Advances in positron emission tomography for the imaging of rheumatoid arthritis.

Imaging techniques, such as US, are well recognized as important tools to aid early diagnosis and assess response to treatment in RA. PET offers a means of imaging the inflammatory processes of RA at a cellular level and thus may be a highly sensitive method of assessing synovitis. In this review we discuss the advantages of PET as an imaging modality for RA and summarize the existing clinical studies of PET in RA. We also highlight potentially important preclinical studies of PET in arthritis and discuss current limitations of PET as well as ongoing developments in PET technology likely to be

Short Dynamic 68 GA-PSMA PET Reconstruction to Differentiate Urinary Excretion From Extra-urinary Uptakes With LAFOV-PET.

LAFOV PET presents an excellent sensitivity, offering the ability of high-quality extra short images. We assess through a case report the additional value of performing 68 GA-PSMA-11 PET/CT dynamic acquisitions on an LAFOV PET system to improve discrimination between pathological uptakes, such as lymph nodes, from urinary physiological uptake. We performed a single-bed 4-minute list-mode acquisition, reconstructed in 4 frames of 1 minute, providing a dynamic visualization of 68 GA-PSMA-11 urinary ureteral excretion over time. This additional reconstruction is easy to implement in clinical prac

Severe Post-treatment Calcification for Prostate Cancer With Dual ¹⁸F-FDG and ⁶⁸Ga-PSMA Avidity on PET/CT.

A 66-year-old man with a history of prostate adenocarcinoma treated with radiotherapy and endocrine therapy underwent 18 F-FDG and 68 Ga-PSMA PET/CT. Both scans showed marked radiotracer uptake, raising suspicion for disease recurrence. However, subsequent transurethral resection of the prostate was performed for treatment with concurrent pathologic biopsy, confirmed only chronic inflammation accompanied by focal necrosis and calcification, emphasizing the false-positive risk of these modalities in nonmalignant conditions.

Superiority of long axial field-of-view 68Ga-PSMA-11 PET over short axial field-of-view PET in prostate cancer: a head-to-head comparison.

BACKGROUND AND PURPOSE: Previous studies have shown that long axial field-of-view (LAFOV) 68Ga-PSMA-11 PET/CT can significantly improve the detection rate compared to short axial field-of-view (SAFOV) 68Ga-PSMA-11 PET/CT in prostate cancer. However, the diagnostic efficiency between the two scans was compared in different cohorts, not directly within the same patients. Whether LAFOV 68Ga-PSMA-11 PET could improve image quality, enhance lesion quantification, and impact patient treatment decisions in the same patients remains unclear. To address this research gap, we performed a direct comparis

Early prediction of response to ADT plus ARPIs in mHSPC using interpretable machine learning on PSMA PET/CT: a real-world study.

BACKGROUND: Reliable tools for early prediction of treatment response to androgen deprivation therapy (ADT) plus novel androgen receptor pathway inhibitors (ARPIs) in metastatic hormone-sensitive prostate cancer (mHSPC) remain lacking. This study aimed to develop and validate an interpretable machine learning model integrating [⁶⁸Ga]Ga-PSMA PET/CT-derived imaging features to predict response before therapy initiation. METHODS: In this real-world study, 212 de novo mHSPC patients undergoing [⁶⁸Ga]Ga-PSMA PET/CT were included. Eighteen imaging and eight clinical features were extracted. Three e

Comparison of clinical performance between late and standard total-body [68 Ga]Ga-PSMA-11 in biochemical recurrent prostate cancer.

BACKGROUND AND PURPOSE: Enhanced lesion detection in prostate cancer is observed with late [68 Ga]Ga-PSMA-11 PET/CT imaging compared to standard [68 Ga]Ga-PSMA-11 PET/CT imaging (50-100 min p.i.). However, the poor image quality of late imaging using short axial field of view (SAFOV) PET/CT has hindered its sole clinical adoption. Conversely, the image quality of late imaging with a long axial field of view (LAFOV) [68 Ga]Ga-PSMA-11 PET/CT fulfills clinical diagnostic requirements. Nonetheless, the diagnostic efficacy of late LAFOV [68 Ga]Ga-PSMA-11 PET/CT with forced diuresis and its impact o

68Ga-Prostate-Specific Membrane Antigen-Avid Malignant Pleural Effusion in a Patient With Metastatic Adenoid Cystic Carcinoma and Concordance With 18F-FDG PET/CT.

Adenoid cystic carcinoma (ACC) is a rare cancer that arises from salivary glands and other secretory glands. Pulmonary metastases are frequently observed in ACC patients with metastatic disease. Previous research showed that ACC often shows high PSMA uptake on 68Ga-PSMA-11 PET/CT. Here, we present PET images from an ACC patient with pulmonal, pleural metastases, and malignant pleural effusion, with comparable tracer uptake on 68Ga-PSMA-11 PET and 18F-FDG PET.

Rare Brain Metastasis in Parotid Adenoid Cystic Carcinoma Detected on 68Ga-PSMA PET/CT.

We present the case of a 45-year-old woman with known adenoid cystic carcinoma of the right parotid gland, postexcision and postoperative radiation therapy. She was followed up on 18F-FDG PET/CT, which revealed minimal local residual disease and bilateral lung nodules, for which chemotherapy was initiated. Postchemotherapy, 18F-FDG PET/CT showed residual lung metastases. As a part of ongoing project, 68Ga-PSMA PET/CT done revealed additional focal uptake in the right cerebellum, missed on FDG PET/CT. The brain lesion was confirmed upon MRI.

Prostate-Specific Membrane Antigen (PSMA) Avid Pancreatic Neuroendocrine Tumor.

Ga-PSMA PET/CT is increasingly used to evaluate recurrent prostatic malignancy due to its high specificity. A 75-year-old man with a previous history of treated prostate cancer 3 years earlier presented with rising prostate-specific antigen (PSA) level and underwent Ga-PSMA PET/CT which demonstrated a PSMA-avid focus in the neck of the pancreas. Triple-phase abdominal CT demonstrated enhancement in the arterial phase and to a lesser extent the venous phase of a soft tissue mass in the neck of the pancreas. Cytological and histopathological examination of the soft tissue mass confirmed a low-gr

Positron emission tomography with 11C-methionine of intracranial tumours compared with histology of multiple biopsies.

Thirty-eight patients with suspected supratentorial gliomas have been investigated with positron emission tomography (PET) using 11C-methionine and computed tomography (CT) before and after intravenous injection of contrast medium. Every examination was performed with the head fixed in a plastic helmet and a baseplate to guarantee that the slices were corresponding in the different examinations and from time to time. After the examinations multiple stereotactic biopsies were performed with the biopsy instrument mounted on the baseplate. Accumulation of methionine was compared with normal brain

Refining deep learning segmentation in gallium-68-prostate-specific membrane antigen-11 positron emission tomography: evaluation of small lesion filtering and intersection-over-union thresholds.

PURPOSE: Evaluate how small lesion filtering and different intersection-over-union (IoU) thresholds influence deep learning segmentation of Ga-68-PSMA-11 PET images in prostate cancer. METHODS: A 3D U-Net was trained on 115 patient scans with manual contours as ground truth. Performance was assessed at voxel, lesion and patient levels. Lesions less than 8 voxels (195 mm 3 ) or less than 27 voxels (658 mm 3 ) were optionally discarded, and lesion-level metrics were computed across 10-50% IoU thresholds. RESULTS: Excluding lesions less than 27 voxels increased voxel-level Dice from 0.7975 to 0

Hepatic Perfusion-Related Pseudolesion Mimicking Metastasis on 68 Ga-PSMA PET/CT.

PSMA is a critical cell-surface molecule and 68 Ga-PSMA PET has become an established imaging modality that improves disease localization for newly diagnosed and recurrent prostate cancer. We report a patient with wedge-shaped increased PSMA uptake in segment VIII of the liver. MRI demonstrated no focal lesion; however, mild diffusion hyperintensity without apparent diffusion coefficient reduction and transient arterial hyperenhancement were observed. These findings were consistent with a perfusion-related pseudolesion rather than metastatic disease.

Divergent PET Tracer Avidity in Synchronous Sarcomatoid Carcinoma With Osteosarcomatous Differentiation and Adenocarcinoma of the Prostate: Insights From Dual-Tracer 68 Ga-PSMA and 18 F-FDG PET/CT Imaging.

A 76-year-old man with mildly elevated prostate-specific antigen (PSA, 5.44ng/mL) and a PI-RADS 5 lesion on multiparametric MRI underwent dual-tracer PET/CT. 68 Ga-PSMA PET/CT revealed focal intense uptake in one prostatic lobe, while 18 F-FDG PET/CT demonstrated avid uptake in the contralateral lobe. Histopathology confirmed synchronous high-grade prostatic adenocarcinoma (Gleason 9) and sarcomatoid carcinoma with osteosarcomatous differentiation. This discordant dual-tracer pattern highlights the complementary role of PSMA and FDG PET/CT in detecting molecularly divergent synchronous prostat

68 Ga-PSMA PET/CT After Open Transvesical Prostatectomy: The Diagnostic Challenge of Postoperative Urinary Reflux.

We report a case of a 68-year-old man with incidentally discovered prostate adenocarcinoma following open transvesical prostatectomy for presumed benign prostatic hyperplasia. Initial ⁶⁸Ga-PSMA PET/CT demonstrated focal uptake in the left seminal vesicle, raising concern for seminal-vesicle invasion. Delayed imaging showed persistent but decreasing activity paralleling urinary clearance, and multiparametric MRI confirmed postoperative changes without evidence of tumor extension. The finding was ultimately attributed to urinary reflux into the seminal vesicle, a potential false-positive pitfall
