Optic nerve sheath diameter (ONSD) is used clinically as a noninvasive measure for elevated intracranial pressure (ICP). This study had two purposes: to investigate the immediate effects optic nerve sheath (ONS) dilation post-ICP increase on trabecular fibers connecting the optic nerve to the ONS and to document any changes in these fibers 30 days post-increased ICP. In a swine model, ICP was increased by inflating a Foley catheter balloon in the epidural space. Three control pigs received the catheter insertion without inflation (no increase in ICP) and four experimental pigs received the cat…

BACKGROUND: Acetazolamide has become a standard treatment for cerebrospinal fluid (CSF) leaks associated with intracranial hypertension. The purpose of the current study was to evaluate the effectiveness of acetazolamide at decreasing elevated CSF pressure in this patient population.
METHODS: Prospective evaluation and data collection of high intracranial pressure CSF leaks was performed. Subjects underwent CSF diversion and postoperative assessment of pressure changes via a standard protocol. Lumbar drains or ventriculostomies were clamped on postoperative day 2 for 4 hours prior to assessme…

Raised intracranial pressure is a life threatening condition; unless recognized and treated early, it may progress into herniation syndrome and death. Symptoms and signs are neither sufficiently sensitive nor specific, hence a high index of suspicion and vigilance are needed for early recognition. Immediate goal of management is to prevent / reverse herniation and to maintain good cerebral perfusion pressure. The therapeutic measures include stabilization of airway, breathing and circulation, along with neutral neck position, head end elevation by 30°, adequate sedation and analgesia, minimal …

We report continuous bilateral intracranial pressure (ICP) monitoring immediately after transtentorial herniation in a patient with massive cerebral infarction to: 1) determine presence and time course of compartmental ICP differences, and 2) to study effects of therapy on both hemispheres. A 55-year-old man admitted with watershed infarctions in the left anterior-middle-posterior cerebral arteries distribution. Initial investigations demonstrated highly narrowed left extracranial internal carotid artery. Eight days later he developed unexplained lethargy and anisocoria. Head computerized tomo…

Therapy of cryptococcal meningitis has been focused primarily on optimizing the antifungal regimen to improve the previously high treatment failure rates. Until recently, relatively little attention has been paid to the impact of increased intracranial pressure, which is a frequent problem that complicates management of patients with cryptococcal meningitis. Patients with elevated baseline opening pressures have higher titers of cerebrospinal fluid cryptococcal capsular polysaccharide antigen, more frequent headaches, meningismus, papilledema, hearing loss, and pathologic reflexes, as well as …

Under physiological conditions, the pressure in the central retinal vein is equal to or higher than the intracranial pressure (ICP) because the cerebrospinal fluid (CSF) passes the sheath of the optic nerve before draining into the cavernous sinus. The optic nerve sheath is where the ICP affects the retinal venous pressure. Ophthalmodynamometry (ODM) is a useful method for determining the central retinal artery pressure. While papilledema and a lack of venous pulsations are commonly used as a vague indication of the ICP, ODM may be advantageous for determining the pressure in the central retin…

Most cases of craniosynostosis are diagnosed during early infancy, but occasionally craniosynostosis evolves with minimal cranial involvement and goes unnoticed until late childhood. Seemingly these mild forms of craniosynostosis cause few, if any, symptoms of neurological involvement. We describe the cases of a 9-year-old girl and a 6-year-old boy who presented with evident signs of raised intracranial pressure (ICP), together with a negligible skull deformity. We have termed these cases as occult craniosynostosis. Differential diagnosis in our patients was established against known causes of…

Two achondroplastic children with ventriculomegaly are reported. The patients had no signs of increased intracranial pressure, but in one blindness had previously been detected by the parents. Neuroradiological examinations showed ventriculomegaly in both. Intracranial pressure remained at high levels (20-55 mmHg) preoperatively; ventriculo-peritoneal shunting was performed in both cases, and postoperatively levels fell to normal (5-16 mmHg). The need for the treatment of ventriculomegaly seen in achondroplastic children is analyzed on the basis of these two patients, with a brief review of li…

Elevated intracranial pressure is a serious complication in the early phase after brain infarction. The most frequent cause of the elevated pressure is oedema following an infarction in the territory of the middle cerebral artery. Non-surgical treatment is inadequate in many of the patients. Although anecdotal, several reports concerning neurosurgical treatment seem to be optimistic. We think that such treatment could be given to selected young patients with large hemispheric infarctions and cerebral oedema.

An analysis of intracranial pressure (ICP), based on an examination of the temporary correlation between the changes in amplitude of the pulse wave and the mean ICP level, is presented. The paper contains a discussion of the preliminary results of the method when applied to the analysis of ICP as monitored during infusion tests in a group of 24 children. Infusion of a certain volume of CSF is a good example of an uncompensated volume process, introduced externally into the intracranial space. Results allow an interpretation of the short term correlation coefficient RAP (correlation coefficient…

Through a study of some syndromes of intra-cranial high and low pressure, the authors try to find out the relations between the intra-crania … y. They show that in these syndromes, the development of frontal sinuses is an inverse ratio to the intracranial pressure. However, they point out that in standard people, the extreme differences in the morphology of their frontal pneumatization can be explained not only by various causes--a number of which is as yet not known--but also by an aleatory distribution.

A patient returns from the operating room with an intracranial pressure (ICP) of 25 mm Hg as measured by an intraventricular catheter. What measures will decrease his ICP, what is the most threatening risk of an intraventricular catheter, and what nursing measures will decrease those risks? The clinical nurse caring for a patient with an intraventricular catheter must understand the dynamics of intracranial anatomy and physiology as well as the methods and rationale for medical and nursing management. An adequate knowledge base of each aspect of the intraventricular catheter and drainage syste…

Two patients who became comatose with Reye syndrome had continuous monitoring of intracranial pressure and blood pressure. Respirations were controlled, and mannitol and urea were administered to control elevated intracranial pressure. Continuous intracranial pressure monitoring proved to be a valuable adjunct to therapy for Reye syndrome.
