Fifteen knees with patellar dislocation after total knee arthroplasty had realignment of the extensor mechanism using a modification of the Trillat procedure. The onset of dislocation occurred on average 4.7 months from the time of surgery. After total knee arthroplasty the patients had an average range of motion of 109 degrees. All patients had medialization of the tibial tubercle and lateral release. No patient had a recurrent dislocation after a minimum 2-year follow-up period. The average knee score was 82 and the average flexion arc was 112 degrees. All but one of the osteotomies healed u

PURPOSE: While rigid knee-spanning external fixation is more commonly utilized in the treatment of complex knee injuries compared to hinged external fixation (HEF), HEF has the added benefit of providing stability while also permitting early controlled range of motion. However, there is limited existing literature on the indications for HEF of the knee. The aim of this study was to review the clinical indications for HEF in the management of acute and chronic knee dislocations (KD), tibiofemoral subluxations and knee fracture-dislocations.
METHODS: Five databases, including PubMed, CINAHL, Co…

BACKGROUND: Acute knee dislocations are devastating injuries that can be challenging for surgeons to treat. The bony integrity of the tibial plateau and the lateral femoral condyle has a fundamental meaning for translational and rotational knee joint stability. Posterolateral tibial plateau depression fractures (PLTPFs) and lateral femoral condyle impaction fractures (LFCIFs) can influence clinical outcomes, but their frequency and characteristics in the setting of acute knee dislocations are unknown.
PURPOSE: To report the incidence of PLTPFs and LFCIFs in acute knee dislocations as well as …

BACKGROUND: Congenital and bilateral dislocation of the knee is an uncommon disorder; it may be isolated or combined with other congenital muscoloskeletal disorders, associated with neurologic disorders or occur in syndromic conditions. The knee presents a typical pathologic hyperextension, with or without joint dislocation. If untreated, the knee may develop stiffness or instability.
CASE SUMMARY: We report about a new-born baby girl presented at birth with an unusual position of both lower limbs with obvious hyperextended knees. Starting from the first hours of life, this baby was treated c

Tibiofemoral knee dislocations are typically a consequence of high-energy mechanisms, causing significant damage to the soft tissue and osseous structures of the knee. Concomitant neurovascular injuries such as popliteal artery and peroneal nerve injuries are also common and can have significant long-term consequences. The mechanism typically involves a traction injury to the peroneal nerve subsequent to an extreme varus moment applied to the knee. Complete nerve injuries typically hold a worse prognosis than incomplete palsies. Rates of functional recovery in the setting of a complete palsy f…

Congenital knee dislocation is a rare anomaly associated with a variety of neuromuscular diseases and deformities. The etiology of this condition remains unclear, but it is usually associated with a variety of disorders, such as Larsen's syndrome, arthrogryposis, spondyloepiphyseal dysplasia, Ehlers-Danlos syndrome, Down syndrome, and Streeter's dysplasia. It is rarely an isolated entity, and 60% of patients with congenital knee dislocation had additional congenital anomalies, most commonly hip dysplasia. The ideal method of treatment is debated. No current treatment algorithms address anterio…

Knee dislocations can cause extensive soft tissue disruption including vascular insufficiency to the leg secondary to popliteal artery injury. Physical exam may miss vascular injury and possible late occlusion, but there is controversy regarding use of angiography to evaluate patients after dislocation. Magnetic resonance angiography (MRA) has been shown to be equally effective as angiography in evaluating vascular injury and to have fewer complications than angiography. Patients with knee dislocations routinely receive magnetic resonance imaging (MRI) to assess ligament integrity. The purpose…

The authors present the results of a retrospective review of popliteal artery injuries associated with fractures and dislocations about the knee. They treated 41 patients with popliteal artery injuries associated with either fractures about the knee or knee dislocations. Thirty-five of the patients were males, 6 females; the mean age was 23 years. The delay before accessing the hospital was 17 hours (range: 3 hours to 10 days). Thirty-two fractures were open. Together with the vascular injury, 12 femoral fractures, 20 tibial and fibular fractures, 5 knee dislocations, 4 femoral + tibial fractu…

Three cases of Larsen's syndrome in siblings are presented to illustrate a paradigm for the age-related treatment of deformity. Each child presented at a different age, with differing degrees of deformity and growth disturbance. Both growth and remodeling potential influenced the choice of treatment and outcome. The surgeon who undertakes the correction or prevention of these deformities must face the problem of and, it is hoped, use, growth potential from open physes and the progressive loss of remodeling capacity with advancing age.

Complete dislocation of the knee is rare but frequently associated with popliteal artery disruption. Prompt recognition and early revascularisation are paramount for a successful and functional result. We report on a case of anterior dislocation of the knee complicated by concomitant popliteal artery injury. The value of pre-operative diagnostic methods including pre-operative arteriography and Doppler-sonography is discussed. Review of literature is presented.

One-third of the adolescent patients followed up in our prosthetic clinic have developed patellar dislocation. Four cases are described, and radiographs are evaluated for femoral-tibial alignment, patellar height, and tibial length. Patella alta was present in each patient. We propose that the forces associated with the patellar tendon-bearing prosthesis may produce elongation of the patellar tendon in young patients and predispose them to patellar instability.

A 49-year-old man incurred a posterolateral dislocation of the knee, a rare type of knee injury. The injury was associated with a significant skin slough as a result of internal pressure from the displaced medial femoral condyle. The dislocation also was associated with a rupture of the quadriceps tendon. The mechanism of injury was a forcible abduction of the flexed knee, coupled with medial rotation of the tibia upon the femur. This type of knee dislocation cannot be reduced closed and therefore immediate open reduction with primary ligamentous repair is the treatment of choice.

A further case of Down-mosaic Turner syndrome is discussed. Both the cytogenetic and the dermatoglyphic data support the clinical diagnosis. The association with dislocated knees and the diagnosis of this polysyndrome at birth have not been reported before.

INTRODUCTION: Dissociation of the knee joint, or knee dislocations (KD), can lead to severe complications, often resulting in multiligament injuries. A subset of these injuries are irreducible by closed reduction and require open reduction. Identifying KDs that necessitate surgical intervention is crucial for optimal outcomes. While previous studies have explored various risk factors, the influence of associated fractures is less understood.
MATERIALS AND METHODS: We queried the Trauma Quality Improvement Program (TQIP) database from 2017 to 2021, for non-congenital closed knee dislocations r…

BACKGROUND: Recurrent patellar dislocation in combination with cartilage injures are difficult injuries to treat with confounding pathways of treatment. The aim of this study was to compare the clinical and functional outcomes of patients operated for patellofemoral instability with and without cartilage defects.
METHODS: Eighty-two patients (mean age 28.8 years) with recurrent patellar dislocations, who underwent soft-tissue or bony procedures, were divided into two matched groups (age, sex, follow up and type of procedure) of 41 each, based on the presence or absence of cartilage defects in
