Optimal use of the Ommaya reservoir in clinical oncology.

A major obstacle to the effective systemic therapy of intracranial malignancies and infections, the bloodbrain barrier was overcome by the development of the Ommaya reservoir in 1963. Initially used in treating cryptococcal meningitis, this closed system for continued access to the ventricular spaces has a variety of applications. They include treatment of cancer pain, chronic or recurrent CNS infection, prophylaxis of CNS involvement in acute lymphoblastic leukemia, and treatment of leptomeningeal malignancy. The authors outline the rationale, latest indications, surgical technique, and poten

Cyclohexyl adenosine protects against neuronal death following ischemia in the CA1 region of gerbil hippocampus.

Sixty-five male gerbils were exposed to 30 minutes of cerebral ischemia induced by a bilateral carotid artery occlusion. One group of 15 gerbils received a single injection of 25 microliter of 5 microM cyclohexyladenosine into the cerebral ventricle 15 minutes after release of the occlusion. Another group of 45 gerbils received a similar injection of the vehicle. Five days after ischemia, the hippocampal histology was examined under light microscopy. In the gerbils treated with the adenosine receptor agonist N-6-cyclohexyladenosine, the CA1 region of the hippocampus showed significant quantita

Emetic effects of centrally administered angiotensin II, arginine vasopressin and neurotensin in the dog.

Short-latency emetic responses were induced in dogs by injecting angiotensin II (AII), arginine vasopressin (AVP), and neurotensin (NTN) into cerebroventricular (ICV) and cisternal (ICT) sites also responsive to the emetic effects of apomorphine (APO). Angiotensin III, bradykinin, bombesin, oxytocin, adrenocorticotropic hormone, substance P, gastrin-related peptide and cholecystokinin were ineffective. The results suggest a possible dopaminergic mediation of peptide-induced emesis by receptors in the area postrema (AP).

Intracerebroventricular infusions of corticotrophin releasing factor (CRF) and ACTH raise blood pressure in sheep.

Infusion of synthetic ovine CRF (10 or 100 micrograms/h) into the lateral lateral cerebral ventricle for 24 h increased mean arterial blood pressure of conscious sheep. CRF infusion also increased urine output and sodium excretion. Intravenous infusion of CRF (100 micrograms/h) or intraventricular infusion of artificial CSF had no effect on blood pressure. Intraventricular infusion of ACTH (1-24) at 0.5 micrograms/kg per day, a rate of infusion which has no systemic effect on blood pressure, also raised mean arterial pressure. These studies suggest that two peptides involved in the physiologic

Intraventricular morphine for control of pain in terminal cancer patients.

Satisfactory control of intractable pain has been achieved in 17 terminal cancer patients by injecting small doses of morphine into the lateral cerebral ventricle via an Ommaya reservoir. Pain relief together with a favorable behavioral response was obtained without interference with other sensory modalities, noticeable physical changes, or side effects annoying or severe enough for the patient to discontinue therapy. Eleven patients developed tolerance, but this phenomenon does not require withdrawal of treatment. Chronic intraventricular morphine administration can be safely performed on an

Parkinsonism associated with intraventricular amphotericin B.

Little information is available regarding the in-vivo effects of amphotericin B on organs other than the kidney. The increasing use of intrathecal amphotericin B has resulted in several reports of neurotoxicity associated with the drug. Development of parkinsonism following intraventricular treatment with amphotericin B for cryptococcal meningitis in a young woman suggested a direct toxic effect on nervous tissue by amphotericin B. Although transient signs of parkinsonism have been described in a patient receiving intraventricular amphotericin B, persistent parkinsonism is an unprecedented occ

Intraventricular morphine produces pain relief, hypothermia, hyperglycaemia and increased prolactin and growth hormone levels in patients with cancer pain.

The effects of analgesic, thermoregulatory and endocrine functions of administering morphine sulphate (0.3 mg) into the lateral cerebral ventricle via an Ommaya catheter were assessed in eight patients with cancer pain. Satisfactory control of intractable pain was obtained in these patients, without any change in other sensory modalities. The delay in the onset of pain relief and the duration of analgesia ranged, respectively, from 20 to 40 min and from 12 to 16 h after drug injection. In addition, intraventricular administration of morphine caused a reduction in rectal temperature in these pa

Nerve growth factor prevents the amblyopic effects of monocular deprivation.

Monocular deprivation early in life causes dramatic changes in the functional organization of mammalian visual cortex and severe reduction in visual acuity and contrast sensitivity of the deprived eye. We tested whether or not these changes could be from competition between the afferents from the two eyes for a target-derived neurotrophic factor. Rats monocularly deprived during early postnatal development were treated with repetitive intraventricular injections or topical administration of nerve growth factor. The effects of monocular deprivation were then assessed electrophysiologically. In

The role of an Ommaya reservoir in the management of children with cryptococcal meningitis.

Cryptococcal meningitis is the most common life-threatening fungal infection and is associated with high mortality in children. Amphotericin B plus flucytosine and fluconazole is the optimal current therapy. Implantation of an Ommaya reservoir for intraventricular infusion of medication and aspiration of cerebrospinal fluid (CSF) for the treatment of increased intracranial pressure (ICP) has been reported. Intraventricular injection of amphotericin B through an Ommaya reservoir in children with cryptococcal meningitis has not been reported previously. We report two children who had cryptococca

Effect of intraventricular perfusion of angiotensin II in conscious normal rats and in rats with hereditary hypothalamic diabetes insipidus.

1. Artificial cerebrospinal fluid was perfused through the cerebral ventricles of conscious rats. A basal secretion rate of 16 +/- 3 X 10(-15) mol of immunoreactive angiotensin II/min was calculated for intact rats. 2. Most of the immunoreactive angiotensin II consisted probably of the heptapeptide or pentapeptide angiotensin II fragments. 3. The pressor response to intraventricular perfusions of angiotensin II were normal in Long-Evans rats, virtually absent in rats homozygous for hereditary hypothalamic diabetes insipidus, irrespective of whether they were injected with vasopressin tannate o

Further studies of blood infectivity in an experimental model of transmissible spongiform encephalopathy, with an explanation of why blood components do not transmit Creutzfeldt-Jakob disease in humans.

BACKGROUND: Solid evidence from experimentally infected animals and fragmentary evidence from naturally infected humans indicate that blood may contain low levels of the infectious agent of Creutzfeldt-Jakob disease (CJD), yet blood components have never been identified as a cause of CJD in humans. STUDY DESIGN AND METHODS: Blood components and plasma fractions were prepared from the pooled blood of mice that had earlier been infected with a mouse-adapted strain of human transmissible spongiform encephalopathy (TSE). Infectivity bioassays were conducted in healthy mice, and the brains of all

Hypothalamic site-dependent effects of neuropeptide Y on gonadotropin-releasing hormone secretion in rhesus macaques.

In rodents and rabbits, neuropeptide Y (NPY) has a bimodal effect on gonadotropin-releasing hormone (GnRH) secretion. Intracerebroventricular (icv) administration or direct infusion of NPY into the median eminence (ime) suppresses GnRH release in ovariectomized (OVX) animals, but stimulates GnRH release in intact or OVX animals treated with ovarian steroids. Specific ovarian steroid-dependent NPY effects are, however, not obvious in non-human primates. In OVX rhesus monkeys, icv administration of NPY has been shown to suppress luteinizing hormone (LH) secretion whereas ime infusion of NPY stim

Cerebral subarachnoid sampling of cerebrospinal fluid in the rhesus monkey.

A previously described rhesus monkey model with two intraventricular catheter systems was expanded to provide a means of studying drug concentrations of chemotherapeutic agents such as methotrexate (MTX) in the cerebral subarachnoid cerebrospinal fluid (CSF) following intrathecal drug injection. A continuous intraventricular infusion of MTX was started through the lateral ventricular catheter 44 h before surgical placement of the cerebral subarachnoid catheter to allow for steady-state concentrations to be attained throughout the subarachnoid space. Monkeys were anesthetized intramuscularly wi

Morphine and morphiceptin increase the threshold for epinephrine-induced cardiac arrhythmias in the rat through brain mu opioid receptors.

1. To determine whether morphine modulates the development of cardiac arrhythmias through mu opioid receptors by an action within the central nervous system (CNS). Catecholamine-induced ventricular arrhythmias were produced, in the rat, by continuous infusion of epinephrine at incremental doses until the development of fatal arrhythmias, usually ventricular fibrillation. 2. Morphine, 0.1 mg/kg i.v., significantly suppressed (P < 0.05) the development of epinephrine-induced arrhythmias compared with the control group. This was opposed by the mu opioid antagonist naloxone (1 or 2 mg/kg) in a dos

Central pressor effects induced by muscarinic receptor agonists: evidence for a predominant role of the M2 receptor subtype.

The cardiovascular effects induced in the rat by several muscarinic receptor agonists were studied. All the agonists produced a clear decrease in heart rate. This decrease appeared to be peripherally mediated, because it was antagonized by methylscopolamine. The effects on blood pressure varied depending on the presence of anaesthesia, previous treatments and the type of agonists tested. When peripheral muscarinic activity was blocked by administration of methylscopolamine, a dose-dependent hypertension was obtained following the injection of oxotremorine, arecoline and aceclidine, by both int

[Mechanism of action of the analgesic flupirtine].

To answer the questions of mode and site of action partly supplementary, partly new investigations with flupirtine (Katadolon) were carried out which are described below. The investigation for opiate receptor affinity of flupirtine in rat brain homogenate did not show any reduction in 3He-etorphine binding up to the highest concentration of flupirtine of 10(-5) mol/1. This result suggests that flupirtine either has a very low opiate receptor affinity or lacks it fully. Therefore the analgesic activity of flupirtine is not based on opiate mechanism. The intracerebroventricular and intrathecal a

Feeding and drinking responses in the golden hamster following treatment with cholecystokinin and angiotensin II.

Drinking and feeding behaviours of female golden hamsters were examined following intracerebroventricular injections of angiotensin II or systemic and intracerebroventricular injections of cholecystokinin octapeptide. Injections of angiotensin II into the brain produced a dose-dependent drinking response in water repleted animals. Systemically, a low dose (0.5 microgram/kg body wt) of cholecystokinin was ineffective at reducing food intake of fasted animals during a 1 hr test. Larger peripheral doses (1.0 to 4.0 microgram/kg body wt), however, were effective at decreasing food intake. Injected
