Hallmarks of primary headache: part 4 - rare headache syndromes.

BACKGROUND AND AIM: “Other primary headaches” is a group of heterogeneous headache disorders described in Chapter 4 of the third edition of the International Classification of Headache Disorders (ICHD-III). Due to the limited number of cases, most of these disorders are considered rare and represent a diagnostic and therapeutic challenge for clinicians and headache specialists. To increase knowledge, improve their recognition, and foster research on these rare headache disorders, this review provides a comprehensive up-to-date description of their hallmarks, including epidemiologic data, key c

Reversible Cerebral Vasoconstriction Syndrome for the Internist-A Narrative Review.

Reversible cerebral vasoconstriction syndrome is a common, increasingly recognized cause of thunderclap headache. Most patients have some type of trigger that precedes the onset (e.g., orgasm, physical exertion, Valsalva maneuvers, exposure to vasoconstrictive medications) followed by multiple short-duration thunderclap headaches that occur over days to weeks. Physical examination is often without focal neurological deficits. Brain computed tomography may be normal, or show a convexal subarachnoid hemorrhage. Angiography shows multifocal areas of cerebral arterial vasoconstriction, although ca

[CME/Answers: New-Onset Headache in Adults: Evidence-Based Assessment in the Primary Care Setting].

CME/Answers: New-Onset Headache in Adults: Evidence-Based Assessment in the Primary Care Setting Abstract. Many people suffer from headaches. Primary (idiopathic) headaches such as migraine and tension type headaches are most common. They may significantly affect occupational and social aspects of daily life, but are usually not dangerous. On the contrary, secondary (symptomatic) headaches may be a symptom for an underlying life-threatening disorder. In these cases, the primary care physician may need to organize certain diagnostic measures such as brain imaging, blood work or lumbar puncture.

Brain structure and function related to headache: Brainstem structure and function in headache.

OBJECTIVE: To review and discuss the literature relevant to the role of brainstem structure and function in headache. BACKGROUND: Primary headache disorders, such as migraine and cluster headache, are considered disorders of the brain. As well as head-related pain, these headache disorders are also associated with other neurological symptoms, such as those related to sensory, homeostatic, autonomic, cognitive and affective processing that can all occur before, during or even after headache has ceased. Many imaging studies demonstrate activation in brainstem areas that appear specifically asso

Epidemiology and Clinical Characteristics of Nummular Headache in Observational Studies: A Systematic Review and Meta-Analysis.

INTRODUCTION: Nummular headache (NH) is a rare primary headache disorder which was first described in 2002. This meta-analysis aimed to evaluate the prevalence and relative frequencies of clinical features of NH. METHODS: PubMed, Embase, Medline, and ScienceDirect were thoroughly searched for observational studies reporting the relevant data regarding NH diagnosed in accordance with ICHD-2, ICHD-3β, ICHD-3, or Pareja's original study. Random-effects meta-analysis was performed in order to calculate the pooled prevalence estimates (PPEs) and the I2 statistics was used to measure the between-st

Epidemiology of primary headache associated with sexual activity: A meta-analysis with diagnostic and management considerations.

OBJECTIVE: To assess the prevalence of primary headache associated with sexual activity (PHS) among adults seeking medical help for a headache in clinic-based settings. BACKGROUND: Primary headache associated with sexual activity is a benign primary headache disorder currently recognized as one of indomethacin-responsive and exercise-related cephalalgias. The epidemiology of PHS among adult patients assessed for headache in the tertiary headache centers is yet to be established. METHODS: PubMed/MEDLINE, Embase, and ScienceDirect databases were thoroughly searched for observational studies pu

Reliability of a telephone interview for the classification of headache disorders.

OBJECTIVE: The study aimed to test the reliability of a semi-structured telephone interview for the classification of headache disorders according to the ICHD-3. BACKGROUND: Questionnaire-based screening tools are often optimized for single primary headache diagnoses [e.g., migraine (MIG) and tension headache (TTH)] and therefore insufficiently represent the diagnostic precision of the ICHD-3, which limits epidemiological research of rare headache disorders. Brief semi-structured telephone interviews could be an effective alternative to improve classification. METHODS: A patient population r

Headache and Barometric Pressure: a Narrative Review.

PURPOSE OF REVIEW: Alterations in atmospheric pressure have been long associated with headaches. The purpose of this review article is to investigate the association of barometric pressure with headache, classifying into two broad categories primary headache disorders (barometric pressure triggering migraine or tension-type headache) and secondary headache disorders (barometric pressure triggering high-altitude headache and headache attributed to airplane travel), discussing the pathophysiology and possible treatments. RECENT FINDINGS: Multiple studies have been performed with inconsistent re

Chronic daily headache in the elderly.

Disabling headache disorders are ubiquitous in all age groups, including the elderly, yet they are under-recognized, underdiagnosed and undertreated worldwide. Surveys and clinic-based research reports on headache disorders in elderly populations are extremely limited in number. Chronic daily headache (CDH) is an important and growing subtype of primary headache disorders, associated with increased burden and disruption to quality of life. CDH can be divided into two forms, based on headache duration. Common forms of primary headache disorders of long duration (>4 hours) were comprehensively d

[Recent new information on epidemiology of headache].

We reviewed the epidemiology of headache disorders for the most frequent primary headache-syndromes: migraine, tension-type headache and trigemino-autonomic headache syndromes. In the last years scientific data about headache disorders have increased. New studies investigated not only the prevalence of headaches, but also economic costs of this disorder. Epidemiologic headache research also investigates the quality of life.

Attachment styles and headache.

The internal working model on attachment dimensions changes with significant emotional experiences. The purpose of this study was to evaluate if and how the internal working models correlate with primary headaches. Attachment dimensions of subjects suffering from primary headaches were studied. One hundred and fourteen subjects [68 with migraine, 23 with tension-type headache (according to ICHD-I criteria), 23 with chronic daily headache (according to Silberstein's criteria)], were studied and compared with a control group of 57 subjects (matched in sex, age and social level) not suffering fro

[Headache in the elderly].

Chronic headache is still a frequent problem in old age, affecting about 10% of all women and 5% of all men older than 70 years. The incidence of primary headache decreases with advancing age, while that of secondary headache increases. The clinical characteristics of migraine can also change with age; for example, vegetative symptoms are less prominent, and less intense migrainous pain localized predominantly in the neck is frequently reported. Migraine aura can also be experienced more frequently in isolation, without a headache. Hypnic headache is a rare primary headache syndrome that occur

Headache and Autonomic Dysfunction: a Review.

PURPOSE OF REVIEW: We explore the anatomy of the central and peripheral autonomic pathways involved in primary headache as well as the mechanisms for secondary headache associated with disorders of the autonomic nervous system. The prevalence and clinical presentation of cranial and systemic autonomic symptoms in these conditions will be discussed, with a focus on recent studies. RECENT FINDINGS: Several small studies have utilized the relationship between headache and the autonomic nervous system to identify potential biomarkers to aid in diagnosis of migraine and cluster headache. Experts' opinion about the primary headache diagnostic criteria of the ICHD-3rd edition beta in children and adolescents.

BACKGROUND: The 2013 International Classification of Headache Disorders-3 (ICHD-3) was published in a beta version to allow the clinicians to confirm the validity of the criteria or to suggest improvements based on field studies. The aim of this work was to review the Primary Headache Disorders Section of ICHD-3 beta data on children and adolescents (age 0-18 years), and to suggest changes, additions, and amendments. METHODS: Several experts in childhood headache across the world applied different aspects of ICHD-3 beta in their normal clinical practice. Based on their personal experience and

Interictal pain in primary headache syndromes.

Primary headache disorders are generally characterized by the pain, time course, and associated symptoms of their attacks, but often are accompanied by milder interictal pain. Patients with chronic migraine, chronic tension-type headache, hemicrania continua, and new daily-persistent headache have constant pain more often than not. Patients with trigeminal autonomic cephalalgias such as cluster headache commonly have interictal pain as well, usually much milder and unilateral to the side of attacks. Even those with rare headache types, including hypnic headache and trigeminal neuralgia, common

Evaluation for secondary causes of headache: the role of blood and urine testing.

Most patients presenting for evaluation of headache meet diagnostic criteria for a benign, primary headache disorder based on history and physical examination findings alone. No further testing is needed in such cases. Additional diagnostic evaluation is needed in cases that do not meet criteria for a primary headache disorder or which are associated with unusual or worrisome features. This article will review secondary causes of headache listed in the International Classification of Headache Disorders-II in which blood and urine testing might aid in diagnosis. We offer recommendations for dia

Clinical Reasoning: A Teenager With Right-Sided Headache and Periorbital Changes.

While migraine is the most common headache disorder in children and adolescents presenting to a neurologist, other primary headache disorders are important to recognize. Trigeminal autonomic cephalalgias represent a rare group of primary headache disorders with different characteristics, workup, and management. In this study, we present an adolescent with 1 common and 1 unique headache phenotype, followed by a guided discussion of the differential diagnoses, workup, treatments, and a brief summary of further management considerations.
