histologic parameters of wound healing subsequent to a standard uterine incision with carbon dioxide laser and electromicrosurgery in the rat model. The following histologic parameters were assessed and scored: scar width; amount of particulate carbon and necrotic debris; number of polymorphonuclear leukocytes, lymphocytes, histiocytes, and giant cells; and extent of edema, exudate, and collagen in the scar. The incisions were evaluated on postoperative days 0, 4, 7, 14, and 21. The ensuing scars were similar with respect to scar width a

Unusual foreign bodies in the orofacial soft tissue spaces: a report of three cases.

The ingestion and inhalation of foreign bodies with their associated morbidity and mortality have been widely reported in the literature. However, reports on foreign body impaction in the orofacial region are comparatively fewer. Three unusual cases of foreign body impaction into the buccal and submasseteric spaces are presented. The three patients were adults and they presented with history of painful facial swelling associated with limited mouth opening. Plain skull radiographs revealed the foreign bodies within the facial soft tissue spaces in two patients while it was not helpful in the th

Rare foreign bodies in oro-facial regions.

Accidental entry of foreign bodies into the oro-facial region could be due to trauma, therapeutic interventions or iatrogenic. Various foreign bodies and locations have been reported, for example, wood in the orbit, impression material in the maxillary sinus, tooth fragments in the orbit. All these cases presented with inflammatory reaction and formation of infected granuloma, pus discharging sinus and serious complications like intra-cranial abscesses. Foreign bodies sometimes migrate within the tissues and become symptomatic after a certain period of time. In these cases, it is very difficul

Pediatric Hand Cyst with Metacarpal Bone Changes: An Embedded Tree Thorn Case.

INTRODUCTION: Retained foreign bodies in pediatric hand injuries are rare but can lead to significant complications if undiagnosed. Organic foreign bodies, such as tree thorns, are often radiolucent and may not be detected on plain radiographs, making diagnosis challenging. This case highlights the importance of considering a retained foreign body in a child presenting with a persistent hand mass and associated bony changes. CASE REPORT: A 3-year-old male presented with a painless, slow-growing mass over the dorsal ulnar aspect of his left hand for 8 weeks. Clinical examination revealed a flu
