Treatment of pediatric lateral condylar humerus fractures with closed reduction and percutaneous pinning.

BACKGROUND: Lateral condylar humerus fractures (LCHFs) are the second most common pediatric distal humerus fractures. Open reduction and internal fixation is recommended for fractures displaced by more than 2 mm. Few studies described using closed reduction and percutaneous pinning (CRPP) for treating fractures with greater displacements. This study aims to explore the feasibility of CRPP in treating displaced LCHFs. METHODS: All patients underwent attempted CRPP first. Once a satisfying reduction was obtained, as determined using fluoroscopy based on the relative anatomical position of the f

Do Emergency Medicine Residency Graduates Feel Prepared to Manage Closed Fractures After Training?

OBJECTIVES: Fractures comprise 3% of all emergency department (ED) visits. Although emergency physicians are often responsible for managing most of the initial care of these patients, many report a lack of proficiency and comfort with these skills. The primary objective was to assess how prepared recent emergency medicine (EM) residency graduates felt managing closed fractures upon completion of residency. Secondary objectives included whether residency training or independent practice contributed most to the current level of comfort with these procedures and which fractures were most commonly

Three-dimensional evaluation of healing joint morphology after closed treatment of condylar fractures.

Closed treatment for condylar fractures has long been widely accepted. With closed treatment, the deviated bone fragments heal in their new positions, and this may subsequently cause a range of functional impairments. The association between healing morphology and post-treatment functional impairment is unclear. In this study, computed tomography images of 26 patients (35 sides) who had undergone closed treatment for condylar fractures were used to perform a comparative investigation of three-dimensional (3D) bone morphology before and after treatment. As a result, the morphology of the condyl

Closed reduction of mandibular condyle fractures using C-arm fluoroscopy: a technical note.

We describe a C-arm technique for mandibular condylar fractures in an anatomic study using a model skull and show its feasibility in a clinical case. The C-arm allowed posterior-anterior visualization of the condylar process. The X-ray axis was canted ∼15 degrees cranially to the Frankfort horizontal line. The skull's sagittal plane was rotated ∼15 degrees ipsilaterally to the X-ray axis. This technique facilitates clear visualization of the condylar neck with easy, flexible, and timely adjustments. In selected cases, this method would convert the clinical settings of the condylar fracture pat

Results of Closed Intramedullary Nailing using Talwarkar Square Nail in Adult Forearm Fractures.

ABSTRACT: The aim of the study was to evaluate results of closed intramedullary nailing using Talwarkar square nails in adult forearm fractures. We prospectively evaluated 34 patients with both bone forearm fractures. The average time to union was 12.8 (SD +3.2) weeks with cast support for a mean of 8.2 weeks. Union was achieved in 31 out of 34 patients. Using the Grace and Eversmann rating system, 17 patients were excellent, 10 were good, and 4 had an acceptable result. Three patients had non-unions, 2 for the radius and one for the ulna. There were two cases of superficial infection, one sub

Closed reduction and percutaneous cannulated screws fixation of displaced intra-articular calcaneus fractures.

BACKGROUND: Displaced intra-articular calcaneal fractures remain a therapeutic challenge due to fracture complexity and different treatment options. One of the adverse effects of operative treatment is secondary damage to soft tissues. To avoid soft tissue complications, several less invasive procedures have been introduced. The most frequently used minimally invasive technique is closed reduction of fracture and percutaneous cannulated screws fixation. METHOD: This study evaluates the medium-term outcome of a new technique of percutaneous treatment in 60 cases operated in Al-Razi orthopedic

Antibiotic prophylaxis for surgery for proximal femoral and other closed long bone fractures.

BACKGROUND: Surgical site infection and other hospital-acquired infections cause significant morbidity after internal fixation of fractures. The administration of antibiotics may reduce the frequency of infections. OBJECTIVES: To determine whether the prophylactic administration of antibiotics in people undergoing surgical management of hip or other closed long bone fractures reduces the incidence of surgical site and other hospital-acquired infections. SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (December 2009), the Cochrane Central Reg

Function impairment and pain after closed treatment of fractures of the mandibular condyle.

BACKGROUND: To determine the prognosis of fractures of the mandibular condyle after closed treatment. METHODS: Patients (n = 144) with a fracture of the mandibular condyle, all treated closed, were included in the study. Fracture types and position of the fracture parts were determined on radiographs. Follow-up was after 12 months in which the average pain, experienced during the last week (visual analog scale, 100 mm), and mandibular functioning were assessed (mandibular function impairment questionnaire (MFIQ)). RESULTS: Data of 116 (81%) patients, 41 women (35%) and 75 men (65%), were ava

Closed reduction and percutaneous stabilization of pediatric T-condylar fractures of the humerus.

T-condylar fractures of the humerus are a rare fracture pattern in children. The usual recommendation is to treat them with open reduction and internal fixation, in accordance to the usual practice in adult fracture patterns. This involves extensive surgical approach to the elbow to allow anatomic reduction and placement of hardware for rigid fixation. The authors present a technique of closed reduction of the intra-articular component of the T-condylar fracture that is stabilized with partially threaded pins that afford interfragmentary compression, followed by the use of two elastic titanium

Displaced supracondylar humeral fractures in children: open reduction vs. closed reduction and pinning.

INTRODUCTION: In the literature the best results for pediatric supracondylar humerus fractures have been achieved by closed reduction and wire fixation. However, in these reports the patient group of open reduction and pinning contained the patients who had had previous ineffective closed reduction trials. This retrospective study compared open and closed reduction with pinning, in which the first group of patients was all consecutively treated with open reduction. MATERIALS AND METHODS: The study included 99 children with displaced extension-type supracondylar fractures of humerus who had co

Functional results of unilateral mandibular condylar process fractures after open and closed treatment.

BACKGROUND: This retrospective study compared the functional results of unilateral mandibular condylar process fractures treated either by open reduction or by closed treatment. METHODS: Sixty-six patients with unilateral mandibular condylar process fractures were reviewed. Thirty-six patients received open reduction, and the other 30 underwent closed treatment (intermaxillary fixation only). Each group was further divided into condylar and subcondylar subgroups according to fracture level. The functional outcome was evaluated by posttreatment occlusion status, maximal mouth opening, facial s

Compression plating versus hackethal nailing in closed humeral shaft fractures failing nonoperative reduction.

Prospectively, 40 patients with an average age of 45 (20-65) with closed transverse fractures of the middle one-third of the humerus without associated radial nerve palsy were treated. All had failed nonoperative reduction and were then operated on with either compression plating or intramedullary fixation with Hackethal nails and a postoperative ready-made fracture brace. These surgeries were performed at an average of 7 days (4-10) after closed reduction. The Hackethal nailing group consisted of 20 patients (17 men, 3 women) with a mean age of 47 years (22-64); within 2 days after surgery th

[Surgical treatment of closed articular fractures of the proximal interphalangeal joints (20 cases)].

20 cases of intra-articular fractures of proximal interphalangeal joints were reviewed with a mean follow-up of 3 years 9 months, and a minimum follow-up of 1 year, corresponding to 10 fractures of the head of P1 (7 condylar fractures, 2 supra-intercondylar fractures and 1 diaphyseal fracture with an articular crevice) and 10 fractures of the basis of P2 corresponding to distraction-impaction fractures in 8 cases. The fractures were treated by open reduction and osteosynthesis by screws, mini-screws, or pins, completed by an arthrodesis pin in 3 cases. All fractures were closed, isolated, and

[Is surgical treatment of fresh, closed humeral fractures justifiable as a routine procedure?].

Excellent results were found to be achievable from conservative functional treatment of upper arm fractures. A critical appraisal is made in this paper of the authors' own cases and of literature published over the past 25 years for comparison of the above results with surgical therapy. The conservative functional approach was found to be the optional method to cope with closed upper arm fractures, since it provides all the advantages of surgical therapy, yet, without the risk implied in surgery. Publications on improved results from surgical therapy cannot be accepted as reasons for a general

[Radionuclide studies on healing in closed transverse fractures of the long tubular bones].

A total of 86 patients were examined by 99mTc-pyrophosphate in the course of therapy using the transosseous synthesis method. An elevated accumulation of the radiopharmaceutical was observed since the first day following a trauma. Maximum enhancement of metabolic processes in the absence of fragment displacement was noted on the 8th week and in the lack of displacement on the 9th-10th week. Then the intensity of bone formation got weaker. It was found appropriate that G. A. Ilizarov's apparatus should be removed exactly at that time. The content of mineral components in the regenerate was over
