Anatomical Relationship Between the Kidney Collecting System and the Intrarenal Arteries in the Sheep: Contribution for a New Urological Model.

Previous studies have demonstrated that the pig collecting system heals after partial nephrectomy without closure. Recently, a study in sheep showed that partial nephrectomy without closure of the collecting system resulted in urinary leakage and urinoma. The aim of this study was to present detailed anatomical findings on the intrarenal anatomy of the sheep. Forty two kidneys were used to produce tridimensional endocasts of the collecting system together with the intrarenal arteries. A renal pelvis which displayed 11-19 (mean of 16) renal recesses was present. There were no calices present. T

Transitional cell carcinoma of the pelvicalices and ureter.

Transitional cell carcinoma accounts for about 90% of all cancers of the renal pelvis and more than 90% of all cancers of the ureter. Its clinical presentation is nonspecific. Radiology plays a critical role in detection, evaluation, and disease monitoring. We reviewed the pathologic and clinical features of transitional cell carcinoma of the upper urinary tract, with attention to its radiology appearance, staging, and treatment.

[Pathology of renal tuberculosis (author's transl)].

Introduction of specific chemotherapy and vaccination leads to a remarkable recession of renal tuberculosis in the younger age group. Renal tuberculosis is the result of a haematogenous spread of tubercle bacillus. Haematogenous spreading occurs immediately after primary infection, or, in elderly patients, in combination with recurrency of tuberculous foci in lungs and hilar-lymphnodes. Simultaneous metastasis in the skeleton, especially in the vertebrae, are observed in 30%. The incubation period between tuberculous spread and clinical manifestation of renal tuberculosis lasts several years,

Optimizing aspiration efficiency in RIRS: the impact of ratio of endoscope-sheath diameter, irrigation flow, and negative pressure in an vitro study.

To examine the impact of ratio of endoscope-sheath diameter (RESD), irrigation rate, and negative pressure on the efficiency of stone fragment aspiration during retrograde intrarenal surgery (RIRS) in an in vitro setting. Five grams of stone fragment were precisely measured and placed in renal calyx. A flexible ureteroscope with a Flexible Adjustable Navigation Suction Ureteral Access Sheath (FANS) was introduced into the renal calyx, connected to an irrigation pump and suction pump. A urologist performed 1-minute simulated intraoperative aspiration maneuvers. Residual stone dust was collected

Defining Thermally Safe Laser Lithotripsy Power and Irrigation Parameters: In Vitro Model.

Introduction: High-power laser settings are commonly employed for stone dusting techniques. Previous in vitro and in vivo studies have demonstrated that a toxic thermal dose can result from treatment within a renal calix without adequate irrigation. Hence, both laser power and irrigation rate must be considered together to determine safe laser lithotripsy parameters. The objective of this in vitro study was to map parameter safety boundaries and create guidelines for selection of safe laser and irrigation settings. Methods: The experimental system consisted of in vitro models simulating ureter

[Anatomy and function of the upper urinary tract].

This report deals with the histologic and gross anatomy of the upper urinary tract (calyces, pelvis, and ureter) as well as the nerve supply to this region. It also covers the physiological transport of urine from the kidneys to the bladder, which is reviewed on the basis of experimental and clinical studies. A pacemaker system present in the proximal calyces has been found to have an important physiological role in urine transport. However, clinical experience has shown that urine transport is not affected by surgery such as pyeloplasty and pyelolithotomy which impairs the activity of this pa

Effects of beta-adrenergic agents on the pacemaker of ureteral peristalsis.

An electromyographic study of the responses of the pelvicalyceal border which is the pacemaker region of ureteral peristalsis, and of the renal pelvis and ureter to dobutamine and terbutaline, beta 1- and beta 2-adrenergic stimulants, respectively, was performed using isolated canine pelviureteral preparations. The beta 1-adrenergic stimulant produced a marked enhancement of discharge potential in the pacemaker region, but, in the renal pelvis and ureter, it did not evoke any noticeable response. Application of the beta 2-adrenergic stimulant led to disappearance of discharges in the renal pel

The interrelationship between paraureteric diverticula, vesicoureteric reflux and duplication of the pelvicaliceal collecting system: a family study.

The incidence of a bifid pelvicaliceal system, vesicoureteric reflux, its sequelae and paraureteric diverticulum was determined in the patients and siblings of 22 patients with a paraureteric diverticulum. The results support the hypothesis that there is a direct genetic relationship between a paraureteric diverticulum, duplication of the pelvicaliceal system and vesicoureteric reflux. This suggests that the vesicoureteric reflux associated with a parauretic diverticulum is primary and related to lateral ectopia of the ureteric orifice. This syndrome complex may be inherited by multiple genes

Ultrasonic assessment of renal transitional cell carcinoma: preliminary report.

The value of ultrasound in determining the etiology of nonopaque filling defects seen in the renal pelvis at intravenous urography is under study. The ultrasound findings in three patients with proven transitional cell carcinoma of the kidney were similar and consisted of separation of the central renal echo complex by a region of low-intensity echoes. This pattern is distinct from characteristically echogenic nonopaque renal calculi and anechoic hydronephrosis. Renal collecting system blood clots may have an echo pattern similar to collecting system tumors, but can usually be distinguished by

Rupture of the maternal renal calyx secondary to fetal malpresentation: a case report.

BACKGROUND: Rupture of the renal collecting system is a potentially life-threatening condition in pregnancy. Most cases are associated with obstruction from nephrolithiasis or diseased renal parenchyma. CASE: A 24-year-old, nulliparous, African American woman at 38 weeks + 3 days' gestation presented with left flank pain refractory to conservative pain management. Computed tomography was negative for a stone but significant for infrarenal fluid and rupture of a left renal calyx. The fetal head was not flexed and appeared to be compressing the left ureter. A face presentation became apparent d

Perirenal pyelocaliceal diverticulum in an infant cat.

A 3-month-old female tortoise-shell cat showing azotemia died with a marked swollen abdomen. Necropsy revealed a huge perirenal cyst (8.5 x 6.0 x 4.5 cm) on the ventral aspect of the right kidney. The cyst was filled with the pellucid yellow fluid with a smell of urine. The lumen was connected with irregularly dilated renal pelvis by a narrow channel passing through the renal parenchyma. The cyst was lined by epithelial cells and its wall was consisted of collagen fibers and smooth muscle cells as that of the renal pelvis and ureter. Renal parenchyma adjacent to the channel showed interstitial

Boari flap calycovesicostomy: a salvage procedure for giant hydronephrosis due to ureteropelvic junction obstruction.

The surgical management of two patients with giant hydronephrosis in a solitary kidney treated by Boari flap calycovesicostomy is presented. In one patient, this operation was done following unsuccessful previous pyeloplasty, while in the other this was done as the primary operation. Though free reflux was observed in both the cases, the refluxed contrast emptied satisfactorily after double voiding. No deterioration of renal function was noted during the follow-up period of 12 months.

[Ureteral hemangiomas: the intramural arteriovenous hemangioma].

A report is given on a 42-year-old female patient with a ureteropyelokaliectasis in a solitary left kidney with a calyx stone in the middle group of calyces. An intramural arteriovenous haemangioma in the prevesical section of the ureter was found to be the cause of urinary stasis. After fitting a pull-through nephrostomy and removing the calyx stone the affected part of the ureter was resected and a ureterocystoneostomy was carried out with good late results.

Nonvisualized ("phantom") renal calyx: causes and radiological approach to diagnosis.

A calyx which fails completely to opacify on excretory urography (phantom calyx) is often the harbinger of serious underlying renal disease. Causes of a phantom calyx include tuberculosis, tumor, calculus, ischemia, trauma, and congenital anomaly. The pathologic basis for the radiographic findings in each of these entities is described and an overall approach to diagnosis is set forth.

Hypoplasia of the major renal conduits.

An unusual case of a patient surviving a bilateral congenital anomaly of the major renal conduits is presented. Anatomical features of this case, considered with present embryological ideas in mind, support the concept of a continuum of disorders of the major renal conduits. The literature is reviewed and interesting microsurgical techniques with hypothermia enabling reconstruction of the collecting systems and renal preservation are discussed.

Evaluation of a system for classification of stones and their sites in kidneys treated with extracorporeal shock wave lithotripsy.

The results of treatment with extracorporeal shock wave lithotripsy (ESWL) were recorded in 1067 patients with renal calculi during their first admission to hospital. All treatments were performed in an unmodified Dornier HM3 lithotripter according to the original recommendations whereby the generator voltage was usually set between 18 and 23 kV. The stones in kidneys treated with ESWL alone were first classified into four different types (A. B. C. D) and after which a further subgrouping was carried out according to the number and sites of stones in the renal pelvis or calyces. The number of
