The asymmetric gait toenail unit sign.

The aim of this investigation was to resolve a diagnostic problem and report toenail unit changes attributable to shoe friction that resemble onychomycosis, but that are fungus-negative, and identify common skeletal causes in patients with an asymmetric walking gait. X-ray and clinical feet inspections were performed to evaluate skeletal components that change normal foot biodynamics. Forty-nine patients, all dermatophyte-negative, were reviewed. All patients were those seen in our private practice who demonstrated skeletal and toenail unit abnormalities such as onycholysis, nail bed keratosis

Subclinical hypothyroidism presenting with gait abnormality.

Subclinical thyroid disease is a common disorder, particularly in middle-aged and elderly individuals. Some patients with manifest hypothyroidism complain of unsteadiness of gait. The management of subclinical hypothyroidism is controversial. A 61-year-old man presented with a mild gait abnormality. He walked unsteadily on a broad base. Examination revealed a slight cerebellar ataxic gait. Laboratory evaluation showed elevated levels of thyroid-stimulating hormone. Serum free thyroxine was normal and thyroglobulin antibodies were present. Computer tomography of the patient's head was normal, a

Proprioceptive perturbations of stability during gait.

Through recent studies, the role of proprioceptors in reactions to perturbations during gait has been finally somewhat better understood. The input from spindle afferents has been investigated with tendon taps, vibration and other forms of muscle stretches, including some resembling natural perturbations (stumbling, slips, and ankle inversions). It was found that activation of spindle afferents produces short-latency response (SLR), consistent with a fast spinal pathway. These reflexes induce relatively minor activation in the stretched muscles. A central question is whether stretch reflexes c

Gait apraxia: further clues to localization.

BACKGROUND/AIMS: Gait apraxia characterized primarily by gait ignition failure has been linked to lesions involving the dorsomedial frontal lobes, but the precise locus within this general region has not been determined. It has previously been hypothesized by Thompson and Marsden that disease, disconnection, or dysfunction of supplementary motor area (SMA) may account for the similarities in the gait disorders observed in Binswanger's disease, hydrocephalus, frontal lobe lesions, and Parkinson's disease. We reevaluate this hypothesis. METHODS: Clinical description and MRI of 2 subjects with g

Gait initiation in bilateral vestibular loss.

OBJECTIVE: Gait initiation is a transient procedure between orthostatic posture and steady-state locomotion and includes anticipatory anteroposterior (AP) and lateral movements. Commands for this task are located in some levels of brain stem, which modulates activity of central pattern generator in the spinal cord. The purpose of this work was to explore the role of the vestibular system in this voluntary stepping. METHODS: Six patients with chronic bilateral vestibular loss (BVL) were measured by means of a large force plate (70 cm x 120 cm ) and the displacement of the center of pressure (C

Calcaneal gait. Etiology and clinical presentation.

Equinus deformities are common in neuromuscular and trauma patients. In contrast, calcaneal gait disorders tend to be overlooked, and few reports characterize and review calcaneal gait problems and disabilities. In the past 3 years, the authors treated 17 patients with calcaneal gait. Six of these cases are reviewed; in addition, the general clinical characteristics of the patient with calcaneal gait pattern are described.

Kinematic gait analysis in hemiplegic patients.

Temporal-distance variables of gait were investigated in 8 female and 23 male hemiplegic patients in order to assess the distribution of these variables according to functional ambulation category and to evaluate their validity. Video-recording technique was used for obtaining the temporal-distance values. Velocity, step-time, stride length and stride length in relation to lower extremity length proved to be valuable measures in the gait analysis, while cadence, step-time and step-time differential values seemed to be less important.

Gait disorder in late-life hydrocephalus.

Computerized analysis of gait was performed in six patients with hydrocephalus (mean age, 75.7 years), for comparison with other elderly patients with gait problems and age-matched controls. A decrease in velocity and stride, an increase in sway, and the proportion of time spent in double-limb stance were nonspecific features correlated with declining performance. Certain features appeared to characterize hydrocephalic gait when compared among groups; cadence was diminished, and there was a reduction of step height and a decreased counterrotation of the shoulders relative to the pelvis. An abn

Temporal gait monitoring: a new device.

A new device that provides continuous recording of gait and gait events is presented. It can be used with motor point electromyography to perform walking electromyography, an electrogoniometer to continuously record joint position, or other measuring devices to record events or activities related to phase of gait. The major advantages of this system as compared with available systems are ease of use, decreased patient encumbrance, potential for portability, potential for digitizing the signal for minicomputer compatibility, and greatly reduced expense.

Gait analysis in surgically treated clubfoot.

Clubfoot, a congenital foot abnormality, has been successfully managed with surgical treatment. To assess gait in individuals with surgically treated clubfeet, the patterns of electromyographic activity for the medical gastrocnemius and tibialis anterior muscles and the stride characteristics during gait were measured and compared with normal values. The duration of medial gastrocnemius activity was significantly greater than normal, whereas tibialis anterior activity, stride length, and single limb support times were not different. No correlation was found between the results of the gait anal

Reliability of observational kinematic gait analysis.

Gait analysis, like all clinical assessments, is subject to measurement error. Specification of the extent of measurement error is imperative before drawing conclusions from any test. The purpose of this study was to determine the within-rater and between-rater reliability of observational gait analysis in a pediatric sample wearing knee-ankle-foot orthoses. Three expert observers, using a 3-point scale, rated videotaped gait kinematics of 15 children who had lower limb disability and who wore braces. The rating sessions were then repeated, with one month between sessions. Total agreement (ide

Steady state gait and beyond: Complex mobility tasks in the laboratory reveal gait changes in Parkinson's Disease.

BACKGROUND: Parkinson's Disease (PD) is a progressive neurodegenerative disorder that impairs gait and balance. However, most existing objective gait assessments focus on parameters from steady state gait (SSG) and coarse measures of turns. As a result, these analytical approaches overlook the challenges people with PD encounter during more complex mobility tasks such as gait initiation (GI), turning, and gait termination (GT). Identifying relevant parameters of complex mobility tasks would improve mobility assessments. PURPOSE: The primary focus of this study was to evaluate gait parameters

Age-related and dual task-induced gait alterations and asymmetry: optimizing the Semmelweis Study gait assessment protocol.

Gait alterations are recognized as early markers of age-related decline and cognitive impairment. Dual-task assessments, which impose cognitive load while walking, provide valuable insights into gait control limitations and cognitive-motor interactions in aging populations. This study evaluates age-related and cognitive load-induced changes in gait parameters, with a particular focus on asymmetry, and aims to optimize the gait assessment protocol for the Semmelweis Study framework. The Semmelweis Study is a large-scale workplace cohort investigating the determinants of unhealthy aging and prom

Lower limb intelligent rehabilitation robot based on human-gait coupling, spatiotemporal gait sensing, and fuzzy PID control.

For individuals with dyskinesia, postoperative walking rehabilitation is crucial, and lower limb exoskeletons provide effective training assistance. This paper proposes a method for designing a lower limb exoskeleton based on sensor feedback and human-robot gait simulation. Firstly, a mathematical model of lower limb motion and an exoskeleton robot model are designed based on hip/knee joint motion and gait mechanisms. Secondly, gait simulation is performed via human-robot coupling, with a comparative analysis of lower limb muscle dynamics during walking. Hip/knee motion data is converted into

Can Gait Deviations Be Identified through Video-Based Gait Analysis? Validation of the BlazePose Pose Estimation Algorithm.

Accurate identification of gait deviations is critical for effective intervention in children with cerebral palsy (CP). However, traditional marker-based gait analysis systems, while highly precise, are costly and impractical for broad clinical applications. Video-based pose estimation algorithms (PEAs) such as BlazePose offer a potential alternative, leveraging standard video inputs to provide scalable and accessible gait assessment. This study assessed the validity of BlazePose in diagnosing gait deviations and analyzing joint kinematics in children with CP, using marker-based motion capture

Towards a diagnostic tool for neurological gait disorders in childhood combining 3D gait kinematics and deep learning.

Gait abnormalities are frequent in children and can be caused by different pathologies, such as cerebral palsy, neuromuscular disease, toe walker syndrome, etc. Analysis of the "gait pattern" (i.e., the way the person walks) using 3D analysis provides highly relevant clinical information. This information is used to guide therapeutic choices; however, it is underused in diagnostic processes, probably because of the lack of standardization of data collection methods. Therefore, 3D gait analysis is currently used as an assessment rather than a diagnostic tool. In this work, we aimed to determine

Novel attentional gait index reveals a cognitive ability-related decline in gait automaticity during dual-task walking.

INTRODUCTION: Gait automaticity refers to the ability to walk with minimal recruitment of attentional networks typically mediated through the prefrontal cortex (PFC). Reduced gait automaticity (i.e., greater use of attentional resources during walking) is common with aging, contributing to an increased risk of falls and reduced quality of life. A common assessment of gait automaticity involves examining PFC activation using near-infrared spectroscopy (fNIRS) during dual-task (DT) paradigms, such as walking while performing a cognitive task. However, neither PFC activity nor task performance in

Beta Burst-Driven Adaptive Deep Brain Stimulation Improves Gait Impairment and Freezing of Gait in Parkinson's Disease.

BACKGROUND: Freezing of gait (FOG) is a debilitating symptom of Parkinson's disease (PD) that is often refractory to medication. Pathological prolonged beta bursts within the subthalamic nucleus (STN) are associated with both worse impairment and freezing behavior in PD, which are improved with deep brain stimulation (DBS). The goal of the current study was to investigate the feasibility, safety, and tolerability of beta burst-driven adaptive DBS (aDBS) for FOG in PD. METHODS: Seven individuals with PD were implanted with the investigational Summit™ RC+S DBS system (Medtronic, PLC) with leads
