Right Ventricular Strain With 4-Dimensional Computed Tomography Identifies Pulmonary Hypertension in Adults With Repaired Tetralogy of Fallot.

BACKGROUND: This study evaluated right ventricular (RV) volume, strain, and morphology using cardiac 4-dimensional computed tomography (4D-CT) to detect pulmonary hypertension (PH) in adults with repaired tetralogy of Fallot (TOF) scheduled for transcatheter pulmonary valve implantation (TPVI). METHODS AND RESULTS: Using cardiac 4D-CT data, we calculated RV strain in 3 different geometries and RV outflow tract (RVOT) mass in 42 patients with repaired TOF. We compared RV strain and RVOT mass between patients with and without PH. Receiver operating characteristic (ROC) analysis was conducted to

Four-Dimensional Thoracic CT in Free-Breathing Children.

In pediatric thoracic CT, respiratory motion is generally treated as a motion artifact degrading the image quality. Conversely, respiratory motion in the thorax can be used to answer important clinical questions, that cannot be assessed adequately via conventional static thoracic CT, by utilizing four-dimensional (4D) CT. However, clinical experiences of 4D thoracic CT are quite limited. In order to use 4D thoracic CT properly, imagers should understand imaging techniques, radiation dose optimization methods, and normal as well as typical abnormal imaging appearances. In this article, the imag

Usefulness of four-dimensional computed tomographic angiography in a vertebral artery dissection case.

Discrimination of intramural hematoma from intra-aneurysmal blood flow is a critical issue for determining the necessity of treatment in vertebral artery dissection (VAD) cases. We describe modified four-dimensional computed tomographic angiography (4D-CTA) that is useful for evaluating blood flow in a dissected vertebral artery in a 41-year-old man who was diagnosed as VAD. Magnetic resonance angiography (MRA) showed a growth of intramural hematoma, which was confusingly similar to intra-aneurysmal blood flow. Modified 4D-CTA enabled us to discriminate the intramural hematoma from blood flow

Dynamic CT for parathyroid disease: are multiple phases necessary?

BACKGROUND AND PURPOSE: A 4D CT protocol for detection of parathyroid lesions involves obtaining unenhanced, arterial, early, and delayed venous phase images. The aim of the study was to determine the ideal combination of phases that would minimize radiation dose without sacrificing diagnostic accuracy. MATERIALS AND METHODS: With institutional review board approval, the records of 29 patients with primary hyperparathyroidism who had undergone surgical exploration were reviewed. Four neuroradiologists who were blinded to the surgical outcome reviewed the imaging studies in 5 combinations (une

Measurement of regional compliance using 4DCT images for assessment of radiation treatment.

PURPOSE: Radiation-induced damage, such as inflammation and fibrosis, can compromise ventilation capability of local functional units (alveoli) of the lung. Ventilation function as measured with ventilation images, however, is often complicated by the underlying mechanical variations. The purpose of this study is to present a 4DCT-based method to measure the regional ventilation capability, namely, regional compliance, for the evaluation of radiation-induced lung damage. METHODS: Six 4DCT images were investigated in this study: One previously used in the generation of a POPI model and the oth

Multimodality Evaluation of Persistent Hyperparathyroidism in a Rare Case of Ectopic Submandibular Parathyroid Adenoma.

A 53-year-old woman of primary hyperparathyroidism with both ultrasound neck examination and planar 99m Tc-MIBI scan being negative revealed a tracer-avid focus in the left submandibular region in early (15 minutes postinjection) 99m Tc-MIBI SPECT/CT views, raising a suspicion of rare ectopic parathyroid adenoma. This finding was correlated on 4D-CT and confirmed on histopathology following surgical excision. Submandibular region is an unusual location for ectopic parathyroid adenoma. Nevertheless, high degree of suspicion with utilization of multimodality imaging including 99m Tc MIBI-SPECT/C

Markerless tracking of tumor and tissues: A motion model approach.

BACKGROUND: Respiratory motion management is essential in order to achieve high-precision radiotherapy. Markerless motion tracking of tumor can provide a non-invasive way to manage respiratory motion, thereby enhancing treatment accuracy. However, the low contrast in real-time x-ray images for image guidance limits the application of markerless tracking. PURPOSE: We present a novel approach based on a motion model to perform markerless tracking of tumor and surrounding tissues even when they have low contrast in real-time x-ray images. METHODS: A proof-of-concept validation of the method has

Circle of Willis Collateral Flow in Carotid Artery Occlusion Is Depicted by 4D-CTA.

BACKGROUND: In case of carotid artery occlusion, the risk and extent of ischemic cerebral damage are highly dependent on the pathways of collateral flow including the anatomy of the circle of Willis. In this report, cases are presented to illustrate that 4-dimensional computed tomography angiography (4D-CTA) can be considered as a noninvasive alternative to digital subtraction angiography for the evaluation of circle of Willis collateral flow. CASE DESCRIPTION: Five patients with unilateral internal carotid artery (ICA) occlusion underwent 4D-CTA for the evaluation of intracranial hemodynamic

A Cadaveric Study of the Arterial Blood Supply of the Labia Minora.

BACKGROUND: Labiaplasty has become a rather common operation. The main complication of the different surgical techniques is the dehiscence of the suture line. The dehiscence rate varies among the different techniques, and this may imply that the vascular anatomy is not respected in some cases. The detailed arterial anatomy of labia minora is not well described, so the aim of the authors was to describe this anatomy with a cadaveric study. METHODS: Eleven fresh cadavers were dissected, and arterial study was made with injected computed tomography scans and rotational angiography. At the end, a

Clinical necessity of multi-image based (4DMIB) optimization for targets affected by respiratory motion and treated with scanned particle therapy - A comprehensive review.

4D multi-image-based (4DMIB) optimization is a form of robust optimization where different uncertainty scenarios, due to anatomy variations, are considered via multiple image sets (e.g., 4DCT). In this review, we focused on providing an overview of different 4DMIB optimization implementations, introduced various frameworks to evaluate the robustness of scanned particle therapy affected by breathing motion and summarized the existing evidence on the necessity of using 4DMIB optimization clinically. Expected potential benefits of 4DMIB optimization include more robust and/or interplay-effect-res

Combining 4DCT and [18F]choline PET/CT as first-line imaging in primary hyperparathyroidism patients; a one-stop shop.

BACKGROUND: Primary hyperparathyroidism (PHPT) is an endocrine disorder characterized by excessive parathyroid hormone secretion, typically due to a solitary parathyroid adenoma (PA). Accurate preoperative localization is crucial for successful minimal invasive surgical management. Four-dimensional computed tomography (4DCT) is increasingly used as a first-line imaging modality due to its superior sensitivity and specificity compared to ultrasound and [99mTc]Sestamibi SPECT. Recent studies have highlighted the potential role of [18F]Choline PET/CT in PA detection. This study evaluates the feas…

Diagnostic ability of computed tomography using DentaScan software in endodontics: case reports.

Radiographic examination is essential in diagnosis and treatment planning in endodontics. Conventional radiographs depict structures in two dimensions only. The ability to assess the area of interest in three dimensions is advantageous. Computed tomography is an imaging technique which produces three-dimensional images of an object by taking a series of two-dimensional sectional X-ray images. DentaScan is a computed tomography software program that allows the mandible and maxilla to be imaged in three planes: axial, panoramic, and cross-sectional. As computed tomography is used in endodontics,

Technical Note: Correction for the effect of breathing variations in CT pulmonary ventilation imaging.

PURPOSE: The accuracy and precision of computed tomography (CT) pulmonary ventilation imaging with conventional CT scanners are limited by breathing variations. We propose a method to correct for the effect of breathing variations in CT ventilation imaging based on external respiratory signals acquired throughout a scan. METHODS: The proposed method is based on: (a) calculating voxel-by-voxel abdominal surface motion ranges using four-dimensional (4D) CT image datasets spatiotemporally correlated with external respiratory monitor data, and (b) applying the correction factor, which is defined
