Cyclosporine metabolism occurs in the liver via hepatic cytochrome P-450 microsomal enzymes. Ketoconazole, an imidazole derivative, has been shown to inhibit the cytochrome P-450 enzyme system. Thirty-six renal transplant recipients receiving cyclosporine as part of a triple immunosuppressive drug regimen were started on 200 mg/day of oral ketoconazole. Thirty-seven patients with seborrheic dermatitis were treated topically with a 2% ketoconazole cream or its vehicle control in a double-blind study. The subjects were studied for numbers of Malassezia ovalis (Pityrosporum ovale) cells in their scalp scale; changes in numbers of yeast cells and morphology of M. ovalis were tabulated along with clinical assessment of improvement. Basidiobolomycosis is a deep mycosis which preferentially affects rural young people in tropical countries. We report a case of basidiobolomycosis successfully treated with ketoconazole. It was a 9-year-old boy of rural origin in whom the diagnosis of basidiobolomycosis was suspected due to a deep skin infiltration involving the chest and neck. Fifty patients suffering from pityriasis versicolor were treated with either 2% ketoconazole cream (25 patients) or 1% terbinafine cream (25 patients). Diagnosis was confirmed by microscopic potassium hydroxide (KOH) examination and either of the cream was applied once daily for a duration of 2 weeks. Global assessment was done at the end of treatment with mycological and clinical cure rate of 88% and 96% in ketoconazole and terbinafine treated groups respectively. After a follow up of 3 months the relapse in ketoconazole group was 13.53% and 8.33% in terbinafine group with no side effects.
