Dual-energy computed tomography of cruciate ligament injuries in acute knee trauma.

OBJECTIVE: To examine dual-energy computed tomography (DECT) in evaluating cruciate ligament injuries. More specifically, the purpose was to assess the optimal keV level in DECT gemstone spectral imaging (GSI) images and to examine the usefulness of collagen-specific color mapping and dual-energy bone removal in the evaluation of cruciate ligaments and the popliteus tendon. MATERIALS AND METHODS: At a level 1 trauma center, a 29-month period of emergency department DECT examinations for acute knee trauma was reviewed by two radiologists for presence of cruciate ligament injuries, visualizatio

Treatment of PCL, ACL, and lateral-side knee injuries: acute and chronic.

Combined posterior cruciate ligament, anterior cruciate ligament, and lateral-side disruption is one of the more common patterns of multiligament knee injury. This is a devastating injury with significant long-term functional sequelae, making accurate diagnosis and management extremely important. While surgical intervention is necessary to restore function, the specific management strategies remain controversial. This article will review the current literature and the authors' preferred approach including physical examination, imaging, timing of surgery, surgical technique, and postoperative r

Surgical treatment of extension contractures caused by war injuries to the knee.

Immobilization with external fixation bridging the knee joint in extension is frequently used after sustaining a war injury to the region of the knee joint with femoral and tibial bone fractures. Immobilization of the knee with plaster splints is performed in the same position. This usually prolonged treatment results in extension contractures of the knee joint. From June 1991 until March 1994, 54 patients with extension contractures of the knee caused by war wounds were treated at the Department of Orthopedics, Zagreb University Hospital Center, in Zagreb, Croatia. Results of surgical treatme

Knee ligament injuries in adolescents. Eight year follow-up of conservative management.

We have reviewed 32 patients who sustained a substantial knee ligament injury during adolescence when their knee epiphyses were open. They were all treated non-operatively and re-examined and evaluated in detail after an average of eight years. There were 25 Grade II partial tears and seven Grade III complete tears. After Grade II injuries the functional results were excellent or good, though static instability had not improved from the initial post-traumatic examination. The long-term results of Grade III injuries were poor, because of chronic functional instability, with continuous symptoms

Floating knee injuries: a retrospective analysis of treatment outcomes and prognostic factors.

BACKGROUND/ AIMS: Floating knee injury is a rare high-energy trauma pattern frequently associated with concomitant organ injuries. Optimal t … d Gustilo-Anderson grade did not significantly influence functional outcomes. CONCLUSION: Floating knee injuries carry significant morbidity despite favourable functional outcomes in the majority of patients. External fixation prolongs hospitalisation and increases infection risk, highlighting the importance of early conversion to definitive fixation in eligib …

Nerve Injuries After Total Knee Arthroplasty: A Systematic Review and Meta-Analysis.

BACKGROUND: Nerve injury is an uncommon but clinically meaningful complication following total knee arthroplasty (TKA). Reported incidences vary widely across the literature, and the specific patterns, predictors, and recovery trajectories remain unclear. This systematic review and meta-analysis aimed to determine the prevalence, types, risk factors, management approaches, and recovery outcomes of nerve injuries after TKA. METHODS: A comprehensive search identified eight eligible studies reporting postoperative nerve injuries following primary or revision TKA. Data extracted included patient

Diagnosis, Treatment, and Current Controversies in Multiligament Knee Injuries: A Narrative Review.

Multiligament knee injuries are infrequent but potentially severe, defined by the simultaneous rupture of two or more major knee ligaments, with a considerable risk of neurovascular injury. This narrative review, based on a search of PubMed, the Cochrane Library, and Google Scholar covering 2010 to 2026, examines their diagnosis, treatment, and outcomes, with emphasis on current controversies in surgical management. The Schenck classification remains the reference system, but its initial category is imprecise, and it does not reliably predict neurovascular injury; it should be interpreted alon…

Evaluation of Risk Factors and Injury Mechanisms for Soft Tissue Knee Injuries.

Identifying risk factors is essential in diagnosing and preventing soft tissue knee injuries (STKIs). These risk factors are broadly categorised into patient (intrinsic) and external (extrinsic), and non-modifiable and modifiable. Non-modifiable factors predispose individuals to injury, while modifiable ones offer opportunities for intervention and prevention. Injury mechanisms are also important for biomechanical consideration and can help guide diagnosis. Literature on risk factors and mechanisms is limited by its focus on specific subgroups, such as elite athletes or single injury types, re…

Knee injuries concomitant to posterior acetabular fractures: an association to look for.

PURPOSE: Acetabular fractures are frequently associated with other skeletal injuries, particularly knee injuries. This study aims to evaluate the relationship between posterior acetabular fractures and knee injury, analyzing its patterns and association with other injuries. METHODS: This is a retrospective monocentric study in a tertiary referral centre. We reviewed the data (age, sex, type of fracture, and concomitant injury) of patients hospitalized for a posterior acetabular fracture. We recorded concomitant knee injury diagnosed up to one year following acetabular fracture. RESULTS: Two-…

The Management of Bone Defects in Periarticular Knee Injuries: A Review Article.

Traumatic bone defects of the distal femur and proximal tibia present treatment challenges for the orthopaedic traumatologist. In addition to bone loss, significant soft tissue compromise and cartilage defects leave the patient and surgeon with few reconstructive options for proper long-term joint function. These injuries are often complicated by delayed healing, nonunion, malunion, infection, and poor subjective patient outcomes. There are a variety of clinical problems associated with the treatment of periarticular fractures about the knee. This article will describe the surgical options for

Imaging of Athletic Injuries of Knee Ligaments and Menisci: Sports Imaging Series.

Acute knee injuries are a common source of morbidity in athletes and if overlooked may result in chronic functional impairment. Magnetic resonance (MR) imaging of the knee has become the most commonly performed musculoskeletal MR examination and is an indispensable tool in the appropriate management of the injured athlete. Meniscal and ligamentous tearing are the most frequent indications for surgical intervention in sports injuries and an understanding of the anatomy, biomechanics, mechanisms of injury, and patterns of injury are all critical to accurate diagnosis and appropriate management. …

Is ultrasound diagnosis reliable in acute extensor tendon injuries of the knee?

Ruptures of the patellar and quadriceps tendon are rare injuries requiring immediate repair to re-establish knee extensor continuity and allow early motion. Ultrasound is extensively used as a diagnostic tool before surgery on acute traumatic tears of the patellar tendon and quadriceps tendons. The aim of our study was to evaluate the role of sonography in diagnosing quadriceps and patellar tendon rupture and in differentiating partial from complete tears. We conducted a retrospective review of 51 consecutive patients who had a surgical intervention for suspected acute quadriceps and patellar

Tendon transfers for peroneal nerve injuries in the multiple ligament injured knee.

Although knee dislocations are relatively rare injuries; associated drop foot complication as a consequence of common peroneal nerve palsy (CPN) is substantially high. Even after successful ligament constructions; unresolved CPN palsy is a factor contributing to bad outcome after knee dislocations. CPN palsy is seen more after open dislocations, rotatory dislocations, and especially in patients with posterolateral corner injuries. CPN palsy can readily be diagnosed clinically, although high index of suspicion is needed. Surgical exploration in the acute setting is still debatable. Conservative

[Characteristics of intra-articular injuries of the knee joint in coal miners].

This study presents an analysis of chronic knee injuries among coalminers. The aim of this paper was to individuate a relationship between the work conditions in coal mines and the observed pathological changes in the knee joint, as well as to assess the results of treatment. A population of 120 coal miners age ranging from 20 to 46 years (mean age 33.4 years) working in conditions continuously stressing the knee joints was evaluated. It was found that the lower the seam where the miners worked, the greater was the damage to the cartilage. Meniscus injuries were found mainly in miners working …
