Application of computerized virtual preoperative planning procedures in comminuted posterior wall acetabular fractures surgery.

BACKGROUND: The treatment of comminuted posterior wall acetabular fractures remains challenging due to the difficulty in understanding of fracture patterns and lack of appropriate preoperative planning process. Virtual preoperative planning procedures are now being commonly used in orthopedic surgery to aid in management of such complex problems. Our aim was to evaluate the feasibility and clinical value of a new method by applying computerized virtual preoperative planning procedures in the treatment of comminuted posterior wall acetabular fractures. METHODS: A total of 45 patients with comm

Titanium mesh osteosynthesis for the treatment of severely comminuted maxillofacial fractures in four dogs.

INTRODUCTION: Major goals in maxillofacial fracture treatment include to restore the dental occlusion, stabilise the major skeletal supports, restore the contour of the face and achieve proper function and appearance of the face. Titanium is considered an optimal material for maxillofacial reconstruction due to its biocompatibility, high strength, minimal inflammatory reaction and minimal imaging artefact. OBJECTIVES: To describe the clinical details, surgical technique, pre- and postoperative imaging and short- and long-term follow-up of severely comminuted maxillofacial fractures treated wi

External fixation of the hand: a simple approach to comminuted proximal interphalangeal joint fractures.

Proximal interphalangeal joint (PIPJ) fractures are a treatment challenge for hand surgeons. Poor treatment options may lead to stiffness, non-union and markedly reduced range of movement (ROM). We describe our experience using the Hoffman-2 micro static external fixation device (Stryker) in a case series of ten patients with closed comminuted intra-articular PIPJ fractures. The use of this device in the management of these fractures has not previously been described. The mean total active motion (TAM) of the injured joint had recovered to 80% of normal at the six-month follow up.

Posterior wall reconstruction using intertrochanteric crest strut graft in comminuted posterior acetabular wall fractures.

Posterior wall fractures are the most common acetabular fractures. Despite being frequently the easiest to treat, posterior acetabular wall reconstruction following such injuries can be challenging. A prospective study was conducted to evaluate the efficacy of a new surgical technique using gluteus medius muscle-pedicled intertrochanteric crest graft for the reconstruction of comminuted posterior acetabular wall fractures where primary comminuted fragments were beyond fixation. This technique has been performed on six patients from 2010 to 2012. The radiologic (Matta score) and clinical evalua

Lateral malleolus hook plate for comminuted Weber A and B fractures: A retrospective study.

BACKGROUND: The goal of managing the comminuted fracture of lateral malleolus is to restore length, rotation and alignment which might be more challenging with extensive comminution around the area of the distal tip. The common osteosynthesis techniques such as the one-third tubular plate, tension band wiring, K-wires, screws, or intramedullary nail may be insufficient in cases with a comminuted lateral malleolus. The anatomical hook plate is an alternative implant in such cases. We present our results of the comminuted lateral malleolar fractures (Weber A, B), managed by open reduction and in

Comminuted diaphyseal fractures of the radius and ulna: does bone grafting affect nonunion rate?

BACKGROUND: The recommendation of Anderson and colleagues to bone graft even relatively minor amounts of comminution (a third of the bone diameter) in the treatment of diaphyseal forearm fractures with plate-and-screw fixation has been questioned. This study examines factors related to nonunion in adult patients with comminuted diaphyseal fractures of both the radius and the ulna to determine the relative influence of bone grafting. METHODS: Over a 15-year period at two hospitals, 41 patients with diaphyseal fractures of both forearm bones satisfied the following criteria: comminution (Orthop

Dual Plating and Dorsal Bridge Plating for Comminuted Distal Radius Fractures: A Systematic Review.

Background/Objectives: Complex AO type C distal radius fractures remain amongst the most difficult injuries to treat, often requiring advanced fixation techniques. This systematic review focuses on two commonly used strategies for complex injuries: combined dorsal and volar plating and dorsal bridge plating. The goal is to examine reported functional and radiographic outcomes as well as complications of both techniques. Methods: A search was conducted across Embase, Medline, and Web of Science following PRISMA guidelines. Studies were included if they reported clinical or radiographic outcomes

Computer-assisted intraoral approach in the management of infected comminuted mandibular fractures: Technical notes.

In the extraoral approach for mandibular fractures, facial scars and facial nerve injury are common complications. Herein, we introduce an improved surgical technique to reduce fragments of infected comminuted mandibular fractures with the assistance of computer software. Firstly, the model was reset by 3-dimensional (3D) printing, and then the titanium plate was reconstructed on the model. Finally, open reduction and internal fixation through intraoral approach. This method can effectively control infection, reduce facial scars and avoid the risk of facial nerve injury.

Ex situ reconstruction of comminuted radial head fractures: is it truly worth a try?

INTRODUCTION: Complex radial head fractures are difficult to treat. In cases where stable fixation cannot be achieved, radial head resection or primary arthroplasty are frequently performed. Ex situ reconstruction of comminuted fractures may also be an option. This technique has widely been neglected in the literature, and only two small case series report satisfactory results. The aim of the present case series was to determine the functional and radiological outcomes of ex situ reconstructed Mason III and Mason IV fractures. We expect that the on-table reconstruction of comminuted radial hea

Biomechanical Comparison of Fixed- versus Variable-Angle Locking Screws for Distal Humerus Comminuted Fractures.

BACKGROUND: To compare the stability of fixed- versus variable-angle locking constructs for the comminuted distal humerus fracture (AO/OTA 13-A3). METHODS: Eight pairs of complete humeri harvested from eight fresh frozen cadavers were used for the study. We fixed the intact humeri using 2.7-mm/3.5-mm locking VA-LCP stainless steel distal humerus posterolateral (nine-hole) and medial (seven-hole) plates. An oscillating saw was used to cut a 1-cm gap above the olecranon fossa. The specimens were loaded in axial mode with the rate of 1 mm per 10 seconds to failure, and stress-strain curves were
