Case report: osteonecrosis of the mandible after laryngoscopy and endotracheal tube placement.

Four patients underwent laryngoscopy and general anesthesia surgery without apparent perioperative complications. Within days of their procedure, throat and mouth pain were reported and intraoral examination in all cases revealed exposed bone in the posterior mandible. The sequestra were easily removed and healing was uneventful. These lesions are likely associated with localized oral trauma during intubation causing periosteal damage, compromised blood supply, and subsequent bone necrosis. Because trauma to the mylohyoid ridge during intubation is likely more common than previously appreciate

Increased body mass index per se is not a predictor of difficult laryngoscopy.

PURPOSE: We investigated the association between morbid obesity and difficult laryngoscopy (DL). METHODS: In a prospective, controlled study we evaluated the impact of different variables on the prediction of DL in 200 morbidly obese (study group-SG), and 1272 non-obese (control group-CG) patients undergoing elective surgery. Variables assessed included age, sex, body mass index (BMI), protruding, loose, and missing upper teeth, thyro-mental distance, temporo-mandibular joint (TMJ) function, neck extension, and Mallampati class. A Cormack grade III or IV was considered DL. RESULTS: The SG pa

Sternomental distance predicts vagal reflex during suspension laryngoscopy: A prospective cohort study.

OBJECTIVES: Vagal reflex (VR) during suspension laryngoscopy (SL) is a potentially life-threatening complication. However, preoperative risk stratification tools are lacking, especially for healthy adults. This study aimed to identify the anatomical and clinical predictors of intraoperative VR during SL and establish actionable thresholds for risk stratification. METHODS: This prospective observational cohort study enrolled 371 adult patients who underwent SL under general anesthesia at a tertiary academic hospital between January 2022 and December 2023. Preoperative anatomical measurements,

Frenotomy Provides Improved Direct Laryngoscopy Exposure in Select Adult Patients With Ankyloglossia.

Objective: To analyze the effects of lingual frenotomy on adults with restrictive ankyloglossia and difficult direct laryngoscopy exposure. Methods: A case series study was conducted. Restriction of tongue protrusion due to ankyloglossia was identified in 2 patients who had a history of suboptimal true vocal fold (TVF) exposure on direct laryngoscopy for phonomicrosurgery. Inadequate exposure of the vocal folds was achieved on direct laryngoscopy attempts with manual tongue protrusion. Mandibular tori were not present in these patients. An intraoperative lingual frenotomy was performed in both

High-flow nasal oxygen for suspension laryngoscopy: a multicenter open-label study.

In this study, we aimed to assess the efficacy of high-flow nasal oxygen (HFNO) to maintain blood peripheral oxygen saturation (SpO2) in patients undergoing suspension laryngoscopy under general anesthesia. Adult patients were included in this bicenter study. After face-mask oxygenation, HFNO at a flow rate of 70 L min-1 and fraction of inspired oxygen 100% was initiated at loss of consciousness. At the end of HFNO, blood gas analysis was performed. Of the 29 included patients, five (17.2%; 95% confidence interval [CI]: 7.6-34.5) presented SpO2 <95% during the first 15 minutes of the procedure

Characteristics of lingual thyroglossal duct cysts under flexible laryngoscopy and treatment approaches.

OBJECTIVES: Lingual thyroglossal duct cysts (LTGDCs) are clinically rare and easily misdiagnosed as epiglottic cysts. Misdiagnosis leads to mistreatment; thus, simple diagnosis is important. This study aimed to investigate the characteristics of LTGDCs with flexible laryngoscopy and imaging, improve their diagnosis and explore surgical methods for patients undergoing an initial operation and patients with recurrence. METHODS: In total, 10 patients with LTGDCs were admitted from April 2014 to December 2020.By reviewing the diagnosis and treatment of the first typical case, the characteristics o

Tracheal tube exchange: feasibility of continuous glottic viewing with advanced laryngoscopy assistance.

Trachea tube exchange via an airway exchange catheter is commonly combined with conventional laryngoscopy to assist intubation of the trachea. Glottic visualization may not be possible in the difficult airway. A delay in reintubation, airway injury, or intubation failure may complicate "blind" tracheal intubation because of excessive endotracheal tube size or tip impingement on airway structures. Advanced laryngoscopic techniques offering "around the corner" visualization may overcome many of the limitations of conventional laryngoscopy's "line of sight." In this data review, I examined the fe

Multidirectional observations of the larynx using transurethral rigid endoscopes during direct laryngoscopy.

Microscopic direct laryngoscopy (microlaryngoscopy) under general anaesthesia is the optimal method of observing the larynx. However, as microlaryngoscopy does not allow precise observations of the ventricle, inferior surface of the vocal fold and subglottis, multidirectional observations of the larynx using transurethral rigid endoscopes were performed during direct laryngoscopy. This endoscopic technique has been shown to be clinically useful in the diagnosis and treatment of laryngeal lesions. The equipment and methods are introduced herein, and a representative care is presented.

Head extension and laryngeal view during laryngoscopy with cervical spine stabilization maneuvers.

BACKGROUND: Direct laryngoscopy frequently is modified in patients with known or suspected cervical spine injury. The goals of this study were to measure the degrees of head extension required to expose the arytenoid cartilages and glottis if neck flexion were not possible and to determine whether in-line stabilization maneuvers alter the amount of head extension. METHODS: The subjects were anesthesized patients with normal cervical spines and Mallampati class 1 oropharyngeal views. Head extension was measured relative to a line drawn perpendicular to the table. Stabilization consisted of eit

Ultrasound-based assessment of tongue thickness for prediction of difficult laryngoscopy and intubation.

BACKGROUND AND AIMS: Predicting difficult airway and preparedness for the same can help prevent catastrophic situations while handling the airway. With the increasing familiarity of anaesthesiologists to the use of ultrasound machine and its easy availability and non-invasiveness, we sought to study its utility in airway assessment, by measuring the thickness of tongue, to predict difficult laryngoscopy and intubation. MATERIAL AND METHODS: A total of 85 patients undergoing elective surgeries under general anaesthesia with endotracheal intubation were examined preoperatively. Tongue thickness

Value of the Hyomental Distance Measured With Ultrasound in Forecasting Difficult Laryngoscopy in Newborns.

PURPOSE: Preoperative evaluations of difficult airways are imperative, especially in newborns. The hyomental distance is a reliable index for predicting difficult airways in adults. However, few studies have evaluated the value of the hyomental distance for predicting difficult airways in newborns. It is unclear whether the hyomental distance forecasts a restricted or difficult view when using direct laryngoscopy. We intended to develop an effective system for predicting difficult tracheal intubation in newborns. DESIGN: A prospective observational clinical study. METHODS: Newborns aged 0 to
