Hypermelanoses of the newborn and of the infant.

This article discusses hypermelanoses present at birth or appearing in the first months of life. They may be transient or permanent, localized - as in café-au-lait spots - or segmental, or more rarely, complex or generalized. In most pigmentary diseases, physical examination, including Wood's lamp examination and a detailed history, is usually sufficient. Time of onset, distribution pattern, and associated clinical and sometimes histopathologic findings are helpful in differentiating these disorders. Recently, molecular diagnosis has become available for some rare entities, such as hereditary

Stroke in newborn infants.

The few days before and after birth are a time of special risk for stroke in both mother and infant, probably related to activation of coagulation mechanisms in this critical period. Arterial ischaemic stroke around the time of birth is recognised in about one in 4000 full-term infants, and may present with neurological and systemic signs in the newborn. Neonatal seizures are most commonly the clinical finding that triggers assessment. In other children, perinatal stroke is recognised only retrospectively, with emerging hemiparesis or seizures after the early months of life. Risk factors for p

[Botulism in newborn infants].

Infant botulism, first described in 1976, is the most common form of botulism. The majority of cases are reported from the USA. The disease is rare in Europe, and this article describes the first patient reported in Norway. A three-month-old boy of Norwegian origin who had been fed Argentinian honey developed symptoms of botulism. Electromyography showed presynaptic neuromuscular dysfunction. The diagnosis was confirmed by the demonstration of Clostridium botulinum type A neurotoxin in the faeces. After supportive treatment, breast-milk feeding and lactobacillus supplementation he made a compl

Cholelithiasis in newborns and infants.

Cholelithiasis in infants is rare, and has usually been associated with hemolysis, ileal disease, congenital anomalies of the biliary tree, hyperalimentation, and prolonged fasting. With the increased use of abdominal ultrasonography (US), more cases of cholelithiasis are being discovered. We report our experience with 13 infants diagnosed on abdominal US to have gallstones. There were 9 boys and 4 girls with an average age at diagnosis of 2.6 months (range, 0 to 9 months). Predisposing factors could be identified in only 6 of the 13 patients. Two patients with obstructive jaundice underwent c

The effect of light treatment on neonates with jaundice was discovered in 1958. Since then, many hundred thousand infants have been treated with light. The presumed mechanism of effect is photo-excitation of bilirubin extravascularly in the skin with the formation of bilirubin isomers which can be excreted in unconjugated form in the bile and photo-oxidation products which can be excreted in the urine. The quantitative relationships have only been partially elucidated. Blue light is the most effective. Green light causes lowering of bilirubin but, on the basis of present knowledge, there is no

Transportation of newborn infants.

The benefit of an organized neonatal transport system is well established. Over a 30 month period, January 1987 to June 1989, 172 babies were transported to the Dublin maternity hospitals. Birth weights ranged from 640 to 5,180g 106 (62%) were less than 37 weeks gestation. Indications for transport included respiratory distress syndrome (54), prematurity only (43), convulsions and/or neurologic dysfunction (23), jaundice (11) and apnoea (11). One hundred and sixty-eight were transferred by ambulance and four by helicopter. Twenty travelled more than 100 miles. Forty (23%) received assisted ven

[Triploidy in newborn infants].

Human triploidy, a common condition occurring in about 1 to 2% of all clinically recognizable pregnancies, is a rare finding in live-born children. Not more than 44 live-born triploid infants have been reported in the available literature. Triploid infants surviving for more than a few days have been suspected to be hidden mosaicisms and mostly have turned out to be diploid-triploid mosaics. We observed two live-born female infants with a complete triploidy and describe the typical clinical picture.

[Hydronephrosis in newborn infants].

During a 20-year period the author observed 40 newborn infants with congenital hydronephrosis. In 28 patients hydronephrosis of the normally developed kidney and in 12- of the abnormal organ was noted. Twelve patients were hospitalized for developmental anomalies that necessitated urgent surgical intervention, the diagnosis of hydronephrosis in them being established at autopsy. Twenty eight children were admitted to the clinic with symptoms of hydronephrosis. All children were operated upon, nephrectomy was performed in 18 cases, in the remainder-organ-preserving and corrective operative proc
