Does Management Really Matter? And If so, to Who?: Comment on "Management Matters: A Leverage Point for Health Systems Strengthening in Global Health".

The editorial is commendable and I agree with many of the points raised. Management is an important aspect of health system strengthening which is often overlooked. In order to build the capacity of management, we need to consider other factors such as, the environment within which managers work, their numbers, support systems and distribution. Effective leadership is an issue which cannot be overemphasized as part of management capacity in resource deprived settings as difficult settings require leadership skills in order to achieve managerial success. A primary issue of importance highlighte

Population screening for reproductive risk for single gene disorders in Australia: now and the future.

Abstract As the results of the Human Genome Project are realized, it has become technically possible to identify carriers of numerous autosomal and X-linked recessive disorders. Couples at risk of having a child with one of these conditions have a number of reproductive options to avoid having a child with the condition should they wish. In Australia the haemoglobinopathies are the only group of conditions for which population screening is widely offered and which is government funded. In some Australian states there are also population screening programs for cystic fibrosis and autosomal rece

Thinking ethically about genetic inheritance: liberal rights, communitarianism and the right to privacy for parents of donor insemination children.

The issue of genetic inheritance, and particularly the contradictory rights of donors, recipients and donor offspring as to the disclosure of donor identities, is ethically complicated. Donors, donor offspring and parents of donor offspring may appeal to individual rights for confidentiality or disclosure within legal systems based on liberal rights discourse. This paper explores the ethical issues of non-disclosure of genetic inheritance by contrasting two principle models used to articulate the problem--liberal and communitarian ethical models. It argues that whilst the latter provides a mor

Healthcare utilization of individuals with opiate use disorders: an analysis of integrated medicaid and state mental health/substance abuse agency data.

Data from the Substance Abuse and Mental Health Services Administration's Integrated Database (IDB) were used to examine the service use patterns of individuals with possible opiate use disorders in Washington State. Results indicate that regardless of Medicaid enrollment status, individuals who received mental health (MH) or substance abuse (SA) services only through state agencies received no inpatient substance abuse service. Furthermore, when compared with individuals who received at least one MH/SA service through Medicaid, those who received services only through the state agencies were

Syphilis in children: congenital and acquired.

Syphilis rates in women and congenital syphilis rates have declined steadily in the United States in recent years. However, syphilis remains a worldwide public health problem, with more than 12 million cases in adults and more than half a million pregnancies affected yearly. Prenatal screening and treatment programs are limited or nonexistent in many developing countries. The genome of Treponema pallidum, one of the smallest among prokaryotes, has been sequenced, but methods for continuous in vitro cultivation of the microbe remain elusive. There are no promising candidates for future vaccines

The return of tuberculosis.

At the end of the 20th century, tuberculosis remains a major public health issue. In developing countries tuberculosis is a leading cause of morbidity and mortality, and the spread of the HIV epidemic contributes significantly to the worsening of the situation. Coinfection with tuberculosis and HIV results in special diagnostic and therapeutic problems and uses up larger amounts of medical resources in developing countries. Outbreaks of multidrug resistant tuberculosis (MDR-TB) were first reported from US-American centers caring for HIV patients, but have now been observed in many other countr

SLC6A1 patient & organization perspective: founding of SLC6A1 connect, research, and ongoing efforts.

This paper describes the founding of the SLC6A1 Connect organization, which offers resources to patients and families with SLC6A1 diagnoses while keeping current with a scientific overview of the disorder. Following the birth of her two lovely twins, Amber Freed noticed how her son, Maxwell, missed motor development milestones and would often stare. Eventually, these signs led to a diagnosis of an SLC6A1 variant. The SLC6A1 gene is located on the short arm of chromosome 3 and the gene encodes for the gamma-aminobutyric acid (GABA) transporter 1 (GAT-1) protein. This transporter is responsible

The association between circulating SIGLEC6 and preeclampsia: observational studies of seven cohorts.

BACKGROUND: Preeclampsia is a serious complication of pregnancy. METHODS: We did an observational study using seven tissue bank/cohorts to examine the association between circulating SIGLEC6 and preeclampsia. We included samples from participants with preterm disease (delivering <34 weeks gestation in Australia), examined whether levels altered with clinical disease severity (samples collected in South Africa) and whether there were alterations preceding disease onset using samples collected at 15- and 20-weeks' gestation in New Zealand, samples collected between 26 and 34 weeks in the UK and

The role of genetic liability for psychiatric disorders and personality traits in post covid syndrome: data from three Nordic population cohorts.

BACKGROUND: Post-COVID syndrome (PCS) remains a substantial public health concern, yet its genetic determinants are poorly understood. Psychiatric disorders and related traits influence infection risk and acute COVID-19 outcomes, raising the possibility that shared genetic liability may also shape long-term symptom persistence. We examined whether polygenic scores (PGS) for schizophrenia (SCZ), bipolar disorder (BPD), major depressive disorder (MDD), attention-deficit/hyperactivity disorder (ADHD), and neuroticism are associated with PCS, and explored potential pathways underlying these associ

Investigating cytotoxic and inflammatory human IL-7 receptor low effector memory CD8+ T cells in lupus.

BACKGROUND: This study aims to interrogate the implications of CD8+ T cells in lupus by examining CD8+ T cell heterogeneity and assessing the significance of this heterogeneity in promoting inflammation and tissue damage. METHODS: Our own and publicly available RNA-seq and microarray data from the peripheral blood and kidney tissues of patients with lupus were analysed. Imaging Mass Cytometry (IMC) analysis of immune cells was conducted in lupus and normal kidney tissues. The effects of CD8+ T cell subsets on neutrophils were evaluated ex vivo. FINDINGS: Our scRNA-seq analysis showed an accu

Malignant hyperthermia safety - A nationwide survey of publicly funded Swedish healthcare.

BACKGROUND: Malignant hyperthermia (MH) is a rare pharmacogenetic disorder that can lead to a life-threatening reaction during general anaesthesia with triggering agents. Prompt life-saving treatment includes the immediate administration of the antidote dantrolene. This study investigated Swedish healthcare providers' awareness and adherence to guidelines and recommendations with respect to MH and whether adherence to safe MH-praxis varies with hospital care-complexity level and private versus public management form. METHOD: Agreements and procurement specifications between all 21 Swedish Cou

Health technology assessment criteria as drivers of coverage with managed entry agreements: a case study of cancer medicines in four countries.

BACKGROUND: Managed entry agreements (MEAs) continue to emerge in health technology assessment (HTA)-based decision-making, to address evidentiary uncertainties arising therein. Evidence on the HTA criteria that influence MEAs' uptake remains scarce. This study explores the HTA criteria that determine (i) if an HTA funding decision will be listed with conditions (LWC) other than a MEA, or with a MEA as a condition (LWCMEA), and ii) the MEA type implemented (i.e., financial, outcomes based, or combination). METHODS: HTA reports of all oncology medicines approved since 2009 in Australia, Englan

Clinical trials as disease control? The political economy of sleeping sickness in the Democratic Republic of the Congo (1996-2016).

Human African Trypanosomiasis (HAT), commonly known as sleeping sickness, is closer than ever to being eliminated as a public health problem. The main narratives for the impressive drop in cases allude to drugs discovery and global financing and coordination. They raise questions about the relationship between well-funded international clinical research and much less well-endowed national disease control programmes. They need to be complemented with a solid understanding of how (and why) national programmes that do most of the frontline work are structured and operate. We analyse archives and

How to Survive COVID-19 Even If the Vaccine Fails.

Coronavirus disease 2019 (COVID-19) has created an emergency of epic proportions. While a vaccine may be forthcoming, this is not guaranteed, as discussed herein. The potential problems and ominous signs include (1) lung injury that developed in animals given an experimental vaccine for the severe acute respiratory syndrome coronavirus (SARS-CoV)-1; (2) a perversion of adaptive immune responses called antibody-dependent enhancement of infection that occurs in SARS-CoV-1 and that may occur in people vaccinated for COVID-19; (3) the frequent and recurrent infections that are caused by respirator

Assessment of financial protection mechanisms for tuberculosis services in seven priority countries of the Western Pacific Region: a cross-sectional mixed-methods study.

BACKGROUND: Tuberculosis (TB) remains a major public health and socioeconomic challenge in the Western Pacific Region. Despite the expansion of universal health coverage (UHC), TB-affected households continue to face financial hardship. This study aimed to assess and compare financial protection mechanisms for TB services across the continuum of care in the region. METHODS: We conducted a cross-sectional mixed-methods study in seven TB priority countries in the Western Pacific Region: Cambodia, China, Lao People's Democratic Republic, Mongolia, Papua New Guinea, the Philippines, and Viet Nam.

New and Improved? 21st Century Cures Act Revisions to Certificates of Confidentiality.

Certificates of Confidentiality ("Certificates") are a federal legal tool designed to protect sensitive, identifiable research data from compelled disclosure. Congress first authorized their use in 1970 to facilitate research on illegal drug use. The scope of their use was later expanded to cover mental health research and then again to apply broadly to identifiable, sensitive research data, regardless of topic. Certificates can be critical to enabling conduct of essential research on sensitive topics, such as effective interventions to curb the opioid epidemic or reduce HIV transmission among

Mental health of prerelease incarcerated women.

BACKGROUND: The mental health of incarcerated women is particularly poor, yet little is known about their condition during the prerelease period. This critical transition is shaped by stressors related to incarceration and the challenges of reintegration, raising important concerns. Notably, the release phase that follows is characterized by risks of rehospitalization, reincarceration and suicide among individuals with psychiatric disorders and/or substance use disorders (SUDs). The aim of this study was to estimate the prevalence of psychiatric disorders, SUDs and dual diagnoses (DDs, i.e., t

Directed-complement killing of Pseudomonas aeruginosa protects against lethal pneumonia.

BACKGROUND: Multidrug-resistant Pseudomonas aeruginosa raises major clinical concerns due to its capacity to cause a wide-array of infections in individuals with compromised immune defences and to withstand standard-of-care therapeutic treatments. Antibody-based approaches have proven to be efficient in the treatment of diverse infections. Here we propose an innovative approach harnessing the complement at the surface of bacteria for further killing. METHODS: We developed two Complement-activating Multimeric immunotherapeutic compleXes (CoMiX) targeting the bacterium through a single-chain va

Spatiotemporal patterns of Lyme disease in North Carolina: 2010-2020.

BACKGROUND: Lyme disease is the most common vector-borne disease in the United States with the majority of cases occurring in the Northeast, upper Midwest, and mid-Atlantic regions. While historically considered a low incidence state, North Carolina (NC) has reported an increasing number of cases over the past decade. Therefore, the aim of this study was to characterise the spatiotemporal evolution of Lyme disease in NC from 2010 to 2020. METHODS: Confirmed and probable cases reported to the NC Division of Public Health without associated travel to high-transmission state were included in the

Why don't older adults in England go to have their eyes examined?

PURPOSE: Preventable sight loss is one of the Public Health Outcome Indicators in England. Despite availability of NHS-funded eye examinations, many people do not take up their entitlement. This paper explores older adults understanding of eye health and the purpose of eye examinations and the reasons why they do or do not attend for eye examinations. The aim is to provide evidence to inform policy on increasing uptake of eye examinations among older people who have increased risk of preventable sight loss. METHODS: 10 focus-group meetings were held with people living in deprived areas of Lee
