Cauda equina compression due to a laminar hook: A late complication of posterior instrumentation in scoliosis surgery.

Whilst neurologic injury following correction of scoliosis with CD-instrumentation is generally known to be an early complication, any late occurrence of cauda compression secondary to employment of a laminar hook-rod construct is exceptional. We report on such a rare case of late occurrence of cauda equina syndrome, when a laminar hook at level L2 became symptomatic causing compression of the cauda equina almost a decade after spine surgery. This case demonstrates that one should not only be aware of a potential neural injury at intraoperative placement of laminar hooks, but also one is remin

Liver transplantation complicated by malpositioned transjugular intrahepatic portosystemic shunts.

PURPOSE: To report the surgical problems encountered during orthotopic liver transplantation as a result of a malpositioned transjugular intrahepatic portosystemic shunt (TIPS). PATIENTS AND METHODS: Three patients are described in whom TIPS stents were malpositioned in the following locations: extending into the main portal vein, extending into the suprahepatic inferior vena cava, and extending into the right atrium. RESULTS: Malpositioning of TIPS stents altered and prolonged the operation in all of these patients by interfering with cross-clamping at the usual vascular sites during liver

[Risk in anaesthesia].

BACKGROUND: Modern anaesthesia is still associated with a risk of serious complications. This article focuses on frequency, causes, and prevention of the most important anaesthetic complications. MATERIAL AND METHODS: The article is based on literature identified through a non-systematic search in Pub-Med, and the author's research and experience in this field. RESULTS: The risk of death associated with anaesthesia is closely related to patient age and physical status. In otherwise healthy patients (ASA 1), the risk of such deaths is approximately 1:250 000. Medication errors occur in approx

Intra-operative awareness in children and post-traumatic stress disorder.

Adults who experience intra-operative awareness can develop disturbing long-lasting after-effects, such as daytime anxiety, sleep disturbances, nightmares, flashbacks and, in the worst case, a post-traumatic stress disorder (PTSD). It is unknown whether intra-operative awareness has a similar psychological impact in children. We designed the present study in order to evaluate the incidence of psychological symptoms in children who had either confirmed or possible intra-operative awareness. Attempts were made to locate 11 children who had been identified in a previous study, approximately 1 yea

A review of major reconstructive and gender-affirming surgical techniques in genital surgery and care for the transgender patient population.

The transgender community makes up a significant portion of the American population, yet there is a paucity of data available to surgeons and anesthesiologists. The purpose of this article is to provide information to surgeons, anesthesiologists, and other medical professionals. This narrative review explores the importance of medical information that healthcare professionals should be aware of ahead of gender-affirming surgeries. This review paper's purpose is to explore perioperative considerations for physicians before gender-affirming surgeries. Physicians should be aware of the hormone th

Bispectral index monitoring, duration of bispectral index below 45, patient risk factors, and intermediate-term mortality after noncardiac surgery in the B-Unaware Trial.

BACKGROUND: Postoperative mortality has been associated with cumulative anesthetic duration below an arbitrary processed electroencephalographic threshold (bispectral index [BIS] <45). This substudy of the B-Unaware Trial tested whether cumulative duration of BIS values lower than 45, cumulative anesthetic dose, comorbidities, or intraoperative events were independently associated with postoperative mortality. METHODS: The authors studied 1,473 patients (mean ± SD age, 57.9 ± 14.4 yr; 749 men) who underwent noncardiac surgery at Barnes-Jewish Hospital in St. Louis, Missouri. Multivariable Cox

Acute Ocular Pain Following Endoscopic Mastectomy: A Case Report.

Endoscopic breast surgery (EBS) is a widely adopted minimally invasive technique that offers improved cosmetic outcomes and faster recovery. However, the use of carbon dioxide insufflation can occasionally result in unexpected complications. We report a rare case of acute postoperative ocular pain with suspected transient intraocular pressure elevation following EBS in a healthy 36-year-old female. The symptoms were likely due to CO₂ diffusion into subcutaneous tissue and Trendelenburg positioning, which contributed to venous congestion and elevated episcleral venous pressure. Conservative man

Delirium Prevention and Management in Frail Surgical Patients.

Delirium, an acute, fluctuating impairment in cognition and awareness, is one of the most common causes of postoperative brain dysfunction. It is associated with increased hospital length of stay, health care costs, and mortality. There is no FDA-approved treatment of delirium, and management relies on symptomatic control. Several preventative techniques have been proposed, including the choice of anesthetic agent, preoperative testing, and intraoperative monitoring. Frailty, a state of increased vulnerability to adverse events, is an independent and potentially modifiable risk factor for the

Oral motor dysfunction after carotid endarterectomy: Challenges in diagnosing marginal mandibular nerve injury and effectiveness of rehabilitation.

Carotid endarterectomy (CEA) is a frequently used surgical treatment for carotid artery stenosis. Cranial and peripheral nerve injuries are among the intraoperative complications. Diagnosing isolated injuries of the marginal mandibular branch of the facial nerve after CEA is challenging and leads to oral motor dysfunction that affects the patient's quality of life. Early diagnosis and treatment are valuable, since delayed diagnosis requires a surgical restoration of the affected nerve or muscle. In this article, we present a case of marginal mandibular nerve injury after CEA to increase the aw

Virilizing adrenal cancer and bail-out nephrectomy.

We report a rare case of virilizing adrenal cancer with tumorous invasion of the left renal vein in which a retroperitoneal adrenalectomy with bail-out nephrectomy was performed. A tumor thrombus infiltrated the wall of the left adrenal vein and extended into the left renal vein. Initially, a kidney sparing procedure with partial tangential excision of the involved renal vein wall was performed. After positive vein margins were confirmed with intraoperative histology, the indication for nephrectomy was made. To the authors' awareness, this is the first report of a virilizing adrenal cancer wit

Right-sided chyle leak encountered during supraclavicular brachial plexus exposure: illustrative case.

BACKGROUND: Chyle leaks are a rare but recognized complication of neck surgery, most commonly encountered on the left due to thoracic duct (TD) anatomy. OBSERVATIONS: The authors present an unusual case of a right-sided chyle leak encountered during brachial plexus exposure for nerve transfer for right hemidiaphragm paralysis. Intraoperatively, a moderate leak of milky fluid was identified near a lymph node bed and controlled with suture ligation. Postoperative recovery was uneventful. Right-sided occurrence suggests variant lymphatic anatomy, possibly involving an accessory TD or a large rig

Importance of preoperative HRCT temporal bone in the orientation of the cochlea and its relation to intra-operative difficulties in cochlear implantation - predicting the grades of difficulty.

OBJECTIVE: This study emphasizes the role of preoperative high-resolution computed tomography (HRCT) temporal bone in evaluating the variation in cochlear orientation and proposes a grading system to determine the level of intraoperative difficulties encountered. METHODS: Preoperative correlation of middle ear and inner ear structures along with the basal turn angle (BTA) was done to assess the orientation. Patients were divided into three groups depending on BTA and radiology findings. RESULTS: Group A (BTA = 55°-60°) had statistically significant (p < 0.05) correlation between middle ear t

Perioperative Serotonin Syndrome Manifesting as Gastrocnemius Myofasciculations: A Case Report.

In perioperative settings where a patient under general anesthesia, presentation of serotonin syndrome might be far from the "classical" description of this potentially fatal condition. A patient who manifested signs of serotonin toxicity during an intravenous anesthetic, remifentanil, is presented. At the time of surgery, the patient was being treated with tramadol for pain management. The patient displayed myofasciculations on both gastrocnemius muscles confirmed electromyographically. All other conventional signs of serotonin syndrome were absent except hypotension and nystagmus. A presumpt

Splenic torsion requiring splenectomy six years following laparoscopic Nissen fundoplication.

BACKGROUND: Laparoscopic Nissen fundoplication has become a mainstay in the surgical treatment of gastroesophageal reflux disease, as it has proved to be a durable, well-tolerated procedure. Despite the safety and efficacy associated with this procedure, surgeons performing this advanced laparoscopic surgery should be well versed in the potential intraoperative and postoperative complications. METHODS: A case is presented of a rare complication of splenic torsion following laparoscopic Nissen fundoplication. Diagnostic evaluations and intraoperative findings are discussed. RESULTS: We presen

Ligamentum arteriosum calcification that presented as an esophageal perforation caused by duck bone ingestion.

Ligamentum arteriosum calcification is the calcification or ossification of arterial ligaments. However, on computed tomography images, ligamentum arteriosum calcification is often mistaken for esophageal perforation when a patient has a medical history of foreign body ingestion. Ligamentum arteriosum calcification is uncommon in clinical practice. In this case report, we confirm the presence of this condition intraoperatively, which has seldom been reported previously. Increased awareness of the clinical characteristics of this uncommon disease can help thoracic surgeons with proper patient m
