Our experience with 10 patients who suffered midshaft fractures of the femur and injuries to their ipsilateral hips has resulted in a treatment protocol that we believe can avoid unnecessary complications. The surgical protocol is a three-stage procedure based on (1) intramedullary nailing of the femur with interlocking for preserving the anatomic length and rotation of the femur; (2) treatment of the hip joint injury by means of open reduction and internal fixation of the acetabular fracture and/or reduction and fixation of the fractured femoral neck; and (3) repair of the extensor mechanism.

INTRODUCTION: Bone loss is most common around the knee region in spinal cord injury (SCI) population. However, bone mineral density (BMD) measurement and T score in the knee region, which indicate osteoporosis, are not clear. This study aimed to examine BMD, T and Z scores at hip and knee including femoral neck (FN), total hip (TH), proximal tibia (PT), and distal femur (DF) in individuals with SCI, to investigate the relationship between BMD of the knee and hip, and to define DF and PT T scores thresholds that predict osteoporosis at the hip.
MATERIALS AND METHODS: This cross-sectional study

Femoral nerve injury is a rare but devastating complication of direct anterior approach total hip arthroplasty that occurs in about 1% of the cases and could potentially lead to debilitating loss of knee extension. In this case report, we present a case of femoral nerve injury following direct anterior approach hip arthroplasty with an inability to extend the affected knee, gait instability, and multiple falls. For this patient, an innovative functional adductor magnus muscle transfer was performed to restore knee extension. At 6 months after surgery, the patient's knee extension was partly re

A 71-year-old man who had hip abductor insufficiency due to a chronic injury to the right superior gluteal nerve injury after lipoma resection presented to our outpatient clinic 1.5 years postoperatively with persistent pain, atrophy of the gluteus medius muscle, and Trendelenburg sign with a corresponding limp. A magnetic resonance imaging scan and neurophysiological diagnostics confirmed a chronic lesion of the superior gluteal nerve with completed reinnervation and absent pathological spontaneous activity, excluding neurosurgical options to restore hip abduction. Following interdisciplinary

OBJECTIVE: To examine the clinical risk factors of perioperative pressure injury (PrI) in older adults with a hip fracture, including preoperative chronic comorbidities and postoperative complications.
METHODS: In this retrospective study, the authors queried the PearlDiver Patient Records database between January 2011 and January 2020. Data from 54,194 patients without preexisting PrI were included for analyses. Patients were separated into two groups: (1) one or more perioperative PrI and (2) no PrI. Clinical factors as outcome variables include 21 comorbidities and 10 complications.

BACKGROUND: Hip strength is an important factor for control of the lumbo-pelvic-hip complex. Deficits in hip strength may affect throwing performance and contribute to upper extremity injuries.
HYPOTHESIS: Deficits in hip abduction isometric strength would be greater in those who sustained an upper extremity injury and hip strength would predict injury incidence.
STUDY DESIGN: Prospective cohort study.
LEVEL OF EVIDENCE: Level 3.
METHODS: Minor League baseball players (n = 188, age = 21.5 ± 2.2 years; n = 98 pitchers; n = 90 position players) volunteered. Hip abduction isometric strength w

BACKGROUND: Postsurgical acute nerve injury is rare but potentially devastating following total hip arthroplasty (THA). Previous literature suggests a wide range of incidence from 0.1% to 7.6%. Confirmed risk factors for these injuries remain unclear.
METHODS: THA patients at our institution who developed nerve injury during their admission for THA between January 1, 1998, and December 31, 2013, were systematically identified and matched with 2 control subjects by surgical date. Relevant patient and surgical data were obtained through review of patient charts and electronic health records. We

PURPOSE: Although injury of the lateral femoral cutaneous nerve (LFCN) is a known complication after total hip arthroplasty (THA) via the direct anterior approach (DAA), the impact of this complication on postoperative quality of life (QOL) is unclear. This study aims to investigate the incidence of LFCN injury after DAA for THA, and to determine the impact of LFCN injury on QOL and hip function.
METHODS: We prospectively investigated 122 hips operated upon using the DAA regarding the incidence of LFCN injury using self-reported questionnaires, hip functional scores such as the Harris Hip Sco

The aim of this study is to compare the effects and complications of InterTAN nail versus Dynamic Hip Screw (DHS) in treatment of femoral intertrochanteric fractures. Between January 2006 and December 2013, a total of 75 patients with femoral intertrochanteric fractures were retrospectively reviewed. There were 16 cases of type A1, 51 cases of type A2, and 8 cases of type A3. 37 patients treated with InterTAN nail, and 38 with DHS. The clinic data of surgical trauma, postoperative function and complications were statistically compared in this study. This results indicated that 31 cases by Inte

BACKGROUND: Bilateral asymmetric hip dislocation, with one hip dislocated posteriorly and with anterior dislocation of the contralateral hip, is a rare injury pattern. A total of 34 cases have been reported in English literature, but only 24 cases detail injury mechanism and patient demographic factors, 3 of which reported bilateral asymmetric hip dislocation in female patients. Only one report describes more than one example. We describe four patients with bilateral asymmetric hip dislocation, including one example in a female patient, which represents the largest case series to date. Pertine

Delayed presentation of iliac artery injury by acetabular screw. Screw removal at liner revision of a 10-year-old uncemented acetabular shell resulted in unexpected catastrophic blood loss. Replacing the screw prevented further hemorrhage, and investigation revealed internal iliac artery injury caused by the screw. This was treated successfully by bypass grafting. Careful review of preoperative imaging should aim to identify those at risk and requiring further imaging before undergoing revision surgery.

The relative contributions of hip and knee proprioceptors to the origination of extensor spasms were examined in 11 subjects with chronic spinal cord injury (SCI). Ramp and hold extension and combined hip and knee oscillation movements were imposed to the right leg while the ankle was held in a static position by a custom-designed robot. Isometric joint torques of the hip, knee, and ankle and surface electromyograms (EMGs) from seven leg muscles were recorded following controlled hip and knee extension. A stereotypical torque response consisting of hip flexion, knee extension, and ankle planta

Injury risk curves for the skeletal knee-thigh-hip (KTH) relate peak force applied to the anterior aspect of the flexed knee, the primary source of KTH injury in frontal motor-vehicle crashes, to the probability of skeletal KTH injury. Previous KTH injury risk curves have been developed from analyses of peak knee-impact force data from studies where knees of whole cadavers were impacted. However, these risk curves either neglect the effects of occupant gender, stature, and mass on KTH fracture force, or account for them using scaling factors derived from dimensional analysis without empirical

The use of bone cement during total hip prosthesis implantation despite it's advantages triggers of some local and general complications. The paper presents and classifies most cement related complications, with special attention to vessel complications. This paper includes an external vein injury, which occurred 5 years after a total hip replacement procedure. Chronic erosion and mechanical stress of the iliac vein by a cement hook was the cause of this complication. It has been stress that most vascular complications are a result of imperfect surgical technique.

The factors involved in the mechanism leading to traumatic posterior dislocation of the hip are examined. In 47 adult patients who had previously suffered such a dislocation, ultrasound scans were used to measure femoral anteversion on both the affected and the uninjured side. In 36 normal adult volunteers, used as controls, similar measurements were made. Femoral anteversion on both the injured and uninjured side was significantly reduced in the patients compared with the volunteers. These findings are discussed in the light of previous work which indicates that medial rotation is a factor in

Fractures of the pelvic ring and acetabulum are caused by high-energy trauma, usually by traffic accidents. Acetabular fractures occur when a force drives the head of the femur against the acetabulum. As the prevalence of patients living with a total hip prosthesis (THR) increases, the chance of suffering periprosthetic acetabulum fractures also elevates. However, the injury threshold of forces resulting in a periprosthetic acetabular fracture is unknown. The study aimed to analyze the results of a dashboard injury on the acetabulum after total hip replacement through a head-on collision in an

BACKGROUND: Pressure injuries are a worldwide concern for patients in the acute hospital setting. They are a common hospital-acquired complication and Australian national standards, state policies, and local procedures have been developed to reduce incidence. Risk assessment is purported to be the gold standard in prevention. This study examined the association between the time of pressure injury risk assessment, the presence of a pressure injury prevention management plan, and the development of hospital-acquired pressure injury(s) in the high-risk cohort of hip fracture patients at a regiona

BACKGROUND: It remains unclear as to whether the clinical outcomes of periacetabular osteotomy (PAO) are influenced by the location and severity of preoperative labral injuries. This study aimed to examine the relationship between labral tear severity and clinical outcomes in patients undergoing PAO, with a focus on patient-reported outcomes and magnetic resonance imaging (MRI) assessments.
METHODS: This retrospective study included 40 patients (46 hips) who had undergone PAO for developmental dysplasia of the hip between 2015 and 2022. Patients were selected based on the availability of preo
