There is a paucity of empirical data establishing the efficacy of electroconvulsive therapy (ECT) in patients with mental retardation and psychiatric disorders. This study examines the efficacy of ECT on specific symptoms and between psychiatric diagnoses in patients with mental retardation who are psychiatrically ill. A chart review was performed on 20 inpatients who had received ECT on a dedicated Mental Retardation-Dual Diagnosis Unit and were divided into 3 categories: mood disorders (n = 12), psychotic disorders (n = 6), and intermittent explosive disorder (n = 2). Ratings were performed

OBJECTIVE: Irritability in youth is increasingly understood as comprising tonic irritability, reflecting persistent irritable mood, and phasic irritability, reflecting acute anger outbursts. This study examined whether these dimensions showed distinct associations with autistic characteristics across parent and teacher report.
METHODS: Clinical records were examined for 1534 youth ages 5-18 referred to a university outpatient psychiatric clinic, including 334 youth with a PDD/ASD diagnosis. Analyses were conducted separately in the PDD/ASD-restricted sample (n = 334) and the full diagnostic s…

BACKGROUND: Observational studies have suggested correlations between mood instability and gastrointestinal (GI) disorders, however, the causal relationships remain unclear.
METHODS: We conducted a two-sample Mendelian randomization (MR) analysis to investigate the causal relationships between mood instability and 22 GI disorders using genome-wide association study (GWAS) data from disease-relevant GWAS studies, the UK Biobank, and the FinnGen project. To ensure the robustness of causal estimates, we applied multiple MR methods and extensive sensitivity analyses. Additionally, multivariable M

OBJECTIVE: Irritability and temper outbursts are common presenting concerns in pediatric mental health settings, but systematic monitoring can be difficult to sustain with broad, repeated assessment batteries. This study evaluated the temper outbursts/irritability-2 (TOI-2), an ultra-brief, two-item screener assessing temper outbursts and frequently irritable mood in youth.
METHODS: Participants were 1515 youth aged 5-17 years (M = 11.80, SD = 3.60) referred to an outpatient child psychiatry clinic. Female caregivers (90.4% biological mothers) completed the TOI-2 and criterion measures at int…

To evaluate the relationship between Self-Rated Health (SRH) and cardiorespiratory fitness (CRF) in a sample of children and adolescents enrolled in official schools in Bogotá, Colombia. A cross-sectional study was performed with 7402 children and adolescents between 9 and 17 years of age. Participants were asked to rate their health based on eight validated questions, addressing the participants propensity for headache, stomach-ache, backache, feeling-low, irritability/bad mood, nervousness, sleeping-difficulties, and dizziness. The choices were "rarely or never", "almost every month", "almos

OBJECTIVE: Ever since the introduction of combined oral contraception (COC), one of the major reasons for discontinuing the pill use has been mood-related side effects. Moreover, women who discontinue the pill turn to less effective methods whereby the probability of an unintended conception increases. Approximately 4-10% of COC users complain of depressed mood, irritability or increased anxiety, but drug-related causality has been difficult to prove. Given the lack of randomized controlled trials in this area, we aimed to prospectively estimate the severity of adverse mood in COC users that w

Irritability encompasses both normative misbehavior in early childhood and clinically significant problems across development. Recent studies have distinguished between tonic (i.e., persistently angry or grumpy mood) and phasic (i.e., temper tantrums or outbursts) forms of irritability and shown that they have different implications for psychopathology and functioning. However, data on this distinction in young (i.e., preschool aged) children are nonexistent. We utilized data from a longitudinal study of a community sample of 462 3-year-olds followed to age 15. We conducted confirmatory factor

Irregularities of serotonin function in irritable bowel syndrome (IBS) may be due to changes in the metabolism of the serotonin precursor l-tryptophan. Dietary alteration of tryptophan intake may impact upon the mood and bowel symptoms of IBS. We hypothesized that diarrhea-predominant irritable bowel syndrome (d-IBS) patients would exhibit an increase in plasma tryptophan due to alterations in tryptophan metabolism. We also hypothesized that a diet low in tryptophan would reverse this change and reduce symptoms. Thirteen patients with d-IBS had fasting serum free and total tryptophan, large ne

PURPOSE: Mood disorders frequently coexist with borderline personality pathology (BPP), presenting considerable clinical challenges. Affective temperaments (AT) play a role in modulating mood disorders and influence the manifestation of illness. BPP and AT share common characteristics, such as emotional instability, impulsivity, and difficulties in interpersonal relationships. However, research on the relationship between BPP and AT remains limited. This study employed network analysis to explore the network structure between BPP and AT in a cohort of patients with mood disorders.

OBJECTIVE: Functional gastrointestinal disorders (FGID) have been shown to be associated with both comorbid mood disorders and traumatic events such as abuse earlier in life. In a longitudinal study, we tested a model that hypothesized: (i) childhood abuse was associated with subsequent mood disorder and pain or interference in life by bowel symptoms both directly and indirectly via neurotic personality; and (ii) an ongoing cycle of mood disorder impacts on bowel symptoms.
DESIGN: Subjects from the general population classified as irritable bowel syndrome and/or functional dyspepsia (IBS/FD,

Premenstrual symptoms occur in the late luteal phase as a result of differential sensitivity to normal hormonal fluctuations. These symptoms can include mood symptoms such as mood lability, anxiety, sadness or irritability as well as physical symptoms such as lethargy, bloating and breast tenderness. Symptoms can significantly impact quality of life, particularly in patients with the more severe form of premenstrual symptoms, PMDD. Importantly, symptoms should improve or resolve in the follicular phase. Evidence-based treatment options include SSRIs or hormonal suppression of ovulation with CO

Disruptive mood dysregulation disorder (DMDD) is a relatively new diagnosis in the field of childhood onset disorders. Characterized by both behavior and mood disruption, DMDD is a purportedly unique clinical presentation with few relevant treatment studies to date. The current case study presents the application of cognitive-behavioral therapy (CBT) for anger and aggression in a 9-year-old girl with DMDD, co-occurring attention deficit hyperactivity disorder (ADHD), and a history of unspecified anxiety disorder. At the time of intake evaluation, she demonstrated three to four temper outbursts

BACKGROUND: An increasing number of youth are being diagnosed with, and treated for, bipolar disorder (BD). Controversy exists about whether youth with non-episodic irritability and symptoms of attention deficit hyperactivity disorder (ADHD) should be considered to have a developmental presentation of mania.
METHOD: A selective review of the literature related to this question, along with recommendations to guide clinical assessment.
RESULTS: Data indicate differences between youth with episodic mania and those with non-episodic irritability in longitudinal diagnostic associations, family hi

OBJECTIVE: The new Diagnostic and Statistical Manual of Mental Disorders, 5th ed. (DSM-5) diagnosis, disruptive mood dysregulation disorder (DMDD), has generated appreciable controversy since its inception, primarily in regard to its validity as a distinct disorder from oppositional defiant disorder (ODD). The goal of our study was to determine if the two DSM-5 DMDD symptoms (persistently irritable or angry mood and severe recurrent temper outbursts) occurred independently of other disorders, particularly ODD. Other DSM-5 DMDD criteria were not assessed.
METHODS: Maternal ratings of the two D

OBJECTIVE: The importance of dimensional approaches is widely recognized, but an empirical base for clinical application is lacking. This is particularly true for irritability, a dimensional phenotype that cuts across many areas of psychopathology and manifests early in life. We examine longitudinal, dimensional patterns of irritability and their clinical import in early childhood.
METHOD: Irritability was assessed longitudinally over an average of 16 months in a clinically enriched, diverse community sample of preschoolers (N = 497; mean = 4.2 years; SD = 0.8). Using the Temper Loss scale of

BACKGROUND: The goal of this study was to assess the frequency and spectrum of symptoms related to bipolar disorders in a community sample by means of a self-rating questionnaire.
METHOD: The Mood Disorder Questionnaire, a self-rating scale which evaluates DSM-IV manic/hypomanic symptoms, was administered to a community sample of 1,034 individuals recruited through advertisements. Symptom interrelationships were studied by factor analysis.
RESULTS: 275 individuals (26.6%) reported moderate or severe impairment due to the symptoms. Manic/hypomanic symptoms were present in a high proportion of

INTRODUCTION: Extreme sensory processing patterns may contribute to the pathophysiology of major affective disorders. We aimed to examine whether significant correlations exist between sensory profiles, hypomania, self-reported depression, and hopelessness and whether sensory profiles may be potential mediators of the association between hypomania and depression/hopelessness.
METHODS: The sample consisted of 488 euthymic affective disorder patients of which 283 diagnosed with unipolar and 162 with bipolar disorder with an age ranging from 18 to 65 years (mean = 47.82 ± 11.67).
