Hyperaldosteronism as a common cause of resistant hypertension.

Resistant hypertension affects an estimated 10-15 million American adults and is increasing in prevalence. The etiology of resistant hyperte … ension. The antihypertensive benefit of spironolactone is not limited to patients with demonstrable hyperaldosteronism but instead can be effective in resistant hypertensive patients regardless of aldosterone levels. Chlorthalidone is a potent, long-acting thiazide-like diuretic and should be used preferentially to treat resistant hypertension as it is superior to n …

Changes in the perceived epidemiology of primary hyperaldosteronism.

Primary aldosteronism has been considered a rare disease in the past years, affecting 1% of the hypertensive population. Subsequently, growing evidence of its higher prevalence is present in literature, although the estimates of disease range from 5 up to 20%, as in type 2 diabetes and resistant hypertension. The main reasons for these variations are associated with the selection of patients and diagnostic procedures. If we consider that hypertension is present in about 20% of the adult population, primary aldosteronism can no longer be considered a rare disease. Patients with primary aldoster

[Primary hyperaldosteronisms: from diagnosis to treatment].

Primary hyperaldosteronism is a growing cause of apparently essential hypertension (until 15 % of patients with hypertension). Primary hyperaldosteronism can now be recognized even for patients without hypertension or hypokaliemia. Paradoxically normal levels of aldosterone when serum renin levels and/or plasmatic renin activity and/or kaliemia are low, lead to suggest primary hyperaldostéronism. High values of aldosterone/renin or aldosterone/PRA favour the diagnosis. Adrenal scanography (fine slices) has to be performed, and confronted with one functional procedure (adrenal iodocholestérol u…

[Diagnosis and differential diagnosis of primary hyperaldosteronism].

This literature review concerns diagnosis and differential diagnosis of primary hyperaldosteronism, a frequent cause of arterial hypertension. It describes different phases of diagnosis of this condition including main medicamentous tests and instrumental methods as well as nosological entities from which primary hyperaldosteronism needs to be distinguished.

Primary hyperaldosteronism in a dog with concurrent lymphoma.

An 11-year-old, male castrated English springer spaniel was presented for muscle weakness, lethargy and anorexia while undergoing treatment of Stage IV lymphoma. Persistent hypokalemia prompted multiple diagnostic tests. Serum aldosterone levels, surgical exploration and histopathology confirmed primary hyperaldosteronism. Hyperaldosteronism is a rarely reported endocrinopathy in the dog. This report describes a case in which immunohistochemistry was utilized to confirm the diagnosis of an aldosterone-secreting tumour.

[New potentialities in the treatment of idiopathic hyperaldosteronism].

The study was undertaken to examine whether the drugs affecting the renin-angiotensin system may be used in the combined treatment of patients with idiopathic hyperaldosteronism (IHA). The study covered 20 patients (mean age 46.6 +/- 7.3 years). The diagnosis in all the patients was verified by a complex of instrumental studies, including the determination of plasma aldosterone concentrations (PAC) and plasma renin activity (PRA), and topical diagnostic studies (ultrasonography, computed tomography (CT)/magnetic resonance imaging (MRI), and angiography). The patients were given spironolactone,…

Normokalemic hyperaldosteronism in patients with resistant hypertension.

BACKGROUND: Primary aldosteronism is a common cause of non-renal secondary hypertension. A correct diagnosis results in curing the hypertension or targeting appropriate pharmacotherapy. In patients with low renin resistant hypertension (after treatment with three or more different anti-hypertensive drugs the blood pressure remains above 140/90 mmHg), screening for aldosteronism is mandatory. OBJECTIVES: To demonstrate that normal blood levels of potassium in resistant hypertensive patients do not exclude the possible presence of hyperaldosteronism, and to suggest the use of the plasma aldoste…

Primary hyperaldosteronism in the era of laparoscopic adrenalectomy.

Laparoscopic adrenalectomy has been recommended as the standard method for removing an aldosteronoma. To assess our surgical experience with primary hyperaldosteronism in the era of laparoscopic adrenalectomy a 6-year retrospective review of 30 consecutive patients was done. The 20 men and 10 women ranged in age from 35 to 78 with a mean of 51.2 years. All patients were hypertensive and hypokalemic with a mean serum potassium of 2.9 +/- 0.32 (standard deviation) mmol/L. Serum aldosterone was elevated in 28 of 30 (94%) patients and normal in the remaining two. Serum renin was suppressed in all …

[Hyperaldosteronism and simultaneous pheocromocytoma: a puzzle case].

Pheochromocytoma and primary hyperaldosteronism are well-known causes of hypertension. It was recently reported that their frequency in the hypertensive population is 0.1-0.2% and 3-15%, respectively. We describe the case study of a patient with severe hypertension (200/100 mmHg) and the coexistence of pheochromocytoma and primary hyperaldosteronism attributable to adrenal hyperplasia. The originality of this clinical report is that the patient presented a pheochromocytoma in the left adrenal gland and hyperplasia of the contralateral adrenal gland. The simultaneous presence of pheochromocytom…

[Diagnosis of subtypes and lateralization for primary hyperaldosteronism].

OBJECTIVE: To evaluate the value of CT and posture stimulation test (PST) in the differential diagnosis and lateralization of primary hyperaldosteronism(PA). METHOD: Retrospective study was done based on the clinical data and pathological examinations of 22 patients with PA. RESULTS: The baseline plasma aldosterone concentration (PAC) of most aldosterone-producing adenomas (APAs) was over 250 pg.ml-1. In the diagnosis of APA, the sensitivity of CT was 75% and the specificity of CT was 100%; the sensitivity and specificity of PST were 87% and 100%, respectively. The examination results of CT …

[Role of ketanserin in the treatment of primary hyperaldosteronism].

Ketanserin (Ket), a 5HT-2 serotoninergic receptor antagonist, is currently used as an antihypertensive agent. In our previous studies, both in vitro and in vivo when administered as bolus i.v. in subjects with primary aldosteronism (IAP), Ket exhibited an inhibitory action on aldosterone secretion. Therefore in the present study we investigated, in IAP patients, the effects of chronic treatment with Ket on blood pressure and mineralocorticoid secretion. Ket was administered at increased doses of 20 and 40 mg twice daily per os for 30 days and the following parameters were evaluated: blood pres

[Morphology of the adrenal gland in primary hyperaldosteronism].

Between 1964 and 1980 25 patients (16 women and 9 men) underwent unilateral adrenalectomy for primary hyperaldosteronism at the Zurich University Hospital Department of Urology. In 17 cases (68%) review of the histological slides revealed adenoma, in 3 cases (12%) so-called idiopathic hyperplasia, and in one patient (4%) adrenal carcinoma. The histological features of these 21 cases were in keeping with the postoperative changes in blood pressure. However, in 4 patients (16%) the alterations of adrenal tissue could not be either correlated with the subsequent clinical course or satisfactorily …

[A simple diagnostic test for primary hyperaldosteronism].

Plasma aldosterone levels were measured as part of two suppression tests in 31 hypertensive patients on normal sodium diet and without recen … to 360 pmol.l-1 after sodium load or to 665 pmol.l-1 after Captopril were characteristic of primary hyperaldosteronism due to adenoma. The test using Captopril has the advantages of being rapid, of avoiding acute blood volume expansion and of being applicable to all forms of hypertension, including severe ones.

The mineralocorticoid hormone pathways in hypertension with hyperaldosteronism.

Simultaneous measurement of the 0800-hr plasma concentrations of deoxycorticosterone (DOC), corticosterone (B), 18-hydroxycorticosterone (18 … adrenocortical carcinoma (AP-Ca). Elevated DOC levels are rarely found in patients with idiopathic hyperaldosteronism (IHA or adrenal hyperplasia) and suggest that IHA is not a primary adrenal disorder and should be excluded from the syndrome of primary aldosteronism as they have been heretofore.
