Natural history of primary hyperparathyroidism.

Primary hyperparathyroidism has evolved into a disorder that is largely asymptomatic. Nevertheless, there is ample evidence of target organ effects even in asymptomatic patients. Recent data suggest that the disease is stable in most asymptomatic patients. Little change is observed in biochemical parameters or bone mineral density over time. A subgroup of asymptomatic patients shows biochemical evidence of disease progression, although, in the author's series, no overt clinical complications developed. Surgical cure is associated with biochemical normalization and increased bone density. Sever

Therapeutic controversies in primary hyperparathyroidism.

There is little debate about the primacy of surgery in the management of classical PHPT. Rather, the question has been what to do about the many patients with nonclassical disease. A 1990 NIH consensus conference (55) clearly recommended surgery for patients with significant adverse effects of PHPT, for patients with complicating coexistent illnesses, for younger patients, and for those in whom consistent long-term follow-up could not be assured. It allowed that conscientious surveillance may be justified in patients with minimal hypercalcemia and no adverse effects, but it recognized that for

[Genetic aspects of primary hyperparathyroidism].

The results of the last decade genetic research in the field of oncogenesis of sporadic and familiar parathyroid tumors are summarised. The review of studied focuses on two main topics: multiple endocrine neoplasis syndromes as well as sporadic parathyroid adenomas and carcinomas. The current knowledge of genetic factors associated with the development of these lesions is presented.

Surgical treatment for primary hyperparathyroidism.

OBJECTIVE: To document the recent surgical results in the treatment of primary hyperparathyroidism (HPT) at Queen Mary Hospital. METHODS: From 1995 to 1996, 30 patients underwent 31 operations for primary HPT. The data of 11 men and 19 women with ages ranging from 19 to 86 years (median: 57 years) were prospectively recorded with emphasis on the need of preoperative localization. RESULTS: Symptoms of hypercalcemia were present in 20 (67%) and complications in 17 (57%) patients, respectively. Seventy-seven localization studies were performed in 28 patients (average: 2.6 tests/patient). Locali

Primary hyperparathyroidism: diagnosis and management.

OBJECTIVE: To review the manifestations of primary hyperparathyroidism and the available treatment strategies. METHODS: The epidemiologic and pathologic features of this endocrine disorder are outlined. Guidelines for surgical intervention are discussed, and the effectiveness of various medical treatments is summarized. RESULTS: Primary hyperparathyroidism, a common disorder, is diagnosed on the basis of hypercalcemia in the presence of increased parathyroid hormone levels as measured by immunoassay. Clinical manifestations are variable with many patients being asymptomatic at the time of di

[Primary hyperparathyroidism in elderly patients].

Retrospective analysis has been performed in 404 patients, undergoing initial operation for primary hyperparathyroidism for differences in clinical presentation, laboratory data, pathological findings and outcome of surgery. In elderly patients (> = 60 years of age, n = 116), neuromuscular and psychic symptoms, hypertension, cholelithiasis and pseudogout were significantly more common, while renal stones were less common than in younger patients. There were no significant difference in preoperative laboratory findings, parathyroid pathology and surgical cure rate. Parathyroidectomy could be sa

[Primary hyperparathyroidism associated with sarcoidosis].

Co-existence of primary hyperparathyroidism and sarcoidosis has been described in less than 50 cases since 1958. The mechanism of such an association is unknown. However, the steroid suppression test may be helpful in differentiating these two disorders. The hypercalcemia of sarcoidosis should be corrected with the steroid test. The measurement of parathyroid hormone (PTH) in serum is also helpful, for PTH levels are normal or reduced in sarcoidosis associated with hypercalcemia whereas in patients with primary hyperparathyroidism PTH levels are elevated.

[Persistent and recurrent primary hyperparathyroidism].

Aim of this study is to evaluate clinical, radiological and therapeutic aspects of patients who have persistent or recurrent primary hyperparathyroidism. First, it is essential to reconfirm the diagnosis. Then, the previous operative and pathology reports must be reviewed. In these patients, it is important to use preoperative localization studies. When the results of the various localization tests were combined, the site of the elusive remaining parathyroid tumor could be identified in more than 80%, enabling a focused surgical approach which not only saves time, but also improves the success

Primary hyperparathyroidism--an Indian study.

Twenty eight patients of sporadic primary hyperparathyroidism seen over a period of 10 years were studied. There were 18 females and 10 males with a mean age of 35.9 years. Bone involvement was the commonest clinical presentation (90%) followed by renal involvement (65%) and more than half the patients (54%) had involvement of both the skeletal and renal systems. The tumor was clinically palapable in six patients. Thalliumtechnetium subtraction scan had a sensitivity of 87% followed by computerised tomography (70%), and ultrasound (65%) in diagnosing parathyroid pathology. All the patients und

[Trabecular connectivity in primary hyperparathyroidism].

By bone histomorphometry, 10 cases of primary hyperparathyroidism (PHPT) were used to study the trabecular connectivity, which in a few literatures was mentioned to be reserved somehow. Our data suggest that in comparison with the normal control (10 subjects) the nodes (N.Nd), the number of terminus (N.Tm), the total strut length (TSL), the length between the nodes (Nd.Nd), trabecular separation (Tb.Sp) and trabecular thickness (Tb.Th) were significantly increased while no significance was found in N.Nd/N.Tm, Tm.Tm and TBV. The correlation analyses indicate that there is no correlation between

[Neuropsychiatric symptoms in primary hyperparathyroidism].

Over a period of 3 months a 51-year-old woman developed a depressive syndrome with loss of memory, inability to find words, impaired fine movements and phases of spacial disorientation. Biochemical tests demonstrated occasional slight rises in serum calcium (2.8-2.9 mmol/l) and parathormone (4.6-5.2 pmol/l). She had a grade II nodular goitre with normal thyroid function. Ultrasonography of the thyroid suggested a right cranial parathyroid adenoma, confirmed at a right thyroidectomy with resection of the right upper parathyroid, while the other parathyroid glands were grossly normal. Calcium an

[Chronic pancreatitis and primary hyperparathyroidism].

The frequency of acute or chronic pancreatitis in primary hyperparathyroidism has decreased from the former 5-10% to 1-2% thanks to earlier diagnosis and operative treatment. Chronic pancreatitis, which occurs only in prolonged primary hyperparathyroidism, should therefore virtually disappear. We investigated this topic in a prospective long term study of chronic pancreatitis (1963-1992). Over the last three decades 336 patients with chronic pancreatitis have been studied at regular intervals. 245 suffered from alcohol-induced (84% with calcifications) and 91 from non-alcohol-induced chronic p

[Primary hyperparathyroidism in 2 adolescents].

The authors report on two cases of primary hyperparathyroidism (PHP) in adolescents. In a 15-year-old boy, the clinical picture included skeletal, dental, renal and psychological manifestations. In the second case, the first symptom was urolithiasis at age 12; following a period without any important clinical event, the diagnosis was established when the patient was 18. In both cases, increased calcium and low phosphorus plasma levels associated to moderately elevated plasma parathyroid hormone levels were noted. CT scan and ultrasounds were normal but MRI showed an enlarged gland in the secon
