INTRODUCTION: Preeclampsia is a public health problem which may manifest as pre-eclampsia (hypertension, proteinuria and/or edema) and eclampsia (convulsive crises and rarely coma in pregnant women with previous pre-eclampsia). But the pathology of hypertensive disease of pregnancy can present different clinical forms. Within that spectrum is HELLP syndrome: hemolysis (H), elevated liver enzymes (EL) and thrombocytopenia (LP).
OBJECTIVES: To assess the diagnostic criteria in the literature adopted for HELLP syndrome.

OBJECTIVE: To evaluate maternal deaths associated with eclampsia, HELLP syndrome, and the concurrence of these conditions.
METHOD: A review of Medline studies reporting maternal deaths associated with eclampsia or HELLP syndrome published in English, Spanish, and Portuguese between 1995 and June 2008.

Severe preeclampsia and HELLP syndrome are still the leading causes of maternal and perinatal morbidity and mortality. We present two cases of pregnancies which were complicated by HELLP syndrome at 31 weeks of gestation and 25 weeks of gestation, the first one with maternal and the second with perinatal fatal outcomes. The aim of this report is to draw attention to the life-threatening complications that might occur in cases of preeclampisa and HELLP syndrome. The importance of early diagnosis with implications for management is also discussed.

The present study compares neonatal outcome after preterm delivery of infants in pregnancies complicated by the HELLP syndrome or severe preeclampsia (PS). The maternal and neonatal charts of 71 out of a total of 409 pregnancies that were complicated by hypertensive disorders at Severance hospital between January 1995 and December 2004 were reviewed. Case report was presented of a 22 year old pregnant patient with diagnosed intrauterine fetal death, who developed HELLP syndrome and acute renal failure. Immediate hemodialysis treatment was initiated that permitted management of pulmonary edema; subsequent plasmapheresis eliminated not only elevated bilirubin and toxins liberated to circulation from the dead intrauterine fetus but also allowed for patient recovery. The HELLP Syndrome requires careful obstetric supervision and if possible treatment in the Intensive Care Unit.

A retrospective study was done of thirty-patients with severe preeclampsia and HELLP syndrome whose developed acute renal failure, 25 patients also had acute tubular necrosis and five cases bilateral cortical necrosis with chronic renal insufficiency. Severe hypertension was present in all cases and anti-hypertensive therapy was needed. Six patients died, three due to intracranial hemorrhage, other two secondary to hypovolemic shock, and in one case multiple organ dysfunction.

HELLP syndrome (Hemolysis, Elevated Liver enzymes, Low Platelets) is a rare complication that can appear during and/or after pregnancy in patients with preeclampsia or eclampsia. As the syndrome can have serious complications, early diagnosis is important for management. We describe a patient with HELLP syndrome who developed one of its unusual but severe complications: ruptured liver capsule.
