[Volar transscaphoid perilunate fracture dislocation. A case report].

Volar perilunar dislocations of the carpus are rare. They occur in young patients following a violent injury. Open reduction is often needed because of instability of the lesion. The authors report a case of volar transscaphoid perilunate fracture-dislocation treated surgically, with a satisfactory functional result after a follow-up of two and a half years.

Complete dislocation of the talus unaccompanied by fracture.

Total dislocation of the talus without a concomitant fracture is an extremely rare injury. Such a case was treated by immediate closed reduction and 6 weeks of cast immobilization. At the 9-year follow-up, the patient had a full range of ankle motion without any signs of avascular necrosis or arthritis.

Purtscher's retinopathy after fracture dislocation of shoulder joint.

Purtscher described sudden blindness in patients with severe head trauma due to a remote retinopathy, characterised by bilateral retinal haemorrhages, cotton wool spots, and optic disc swelling seen on fundoscopy. A similar retinopathy has been reported in compressive chest trauma, long bone fractures, and acute pancreatitis. It is less well recognised that Purtscher's retinopathy can occur unilaterally and following less severe trauma. We present a case of unilateral remote traumatic retinal angiopathy following a fracture dislocation of the shoulder joint.

Galeazzi fracture-dislocation: a new treatment-oriented classification.

Forty patients with Galeazzi fracture-dislocations were treated with open reduction and internal fixation of the radial shaft fracture. Intraoperative distal radioulnar joint (DRUJ) instability after anatomic reduction was managed with supplemental wire transfixion of the DRUJ (10 patients) or open reduction and triangular fibrocartilage complex repair (3 patients). Two patterns of fracture-dislocation were identified based on the location of the radial shaft fracture. Twenty-two type I fractures were in the distal third of the radius within 7.5 cm of the midarticular surface of the distal rad

Palmar fracture dislocation of the proximal interphalangeal joint.

Palmar fracture dislocation of the proximal interphalangeal joint is uncommon. Thirteen patients treated for this injury were retrospectively reviewed. There were 9 acute injuries. Seven were treated by closed reduction and percutaneous pin fixation and 2 were treated by open reduction and internal fixation. The 4 chronic injuries (more than 1 month after injury) were treated with open reduction and soft tissue reconstruction. The length of follow-up averaged 55 months. Eight patients were free from pain. Postoperative proximal interphalangeal motion averaged 91 for the acute injuries and 70 d…

[Hip dislocation without fracture--a trauma surgery emergency].

Between 1968 and 1990, 26 patients (medium age 24.2 years) with a traumatic hip dislocation without fracture were treated at the Dept. of Trauma Surgery of the University Erlangen-Nuremberg. Only 26.9% of the patients had no further injuries beside the hip dislocation. An early closed reduction was carried out whenever possible. In 96.0% of the cases, radiological acceptable results have been achieved with the closed reduction. In 4.0% an open reduction became necessary. After a period of three to four weeks of extension, the patients had been mobilized to nonweight bearing status. Stress on t

[MRI-controlled rehabilitation of femur head dislocation fracture].

As exemplified by the case of a 58-year old patient with a femoral head fracture dislocation, which could not be reduced for 15 hours, the early stage of femoral head revitalisation is well documented by computed tomography before and after reduction, as well as MR imaging after 6 weeks, 6 months and one year.

Isolated dorsal fracture-dislocation of the tarsal navicular.

Two cases of isolated fracture and dorsal dislocation of the tarsal navicular are reported. Analysis of the mechanism of injury is made, and it is suggested that the dislocation of the tarsal navicular does not occur without a fracture because of anatomical considerations. The necessity for accurate reduction and internal fixation to hold the fracture is emphasized which, when healed, maintains the reduction. The possibility of orthotic treatment for postfracture morbidity is mentioned. Late pain may occur due to degenerative disease affecting the navicular articulations.

Factors associated with unplanned reoperation in perilunate dislocations and fracture dislocations.

Background Perilunate injuries are complex and uncommon injuries that are typically the result of a high-energy injury and are nearly always treated operatively. Little is known about factors associated with unplanned reoperations after surgery for perilunate injuries. Purpose To assess the rate and types of unplanned reoperation after operative treatment of a perilunate dislocation. Patients and Methods We reviewed 115 patients of all ages with unplanned reoperations after operative treatment of perilunate injuries at five hospitals. Planned removal of implants were not considered as unplanne

[Perilunate dislocations and fracture-dislocations of the wrist, a review of 17 cases].

INTRODUCTION: The authors present the results of a retrospective series of 17 cases of dislocations or perilunate fracture-dislocations of the carpus, treated in emergency (period of less than 7 days). The objective aim of our study was to evaluate the radioclinical fate prognosis of these lesions pathologies and their social commitment impact. PATIENTS AND METHODS: It is a retrospective study covering the period from July 2004 to December 2009 (or 54 months). Were included in the study, patients hospitalised for a pure dislocation or a perilunate fracture-dislocation authenticated confirmed

[Perilunar dislocations and dislocation fractures in polytrauma patients--diagnosis and therapy].

Between 1978 and 1987 about 700 multiply injured patients were treated at the Department of Emergency Surgery (University Hospital Hamburg-Eppendorf). Seven of these suffered from injuries of the wrist, in detail six perilunar dislocations with fractures of the scaphoid and one dislocation of the triquetrum. In six cases the correct diagnosis was found primarily by clinical and X-ray examinations. The real extent of one patient's wrist injury was revealed by control radiograms which were taken twelve weeks after the accident, when the patient was transferred to our hospital. Follow-up examinat

Traumatic dislocation and fracture-dislocation of the hip: a long-term follow-up study.

OBJECTIVE: Traumatic dislocation and fracture-dislocation of the hip is an absolute orthopedic emergency that is steadily increasing in incidence. Early recognition and prompt, stable reduction is the essence of successful management. A delay in recognition and reduction leads to preventable complications and morbidity. The purpose of this retrospective study is to identify prognostic factors that predict long-term outcome after hip dislocation. METHODS: Between 1980 and 1994, 107 patients with traumatic dislocation of the hips were treated, and 62 are reviewed in this study. There were 57 po

Ipsilateral fracture dislocation of the radial shaft head associated with elbow dislocation: case report.

A severe form of an extremely unusual injury, ipsilateral fracture dislocation of the radial shaft head associated with elbow dislocation, is described. Radial shaft fracture was irreducible even after the radial head and the elbow were reduced. Open reduction and internal fixation are viable options.

Traumatic dislocation and fracture dislocation of the hip. A long-term follow-up study.

Of 84 fracture dislocations of the hip reviewed, 73 were posterior fracture dislocations, nine were central, and two were obturator. Fifty-five were followed from one to 30 years (average, 4 1/2 years). The patients' ages ranged from three to 83 years (average, 33.1 years). All had primary attempt at closed reduction, with 15 treated with subsequent open reduction (six immediate and nine delayed). Satisfactory results were obtained in 42% of hips treated with closed reduction and 40% of those treated with open reduction. Statistical analysis of the results was done. A stable closed reduction p…

Perilunate dislocation and fracture dislocation of the wrist: Outcomes and long-term prognostic factors.

INTRODUCTION: Perilunate dislocations and fracture-dislocations are severe injuries that often have serious functional sequelae. Our goal was to evaluate the long-term clinical and radiological results of these perilunate injuries, and to look for prognostic factors of a poor clinical outcome. HYPOTHESIS: All patients who suffered perilunate injuries in their wrist have functional sequelae and long-term radiographic changes despite optimal treatment with anatomical surgical reduction. MATERIALS AND METHODS: We did a single-center, retrospective study of 32 patients who had either an isolated
