A case of right hydronephrosis as a complication of appendicitis is reported. The patient was a 63-year-old male, whose chief complaints were pollakisuria and dysuria. Roentgenographic examination revealed right hydronephrosis due to stenosis of right lower ureter and intrapelvic tumor at the right side. Exploration revealed an abscess in the pelvic cavity, and pathologic examination disclosed periappendiceal perforation and an old inflamed appendix. A review of the literature revealed that it is difficult to diagnose the appendicitis without typical symptoms.

A 41-year-old man was admitted to the hospital complaining of mild epigastralgia. Radiographic examinations revealed a right intrapelvic solid tumor (5 x 3 cm) apparently causing stenosis of the right ureter at the level of pelvic brim. The patient underwent an operation. The tumor which invaded the ileum and ureter was excised with resection of the affected segment of ileum and nephroureterectomy. Histological diagnosis was mesenteric fibromatosis. Barium enema study revealed no polyposis of the colon postoperatively. One year after surgery the patient has no signs of recurrence.

From March 1989 to December 1992, we encountered 25 kidneys in 21 patients with a suspicious ureteropelvic junction (UPJ) obstruction detect … initial DR. Of these, the renal function spontaneously reverted to normal in 2 cases with bilateral hydronephrosis. Another 1 kidney revealed significant UPJ obstruction assessed by pressure flow study and continuous nephrostomy drainage had brought relief from the obstruction 1 year later. Prolonged DT1/2 with normal SRF was found in 7 (28%) kidneys. The renal …

A 67-year-old woman with Sjögren's syndrome was found to have left hydronephrosis and stenosis of the left ureter. Exploratory laparotomy disclosed a nodule at the ureteropelvic junction of the left ureter. Histopathological examinations of the biopsied specimen of this nodule showed lymphoid hyperplasia within the ureteral wall. Low dose prednisolone improved hydronephrosis and pseudolymphoma within several months. Hydronephrosis secondary to pseudo-lymphomatous infiltration of the ureter is a rare but, if properly treated, reversible complication of Sjögren's syndrome.

The surgical cure of hydronephrosis must consist in the resection of the pyeloureteral junction, associated to a pyelic resection leaving an undistended renal pelvis. This article describes a few technical tricks that make the procedure easier and thus allow obtaining results. After proving its value for twenty years, this technique gives just as much satisfaction today.

The paper describes an unusual angiographic pattern within the context of stenosis of the pyeloureteral union and giant hydronephrosis of the adult, with a presumed congenital origin. From the embryonic point of view we discuss the remote possibility of this being a real "agenesis" of the renal artery with no agenesis of the kidney, irrigated by multiple segmental arteries following the characteristic metamerism of the mesonephros. On the other hand, it seems more consistent to explain such vascular pattern as an image of severe atrophy, in correlation with the hydronephrosis advanced stage, a…

This report reviews the management of the fetus with congenital hydronephrosis (CH), a challenging diagnostic and therapeutic problem. Experimental models of obstructive uropathy have produced histologic changes similar to those seen in kidneys of human neonates with congenital hydronephrosis. Relief of obstruction in utero in these models has been shown to prevent some of the dysplastic changes caused by obstruction. These studies have formed the theoretical basis for in utero decompression to restore amniotic fluid dynamics to prevent death from pulmonary hypoplasia, and reverse or arrest dy…

We report on a ten-year-old boy with a huge asymptomatic mass found to be a giant hydronephrosis in an ectopic pelvic kidney. Diagnosis and management of giant hydronephrosis are outlined.

The pelvis, an angulated upper segment of the ureter and the lower anterior renal segmental vessels entangle to produce hydronephrosis. However, which of the 3 structures provokes obstruction is conjectural. The structural relations in this anomaly were compared to those of normal kidneys, hydronephroses from other causes and nonrotated kidneys. This anomaly was unique in that the pelvis and ureteropelvic junction bulged over the lower hilar segmental vessels instead of under as in other forms of hydronephrosis. Transient or permanent defects of medial rotation of the renal pelvis may account …

Micropuncture and metabolic studies were performed using Sprague-Dawley rats with hereditary hydronephrosis secondary to vesicoureteric reflux. Whole kidney and single nephron glomerular filtration rates in hydronephrotic and nonhydronephrotic rats were essentially identical. Filtration rates were maintained near normal in hydronephrotic rats because of an elevation in the mean glomerular transcapillary hydraulic pressure difference which effectively offsets a lower initial glomerular capillary plasma flow rate and a possible decrease in the glomerular capillary ultrafiltration coefficient rel…

During the period from 1963 to 1976 28 patients among the congenital hydronephrosis cases underwent a repeated operation, which was performed within the terms from 13 days to 8 years after the first intervention. Most often the repeated nephrectomy became necessary due to the occurrence of marked changes in the tissues and renal functional loss. The failure of the first surgical correction resulted from tactical errors and gave rise to the development of severe pyelonephritis.
