Intraoperative boost radiation effects on early wound complications in breast cancer patients undergoing breast-conserving surgery.

Background/aim: Intraoperative radiation therapy (IORT) may pose a risk for wound complications. All technical aspects of IORT regarding early wound complications were evaluated. Materials and methods: Ninety-three consecutive patients operated on with the same surgical technique and given (study group) or not given (control group) IORT were included. Wound complications were evaluated in two groups. Results: Forty-three patients were treated with boost dose IORT and 50 patients were treated with breast-conserving surgery without IORT. When both groups were compared in terms of early postopera

Intraoperative ventilation strategies to prevent postoperative pulmonary complications: Systematic review, meta-analysis, and trial sequential analysis.

For many years, mechanical ventilation with high tidal volumes (V(T)) was common practice in operating theaters because this strategy recruits collapsed lung tissue, improves ventilation-perfusion mismatch, and thus decreases the need for high oxygen fractions. Positive end-expiratory pressure (PEEP) was seldom used because it could cause cardiac compromise. Increasing advances in the understanding of the mechanisms of ventilator-induced lung injury from animal studies and randomized controlled trials in patients with uninjured lungs in intensive care unit and operation room have pushed anesth

Needle tip control and its effect in reducing intraoperative complications during tension-free vaginal tape placement.

Tactile needle tip control was used to aid perforation during standard tension-free vaginal tape (TVT) placement to treat urinary incontinence. The success and complications of this novel method were compared retrospectively with the reported results of the standard technique. One hundred nine patients had TVT placement between 1998 and 2001, with follow-up continuing into 2002. Preoperatively, the severity of urinary incontinence was assessed objectively. Postoperatively, TVT effectiveness was assessed subjectively by standardized questionnaire, completed by 78 of the 109 patients (72%). Obje

Intraoperative Indocyanine Green Angiography as a Predictor of Complications in Breast Cancer Patients Undergoing Conservative Mastectomy.

BACKGROUND: Conserving mastectomy techniques, such as nipple-sparing and skin-sparing mastectomy, enable immediate breast reconstruction, improving aesthetic outcomes but increasing the risk of postoperative complications, particularly ischemic events related to mastectomy skin flap perfusion. Intraoperative assessment with indocyanine green angiography has emerged as a tool to evaluate vascularization in real time. PATIENTS AND METHODS: A single-center retrospective study was conducted on patients undergoing conserving mastectomy with immediate prosthetic reconstruction from 2023 to 2026. In

Intraoperative mechanical power and postoperative pulmonary complications: association and threshold analysis in adult intra-abdominal surgery.

BACKGROUND: Postoperative pulmonary complications (PPCs) contribute to perioperative morbidity and mortality. Mechanical powers (MPs) determined using different deviations from originally described MP have been shown to be associated with PPCs. We aimed to evaluate the association between a standardized derivation of MP and PPCs and to explore a threshold associated with increased risk in multicentre cohort of adults undergoing intra-abdominal surgery. METHODS: Data on adult patients who underwent intra-abdominal surgery under general anaesthesia were obtained from the Multicentre Perioperati

Intraoperative biodegradable stent placement to reduce complications after pancreatoduodenectomy - interim results from a randomised clinical trial.

INTRODUCTION: Postoperative pancreatic fistula (POPF) remains a major cause of morbidity after pancreatoduodenectomy (PD), with an incidence of more than 30% in patients with small pancreatic ducts. Evidence supporting preventive measures remains limited. Biodegradable stent placement across the pancreaticojejunostomy (PJ) may reduce the POPF. This study represents the first randomised controlled trial evaluating biodegradable stents in patients with high-risk pancreaticojejunostomies. METHODS: This single centre, patient- and assessor-blinded, randomised clinical trial included patients unde

Intraoperative Ventilation/Perfusion Mismatch and Postoperative Pulmonary Complications after Major Noncardiac Surgery: A Prospective Cohort Study.

BACKGROUND: Postoperative pulmonary complications can increase hospital length of stay, postoperative morbidity, and mortality. Although many factors can increase the risk of postoperative pulmonary complications, it is not known whether intraoperative ventilation/perfusion (V/Q) mismatch can be associated with an increased risk of postoperative pulmonary complications after major noncardiac surgery. METHODS: This study enrolled patients undergoing general anesthesia for noncardiac surgery and evaluated intraoperative V/Q distribution using the automatic lung parameter estimator technique. Th

Sedoanalgesia modality during laser photocoagulation for retinopathy of prematurity: Intraoperative complications and early postoperative follow-up.

BACKGROUND: Laser photocoagulation (LPC) is a surgical procedure used in the treatment of premature retinopathy that may cause retinal detachment and blindness if not diagnosed and treated early. The anesthesia method used in LPC varies from sedoanalgesia to general anesthesia and airway management varies from spontaneous ventilation to endotracheal intubation. In this study, we aimed to evaluate the effectiveness of sedoanalgesia applications and this anesthesia procedure concerning intraoperative and postoperative complications by avoiding intubation and mechanical ventilation in premature i

Risk factors for pulmonary complications after hepatic resection: role of intraoperative hemodynamic instability and hepatic ischemia.

BACKGROUND: Postoperative operative pulmonary complications (PPCs) after hepatic surgery are associated with increased length of hospital stays. Intraoperative blood transfusion, extensive resection and different comorbidities have been identified. Other parameters, like time of hepatic ischemia, have neither been clinically studied, though experimental studies show that hepatic ischemia can provide lung injury. The objective of this study was to determinate the risk factors of postoperative pulmonary complications (PPCs) after hepatic resection within 7 postoperative days. METHOD: Ninety-fou

Major biliary complications in 2,714 cases of laparoscopic cholecystectomy without intraoperative cholangiography: a multicenter retrospective study.

BACKGROUND: The ongoing debate between routine and selective users of intraoperative cholangiography (IOC) during laparoscopic cholecystectomy (LC) has not yet come to an end. Routine users argue that IOC decreases the rate of biliary complications such as bile duct injury, biliary leak and missed common bile duct (CBD) stones, a claim that selective users do not fully support. On the other hand, a third policy that was adopted by many other centers is performing LC without IOC. In this retrospective study, we are exploring the results of a relatively large multicenter series of LC without IOC

Technical pitfalls of transcervical extended mediastinal lymphadenectomy--how to avoid them and to manage intraoperative complications.

Transcervical extended mediastinal lymphadenectomy (TEMLA), introduced by our team in 2004, is a new technique for the preoperative staging of non-small cell lung cancer (NSCLC). The aim of TEMLA is to maximally accurately stage and possibly to improve late results of treatment of NSCLC. Operative techniques include a collar incision in the neck, elevation of the sternal manubrium with a special retractor, bilateral visualization of the laryngeal recurrent and vagus nerves, and dissection of all mediastinal nodal stations except for the pulmonary ligaments nodes (station 9, according to the Mo

Intraoperative complications of revision hip arthroplasty using a fully porous-coated straight cobalt-chrome femoral stem.

The operative complications of a consecutive series of 135 cementless revision total hip arthroplasties using a fully porous-coated straight cobalt-chrome femoral stem were reviewed. Intraoperative complications occurred in 59 cases (44%), 22 of which (37%) involved more than one complication. Three categories of intraoperative complications were recorded: eccentric reaming, femoral perforation, and femoral fracture. Complications were noted to decrease throughout the period of the study. Surgical approach did not affect the complication rate. Stem length, stem diameter, and host-bone quality

Neurotoxicity of subarachnoid Gd-based contrast agent accumulation: a potential complication of intraoperative MRI?

OBJECTIVE: Intraoperative MRI with Gd-based contrast agent (GBCA) improves the extent of resection of contrast-enhancing brain tumors. Signal changes of CSF due to perioperative GBCA leakage in the subarachnoid space have been reported. However, although GBCA potentially exhibits neurotoxic effects, so far no associated complications have been described. In this case series, the authors report a single-center cohort of patients with subarachnoid GBCA extravasation after intraoperative MRI and discuss potential neurotoxic complications and potential ways of avoiding them. METHODS: All patients
