Outbreak of pertussis in a neonatal intensive care unit--Louisiana, 2004.

BACKGROUND: On November 12, 2004, a 5-month-old infant, admitted in the neonatal intensive care unit (NICU) of a Louisiana regional hospital since birth, was diagnosed with pertussis. Measures to prevent further transmission, in the NICU and beyond, were immediately put into place. METHODS: Exposed contacts were identified among other patients of the NICU, health care workers, and family members. All were offered pertussis testing and prophylactic treatment. RESULTS: The source of the outbreak was not identified. Despite the immediate implementation of control measures, a total of 37 additio

Auditory neuropathy: a potentially under-recognized neonatal intensive care unit sequela.

Auditory neuropathy (AN) is a hearing disorder that affects newborns. Those with high-risk neonatal histories, family history of childhood hearing loss, and hyperbilirubinemia are at greatest risk. Current neonatal intensive care unit (NICU) hearing screening methods that rely only on otoacoustic emissions will fail to detect this disorder. Auditory neuropathy differs from conductive hearing loss and sensorineural hearing loss; a specific constellation of findings on audiologic evaluation are diagnostic of this disorder. The pathophysiology of AN is unclear; however, it may be caused by demyel

Use of furosemide and hearing loss in neonatal intensive care survivors.

Hearing loss is a significant morbidity in survivors of the neonatal intensive care unit (NICU). The overall prevalence of hearing loss in neonates is 0.93 per 1000 live births, whereas in neonates weighing less than 2000 g, it is as high as 15.5 per 1000 live births. The increased incidence of hearing loss in NICU graduates has been attributed to their underlying disease process as well as exposure to ototoxic drugs including furosemide. A retrospective chart review of all neonatal intensive care survivors was done to evaluate the potential effect of furosemide on hearing loss. From July 2000

Comparison of beractant and calfactant in a neonatal intensive care unit.

PURPOSE: The relative efficacy, complications and cost of beractant and calfactant were studied. METHODS: All patients admitted to a neonatal intensive care unit between January and November 2001 with an estimated gestational age (EGA) of < or = 29 weeks were included in this retrospective study. Neonates with an EGA of > 29 weeks who required surfactant treatment because of respiratory distress syndrome (RDS) were also enrolled. The prophylaxis group included all neonates with an EGA of < or = 29 weeks who received a prophylactic dose of surfactant within one hour of birth. The rescue group

Yeasts species distribution in Neonatal Intensive Care Units in northeast Argentina.

The aim of this study was to determine the distribution and antifungal susceptibility profile of yeast species isolated from neonates in Neonatal Intensive Care Units (NICU) in northeast of Argentina. With this purpose 92 strains isolated from 25 blood stream cultures, 20 venous catheters, 23 suprapubic aspirations and 24 rectal swabs were studied. Candida albicans and C. parapsilosis appeared with similar frequencies (36%) in blood stream isolates. Candida parapsilosis (50%) was the most frequent catheters colonizer and C. tropicalis (54.2%) was the most frequent yeast associated with gastroi

Association of outcomes with organizational characteristics of neonatal intensive care units.

OBJECTIVE: Health outcomes may be influenced by organizational and management characteristics. We hypothesized that neonatal intensive care unit managerial practices and organizational processes could affect outcomes of births 500-1499 g, and we assessed this in eight neonatal intensive care units. DESIGN: Multiple-center cohort. SETTING: Eight acute care neonatal intensive care units in Washington, DC. PATIENTS: Infants weighing 500-1499 g, born from October 1, 1994, to February 19, 1997, to a resident of Washington, DC. INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: Outcomes includ

Hospital-acquired infections in the neonatal intensive care unit--Klebsiella pneumoniae.

Klebsiella pneumoniae is medically the most important organism of the Klebsiella species. It is responsible for a significant proportion of hospital-acquired infections including septicemias, urinary tract infections, pneumonia, and soft tissue infections especially in the immunocompromised hosts such as the neonate. The hands of healthcare workers and the gastrointestinal tract of hospitalized infants serve as reservoirs for the transmission of the organism and are responsible for multiple hospital outbreaks. In recent years, there has been an increase in the incidence of outbreaks caused by

Evaluation and treatment of thrombocytopenia in the neonatal intensive care unit.

UNLABELLED: Phlebotomy-induced anaemia excepted, thrombocytopenia is the most common haematological abnormality in neonatal intensive care unit (NICU) patients. Roughly one-quarter of all NICU patients and half of all sick preterm neonates develop thrombocytopenia. Whereas a large number of varied precipitating conditions has been identified, early-onset thrombocytopenia (<72 h) is most commonly associated with fetomaternal conditions complicated by placental insufficiency and/or fetal hypoxia, e.g. maternal pre-eclampsia and fetal intrauterine growth restriction. The resulting neonatal thromb

Use of the bleeding time in the neonatal intensive care unit.

UNLABELLED: Impairments of primary hemostasis are frequently responsible for serious bleeding in sick infants in the neonatal intensive care unit (NICU). Therefore, a rational approach to these infants with hemorrhagic manifestations, in addition to an accurate medical history, a careful physical examination and routine screening coagulation tests, may include a bleeding time. The bleeding time is the traditional in vivo test for assessing primary hemostasis. It is a useful clinical tool to detect quantitative or qualitative platelet disorders or microvascular defectiveness. Its diagnostic val

Evaluation and treatment of thrombosis in the neonatal intensive care unit.

Thromboses are infrequent but serious complications of patients in the NICU. Thromboses tend to occur in very sick neonates, particularly preterm neonates, and the majority of such thromboses are related to central vascular catheters. Other risk factors for neonatal thromboses include infants of diabetic mothers, sepsis, small for gestational age, congenital heart disease, maternal antiphospholipid syndrome, and possibly inherited prothrombotic disorders. Appropriate treatment, dosage, and duration of therapy for neonatal thromboses has not been studied in clinical trials. Treatment options in

Ureaplasma urealyticum and pulmonary outcome in a neonatal intensive care population.

OBJECTIVE: To determine whether the presence of Ureaplasma urealyticum in the respiratory tracts of very low birth weight infants is associated with increased risk of pneumonia, radiographic evidence of severe bronchopulmonary dysplasia during the second or third week of life (precocious) and oxygen dependency at 36 weeks of corrected postnatal gestational age. METHODS: From October, 1993, to January, 1996, all infants who met the following entry criteria were enrolled in the study: birth weights < or = 1500 g; admission to the NICU within 24 h after birth; evidence on admission of respirator

Retinopathy of prematurity in a south Australian neonatal intensive care unit.

PURPOSE: To establish the incidence and severity of retinopathy of prematurity (ROP) in an Australian population of premature infants, and define risk factors for this population. METHODS: A survey of neonates born weighing less than 1501 g and/or with gestational age below 33 weeks, was undertaken at a neonatal intensive care unit in South Australia. RESULTS: ROP was diagnosed in 16.0% of the 94 neonates who were screened until retinal vascularisation was complete. Threshold disease occurred in 4.2%. Logistic regression identified three significant risk factors for the development of ROP: d

The use of evoked potentials in the neonatal intensive care unit.

The data reported in the literature and our own findings have shown, that evoked potentials (EPs) can be performed at the bedside and provide additional information about the integrity of the central nervous system. Auditory brainstem responses (ABRs) are especially useful for early identification of audiological problems and whenever possible, at risk newborn infants (table II) should be screened using either ABRs or other methods like the otoacoustic emission before discharge. ABRs appear to be less useful in predicting neurological impairment, especially in the preterm infant. Visual evoked

Epidemiology of Pseudomonas aeruginosa infections in a neonatal intensive care unit.

During the 19-month study period, 48 (2 per cent) of the 2177 neonates admitted to the neonatal intensive care unit (NICU) yielded Pseudomonas aeruginosa growths in blood cultures. All these neonates had clinical and haematological evidences of sepsis. Prominent clinical features included sclerema, violaceus necrotic patches, necrotizing enterocolitis (NEC), conjugated hyperbilirubinaemia, and DIC. Over all mortality was 23 per cent, distinctly higher in premature neonates with RDS. The mean gestational age and birth weights (+/- SD) of these neonates were 36.42 (+/- 2.73) weeks and 2173.34 (+

Drug utilization in very premature infants in neonatal intensive care units.

Neonatal drug utilization in very premature infants (gestational age (GA) 24-29 weeks), requiring intubation and mechanical ventilation at birth was registered as part of a multicenter controlled clinical trial of high-dose versus low-dose bovine surfactant (initial doses 100 mg/kg birth weight (b.w.) versus 50 mg/kg b.w.). Drug utilization during 4 weeks after birth was analyzed in 164 infants (mean GA 27.2 +/- 1.2 (SD) weeks, b.w. 970 +/- 145 g (SD)). More than half of the study infants received antibiotics (98.8%), sedatives and analgesics (91.5%), sodium bicarbonate (78%), solutions for vo

Five years of cefotaxime use in a neonatal intensive care unit.

In 1983 our antibiotic regimen for suspected neonatal septicemia was changed from amoxicillin-gentamicin to cefotaxime-amoxicillin. During the subsequent 5-year period we studied the effect of this change in regimen on the bacterial flora of the infants in the unit and the occurrence of serious infections. This was done with bacteriologic surveillance and analysis of the positive blood cultures from 1978 through 1987. A change in the relative numbers of isolated pathogens was observed; Klebsiella sp. and Escherichia coli decreased whereas Enterobacter sp. increased. The susceptibility of the E
