[Health status and medical care accessibility of single, homeless persons].

The homeless population in Germany is continually increasing. Featuring prominently among those on the increase are women, young persons and homeless people from East Germany. Studies of the health of homeless individuals in recent years show that indices of illness are far higher for many disorders than for comparable groups who are housed. One result from a recent study by the University of Mainz (1994) was that more than 90% of homeless people urgently need medical treatment. According this research, the main health problems of the homeless are: cardiac disease (hypertension, CAD) (52.5%),

Exploring patient's clinical outcomes, hospital costs, and satisfaction after the implementation of integrated clinical pathway-based nursing practice model.

INTRODUCTION: Hospitals provide essential health services, focusing on quality, safety, and patient-centered care. The rising prevalence of communicable and non-communicable diseases in Indonesia has led to increased National Health Insurance (NHI) costs, with communicable diseases representing a significant burden despite global progress in disease control. Tuberculosis remains a major global health concern, causing 1.25 million deaths in 2023, while Indonesia ranks second globally for tuberculosis burden, accounting for 10% of global TB cases with an estimated 969,000 cases annually. Despite

Bridging evidence and practice in heart failure care: barriers and pragmatic solutions to guideline-directed medical therapy implementation in India and other low-resource settings.

Heart failure with reduced ejection fraction (HFrEF) remains a leading cause of morbidity, hospitalization, and mortality in low- and middle-income countries (LMICs). Guideline-directed medical therapy (GDMT) including angiotensin receptor-neprilysin inhibitors (ARNIs), evidence-based beta-blockers, mineralocorticoid receptor antagonists (MRAs), and sodium-glucose co-transporter 2 (SGLT2) inhibitors has consistently demonstrated reductions in mortality and heart failure (HF) hospitalizations. Despite strong evidence, real-world implementation of GDMT in LMICs remains suboptimal. This article s

Home health care for mechanical ventilation-dependent patients: A grounded theory study.

The number of people requiring home mechanical ventilation is increasing. Both family and patients experience a quite different situation. There is a significant knowledge gap about the caring process, families and health care team responsibilities, challenges they face, and how they manage. This study aimed to discover the process of home health care (HHC) to mechanical ventilation-dependent patients. The Straus and Corbin version of the grounded theory method was used. The preliminary study was conducted in 2015, and then for updating the theory, a complementary study was done in 2019. Sampl

Risk of mortality in older adults with loss of appetite: An analysis of Medicare fee-for-service data.

OBJECTIVES: Prior research suggested that loss of appetite (LOA) among adults with Medicare fee-for-service (FFS) insurance in the United States increased the risk of mortality within 1 year; those findings were not adjusted for risk factors and confounders. The objective of this study was to compare the risk of mortality among Medicare FFS beneficiaries with LOA to a control group without LOA while controlling or adjusting for age, comorbidities, body mass index (BMI), and weight loss. DESIGN: Retrospective and observational analysis of Medicare FFS health insurance claims data from October

Late-week surgery and discharge to specialty care associated with higher costs and longer lengths of stay after elective lumbar laminectomy.

OBJECTIVE: In a healthcare landscape in which costs increasingly matter, the authors sought to distinguish among the clinical and nonclinical drivers of patient length of stay (LOS) in the hospital following elective lumbar laminectomy-a common spinal surgery that may be reimbursed using bundled payments-and to understand their relationships with patient outcomes and costs. METHODS: Patients ≥ 18 years of age undergoing laminectomy surgery for degenerative lumbar spinal stenosis within the Cleveland Clinic health system between March 1, 2016, and February 1, 2019, were included in this analys

[Severity assessment strategies based on administrative data using stroke as an example].

BACKGROUND AND OBJECTIVES: Information on disease severity is relevant for many studies with claims data in health service research, but only limited information is available in routine data. Stroke serves as an example to analyse whether the combination of different information in claims data can provide insight into the severity of a disease. METHOD: As a first step, a literature search was conducted. Strategies to assess the severity of a disease by means of routine data were examined with regard to approval and applicability to German sickness fund data. In order to apply and extend the i

Descriptive Analysis of the First 12-Month Activity of Docadom, a Private Physician-Led Urgent In-Home Consultation Service.

Background: Emergency services in urban areas are overcrowded due to a rise in mild-to-moderate acuity consultations, and physician-led home visits are less common. In response, a home-based urgent care model in an urban setting (Docadom) was developed. Consultation requests via a dedicated mobile application or phone call are triaged by a trained emergency nurse. Physicians travel to the patient via an electric cargo bike that has storage for medical equipment. During home visits, physicians perform testing/diagnosis and advanced procedures (e.g., intravenous medication delivery) and prescrib

Regional Factors and Ambulatory Care-Sensitive Condition Hospitalizations in Older Japanese Adults.

IMPORTANCE: Hospitalization in older adults leads to a decline in daily living activities, and countries experiencing demographic aging face an increase in hospitalization rates and health care costs. Health care systems that can reduce the probability of ambulatory care-sensitive condition (ACSC) hospitalization among older populations are needed. OBJECTIVE: To comprehensively assess the association between regional factors and the probability of ACSC hospitalization among older adults and the extent to which regional factors explain this probability. DESIGN, SETTING, AND PARTICIPANTS: A re

Predictors for non-response to PROMs across three areas of inpatient rehabilitation - a nationwide cross-sectional study in Switzerland.

BACKGROUND: Patient-reported outcome measures (PROMs) are valuable tools to integrate the patient perspective and improve quality of rehabilitation services. However, there is a risk of non-response bias. Identifying characteristics associated with non-response is important to reduce barriers to PROM completion and to ensure quality care for all patients undergoing rehabilitation. METHODOLOGY: In this cross-sectional observational study, we used routine data from the Swiss national quality assurance program for inpatient rehabilitation, in which PROMs are employed at admission and discharge f

Healthcare Use and Expenditures in Rural Survivors of Hospitalization for Sepsis.

OBJECTIVES: Sepsis survivors have greater healthcare use than those surviving hospitalizations for other reasons, yet factors associated with greater healthcare use in this population remain ill-defined. Rural Americans are older, have more chronic illnesses, and face unique barriers to healthcare access, which could affect postsepsis healthcare use. Therefore, we compared healthcare use and expenditures among rural and urban sepsis survivors. We hypothesized that rural survivors would have greater healthcare use and expenditures. DESIGN, SETTING, AND PATIENTS: To test this hypothesis, we use

[BPSD from the Perspective of the New Orange Plan].

I have discussed BPSD, especially who should treat BPSD and who can treat them, from the viewpoint of the New Orange Plan. It is desirable for all the doctors to have extensive knowledge about dementia and engage in treatment in cooperation with other departments and, more comprehensively, with nursing-care insurance fields. During the period when a patient has mild BPSD and the burden on caretakers is light, it is possible for his or her family doctor to treat BPSD. However, when a patient has severe BPSD and is in a situation where care is difficult, non-drug therapy often becomes the first

Patterns of utilization and determinants of maternal health services among women residing in low-income communities in Lagos State, Nigeria.

Maternal and perinatal morbidity and mortality can be significantly prevented when women utilize health facilities for antenatal care (ANC), delivery and postnatal care; particularly in low-income communities in sub-Saharan Africa, where facility-based maternal care is underutilized. This study assessed the pattern of utilization and determinants of uptake of maternal health services among women residing in low-income communities in Lagos State, Nigeria. This was a mixed methods cross-sectional study, among women of reproductive age 15 - 49 years. Quantitative data was collected from 3,651 wom

Temporal Patterns of Sexual Assault Presentations in U.S. Emergency Departments: Implications for Staffing and Forensic Readiness.

BACKGROUND: Sexual assault (SA) remains a major public health issue, yet only a fraction of survivors seeks acute medical care in emergency departments (EDs). Although previous studies describe demographic disparities among SA survivors, national patterns of ED arrival timing and their operational implications for emergency care systems remain poorly characterized. STUDY OBJECTIVES: To characterize the demographic and temporal patterns of ED presentations for SA in the United States. METHODS: We conducted a repeated cross-sectional study using the Epic Cosmos database, including U.S. ED enco
