The Korean hip fracture registry study.

BACKGROUND: The purpose of the Korean Hip Fracture Registry (KHFR) Study is to establish a nationwide, hospital-based prospective cohort study of adults with hip fracture to explore the incidence and risk factors of second osteoporotic fractures for a Fracture Liaison Service (FLS) model. METHODS: The KHFR, a prospective multicenter longitudinal study, was launched in 2014. Sixteen centers recruited participants who were treated for hip fracture. The inclusion criteria were patients, who were treated for proximal femur fracture due to low-energy trauma and aged 50 or more at the time of injur

[Orthopedic injuries associated with hip fracture].

INTRODUCTION: There Is little information showing orthopedic injuries at the same time as a hip fracture. Our objective is to determine if there is association between the type of orthopedic injuries associated in patients with hip fractures and their demographic variables, fracture pattern and mechanism of injury. MATERIAL AND METHODS: Transversal design, descriptive and retrospective. We reviewed cases with hip fractures and had an associated orthopedic lesion from November 01, 2012 to October 31, 2016. RESULTS: We included 103 patients, of whom women 61.2%; average age 64 ± 21 years. Over…

Perioperative mortality for patients with a hip fracture.

INTRODUCTION: Studies on mortality following hip fracture surgery have hitherto focused on the 30 day to 1 year period and beyond. This study focuses on the immediate perioperative period. It examines mortality rates, patient characteristics, operative details and post-operative complications. PATIENTS AND METHODS: A retrospective study of a hip fracture database in a large District General Hospital in the United Kingdom, from 1986 to 2015. A dataset of 9393 patients was identified, including patients undergoing surgery for curative and palliative purposes, over fifteen years of age and with

The use of cholecalciferol in patients with hip fracture.

INTRODUCTION: Major osteoporotic fractures are steadily increasing due to population aging. Programs of secondary prevention against refracture are essential to decrease morbidity and mortality and the cost for individuals and the society. Vitamin D supplementation and optimization of calcium intake are of a pivotal importance to start specific osteoporosis treatment and for its safety and efficacy. Cholecalciferol is the most widely employed drug for vitamin D supplementation. PURPOSE: Aim of this study was to assess the trends in the use of vitamin D supplements containing cholecalciferol i

[Lifestyle-related hip fracture risk].

Proximal femoral fracture (PFF) has long become a target for intervention with pharmacologic agents and pharmacologic intervention has been shown to reduce the fracture risk at least by half, compared to placebo, but PFF still continues to increase in incidence. Causes of fracture include bone-related and other factors but data remains yet to be accumulated on the relationship between these causes and lifestyle-related factors, such as nutrition, exercise and lifestyle preferences, which should be essential to establishing osteoporosis as a lifestyle-related disease. Furthermore, despite the p

[Injuries to femoral vessels in fractures of the hip].

Injury to femoral vessels in trochanteric fractures is a rare but a severe complication. Most often it involves the profunda femoris artery, less often the (superficial) femoral artery. This injury may be caused by different mechanisms; most frequently by pressure of a sharp bone fragment (the lesser trochanter), the tip of protruding cortical screws (extramedullary implant) or the distal locking screw (Gamma nail). Less often it is caused by the drill bit or incorrectly placed elevators. Pressure of a sharp bone fragment or contact with protruding tips of screws results in a gradual erosion o

Arthroscopy for hip dislocation and fracture-dislocation.

We reviewed consecutive patients undergoing hip arthroscopy for loose bodies after sustaining hip dislocations and fracture-dislocations not requiring open fracture management. Eleven patients were identified, all with intra-articular loose bodies diagnosed by computed tomography. After 3 weeks, all patients underwent hip arthroscopy in which loose bodies were removed and labral pathology debrided. No patient developed any of the complications (avascular necrosis, heterotopic ossification, nerve injury) associated with hip arthrotomy. Arthroscopic treatment of intraarticular loose bodies after

Incidence of hip fracture in Shiraz, Iran.

Hip fracture, the most serious complication of osteoporosis, puts the burden of a lot of costs for treatment on any health system. Previous reports on rates of hip fracture in Asian countries differ a lot. The aim of this study was to estimate the incidence rate of hip fracture in Shiraz, Iran. In Shiraz, Iran, all operating-room logs with diagnosis of hip fracture were reviewed for hip fractures caused by a simple fall in patients over 50 years, during March 21, 2000, to March 21, 2003. According to patients' medical records, age, sex, type of fracture (intertrochanteric, neck, subtrochanteri…

Outcome following a second hip fracture.

Hip fracture has a significant impact on medical resources and patient morbidity and mortality. Few patients admitted with a hip fracture are considered for prophylactic measures aimed at the reduction of further fracture risk. Currently, 10-13% of patients will later sustain a second hip fracture. In considering the possible role of prophylactic measures in the prevention of second hip fracture, we investigated whether second hip fracture has a significant further impact on patients' mobility and social independence. We also sought to identify a pattern to second hip fracture that may allow t

Hip fracture: risk factors and outcomes.

Hip fracture is an important public health and personal burden, and this burden is anticipated to increase over the next several decades. Although white women experience the greatest lifetime risk of hip fracture, risk extends to men and to nonwhite populations. Bone strength, risk of falling, and individual clinical characteristics combine to affect the risk of hip fracture. Nearly $9 billion were expended in 1995 in the United States for the management of hip fractures. Hip fracture has important sequelae, including loss of bone and muscle mass. Mortality is significantly increased after hip…

Variations in the performance of hip fracture procedures.

OBJECTIVES: Hip replacement is the preferred treatment for displaced femoral neck fractures, whereas other less expensive procedures are preferred for nondisplaced fractures. The authors determined whether there was geographic variation in the use of hip replacement to treat displaced and nondisplaced fractures. METHODS: The authors studied 332 patients, age 65 years or older, hospitalized with a femoral neck fracture in three cities. RESULTS: The population was 55% over age 80, 80% female, and lived in Houston (17%), Pittsburgh (29%), and Minneapolis (54%). Rates of hip replacement varied b

Quantification of osteopenia in hip fracture patients.

Osteopenia was assessed in 56 consecutive hip fracture patients by using dual photon absorptiometry of the lumbar spine and uninjured, contralateral femoral neck and by performing iliac crest biopsy. Of the 56 patients, 47 were female and 9 were male. The mean age was 78 years. There were 31 femoral neck and 25 intertrochanteric fractures. Two comparison populations were also studied. Population 1 consisted of 269 postmenopausal females older than 60 years of age with no history of fracture who were referred for dual photon absorptiometry of the lumbar spine as screening for osteoporosis. Functional outcomes after hip fracture.

The ability to achieve independent ambulation after hip fracture, orthopedic stabilization, and subsequent rehabilitation was studied in 65 inpatients in a specialized geriatric rehabilitation center. The association of 10 factors with the ability to achieve independent ambulation was measured by Kendall's tau B analysis and a two-tailed t test. The 10 factors were sex, age, surgical technique, side of fracture, site of fracture, presence of previous fracture, presence of lower extremity contractures, strength of hip abductor muscles, number of visits to physical therapy, and number of days fr

[Vitamin D deficiency and hip fracture].

Vitamin D deficiency is common in the elderly, especially in patients with hip fracture. Elderly people infrequently stay outside in the sunshine, and nutrition is deficient in vitamin D. In addition, the hydroxylation of vitamin D into active metabolites decreases with age. Vitamin D deficiency ultimately leads to osteomalacia, but in an earlier stage it causes secondary hyperparathyroidism, which is accompanied by increased bone turnover and cortical bone loss. Along these pathways vitamin D deficiency may contribute to the pathogenesis of hip fractures. In a survey in Amsterdam vitamin D de…

[Hip dislocation with femur head fracture].

It is talked about the classification, the therapy and the late results of the traumatic dislocation of the hip joint with a fracture of the femoral head. Twelve patients with an average age of 36 years (from 18 to 77 years) were treated between the years 1972 and 1982. The rarer anterior dislocation of the hip including a fracture of the femoral head should be specifically classified, in order not to cause any errors for the therapeutic and prognostic assessment following the Pipkin classification. Type I and II with posterior dislocation justify a closed trial of reposition. The miscarried t
