Treatment of Helicobacter pylori infection.

Helicobacter pylori infects the human stomach and is associated with chronic gastritis, peptic ulcer disease and gastric cancer. Most patients with peptic ulcers are infected with H pylori. These patients suffer a high relapse rate following initial healing of the ulcer with anti-secretory agents. The rate of relapse can be greatly diminished following successful eradication of the organism. Although experience is still being accumulated, it is reasonable to use eradication therapy in any patient with H pylori positive active peptic ulcer disease. Combination triple and dual therapy regimens a…

Helicobacter pylori and gastrointestinal disease.

Helicobacter pylori infection is quite common. In the United States, prevalence varies considerably with race, nationality, socio-economic status and location of residence. In Western countries, the prevalence of the infection has shown a steady increase with increasing age. H. pylori has been shown to cause chronic gastritis. Most patients infected with H. pylori are asymptomatic and require no therapy. The precise role of the infection in the pathogenesis of peptic ulcer disease is unknown. However, H. pylori infection is associated with a high recurrence rate of both gastric and duodenal ul…

Helicobacter pylori and gastric carcinogenesis.

Recent epidemiologic evidence indicates that Helicobacter pylori infection increases the risk for gastric carcinoma. Infection with H. pylori leads to chronic gastritis, which usually persists for life unless treated with antimicrobial drugs. Because the great majority of gastritis patients never develop neoplasias, research concerning those who do may provide clues about carcinogenesis. In affluent populations, H. pylori infection leads to nonatrophic gastritis, predominantly involving diffusely the antrum (diffuse antral gastritis), the basic lesion seen in patients with duodenal ulcer, whic…

[Helicobacter pylori infection in children].

Helicobacter pylori (Hp) is a pathogen of human gastric mucosa and is considered as the major cause of chronic gastritis and duodenal ulcer disease recurrence. The prevalence rate of Hp infection increases with age and is related to low socioeconomic status in childhood. Childhood appears to be a high-risk period for Hp infection. Clinical and histological features of Hp infection are variable and often moderate in children. This is probably due to both genetic and environmental factors. Abdominal pain is the most frequent symptom and is usually associated with a mild chronic gastritis. Serolo…

Helicobacter pylori in dental plaque.

Human and animal stomachs were considered the only reservoirs of Helicobacter pylori until it was identified in human dental plaque (DP). H. pylori is infrequent in populations from developed countries, but is widespread in those from developing countries. The failure of triple drug therapy to clear H. pylori from DP, despite its clearance from gastric mucosa, raised the possibility of DP as a permanent reservoir of H. pylori and a potential source of reinfection of gastric mucosa. Attempts should be made to eradicate H. pylori both from the gastric mucosa and from DP to ensure a permanent cur…

Unanswered questions about Helicobacter pylori.

There is general agreement that Helicobacter pylori eradication is indicated in all infected patients with duodenal ulcer disease and is probably indicated in all infected patients with gastric ulcer disease. However, translation of treatment recommendations into practice leads to some difficult clinical decisions. Three of the more perplexing questions are whether or not all patients with dyspepsia and H. pylori should be treated, whether or not a definitive diagnosis of ulcer should be established by an invasive method, and whether H. pylori eradication is sufficient to prevent recurrence of…

Treatment of Helicobacter pylori infections.

The available literature on the relationship between several diseases and Helicobacter pylori (H. pylori) is reviewed. Duodenal ulcer, gastric ulcer, complicated peptic ulcer, abdominal symptoms and gastroduodenal mucosal damage during the use of non-steroid anti-inflammatory drugs (NSAIDs), non-ulcer dyspepsia (NUD) and gastric malignancy are discussed. The case for and against eradication is critically discussed. Eradication of H. pylori should be pursued in all patients with peptic ulcer disease, whether they are using NSAIDs or not. Eradication of H. pylori in the treatment of NUD should b…

Barrett's oesophagus and Helicobacter pylori.

In order to demonstrate the presence of Helicobacter pylori in the metaplastic epithelium of Barrett's oesophagus and to evaluate its possible association with this entity, we examined 29 cases of Barrett's oesophagus where concomitant antral biopsies were also available. These cases were compared with an equal number of age and sex matched controls of uncomplicated reflux oesophagitis. H. pylori was present in 11 of 29 cases of Barrett's oesophagus (38%). No increase in the frequency of H. pylori antral gastritis was found in patients of Barrett's oesophagus compared to the control group of u…

[Helicobacter pylori and gastric ulcer].

In connection with longitudinal ulcer studies and the demonstration of Helicobacter pylori as the main cause of chronic gastritis, new aspects of gastric ulcer recurrences and healing become evident. This extends the possibilities to prognosticate the course of gastric ulcer and to use more effective treatment.

Pathogenic mechanisms of Helicobacter pylori.

There is general agreement that motility, urease activity, and association with gastric mucosal cells are important virulence factors of H. pylori. Urease activity is perhaps the best characterized of these factors. Presumably, urease activity creates a "cloud" of ammonia around the bacterium, thus neutralizing the lethal effects of gastric acid. Motility allows the bacterium to penetrate the mucus layer and promotes specific association of the bacteria with epithelial cells, further allowing evasion of gastric acidity. The association between gastrin levels and H. pylori infection is currentl

Helicobacter pylori-associated gastric pathology.

H. pylori is the most common cause of nonerosive nonspecific gastritis; however, its main importance has been as a marker in research studies of eradication in relation to duodenal ulcer relapse. In developed countries, the most common histologic pattern appears to be that of a mild superficial chronic active gastritis. When H. pylori is present in the antrum it is virtually always present in the body as well, although inflammation in body mucosa is usually milder than that in the antrum. The organisms do not overlie areas of intestinal metaplasia; thus, H. pylori is commonly absent in individ

Helicobacter pylori and gastric cancer.

Gastric cancer remains among the leading types of cancer worldwide. There is now convincing evidence linking H. pylori to adenocarcinomas of the gastric antrum, body, and fundus. These tumors are rapidly decreasing in incidence in the United States, whereas cardia tumors, tumors unassociated with H. pylori infection, are on the increase. Although criteria for causality have not been completely fulfilled for H. pylori and adenocarcinoma, there are plausible mechanisms by which chronic inflammation could induce carcinogenesis ("mitosis causes mutagenesis"). Because gastric cancer is unusual in t

Helicobacter pylori and gastric cancer.

This review focuses on the similarities between the epidemiology of gastric cancer and the epidemiology of Helicobacter pylori. Their demographic patterns and the results of studies regarding familial and environmental risk factors are described. The association of gastric cancer and H. pylori infection with both gastric ulcer and chronic atrophic gastritis is also characterized and the possibility that a H. pylori infection could lead to gastric cancer is discussed.

Helicobacter pylori in portal hypertension.

Trichoderma species are widely recognized as the primary causal agents of green mold contamination in mushroom cultivation. The pathogenicity of Trichoderma is mainly due to its competitive ability to rapidly colonize lignocellulosic substrates, thereby outcompeting mushroom mycelia for nutrients and space. Green mold contamination frequently occurs during the spawn-running and cropping stages, leading to substantial yield losses. Consequently, the development of effective and sustainable management strategies for green mold contamination has become a critical priority in mushroom production.

[Helicobacter pylori infections in children].

In our patient population with chronic aspecific abdominal complaints Helicobacter pylori gastritis is no rarity. This entity has been diagnosed in 10 children out of 48 (mean age: 8 years 11 months) who underwent an endoscopy of the upper gastrointestinal tract. After treatment with amoxicillin and colloidal bismuth subcitrate during a period of 2 weeks the complaints disappeared and an eradication was obtained in 7. Histology of the antral mucosa demonstrated a type B chronic active gastritis in all patients. A Helicobacter pylori specific serology was positive in all 10 patients. This test …
