"He's my legs, I'm his rock": experiences of adaptation and change in couple relationships following spinal cord injury.

PURPOSE: Experiencing spinal cord injury (SCI) can cause several challenges and changes within romantic relationships immediately after injury and in the long-term. However, there is a lack of research concerning the formation, maintenance and adjustment of couple relationships post-SCI. This study uses qualitative methods to explore how SCI may impact on couple relationships. METHODS: Four females and five males with SCI participated in a semi-structured interview. Some were with their current partner at the time of injury; others established their relationship following SCI. The data were a

Single-leg Landing Coordination Variability and Second Injury in Young Athletes after Primary Anterior Cruciate Ligament Reconstruction.

PURPOSE: Young, active individuals who undergo anterior cruciate ligament (ACL) reconstruction (ACLR) after an ACL injury often experience suboptimal outcomes, including a high risk of second ACL injury. Coupling angle variability has been used previously to investigate the interactions between joints and their relative coordination, but it has not yet been used to investigate the future risk of subsequent ACL injury during dynamic tasks post-ACLR. As such, the purpose of this work was to examine the relationship between coordination variability and second ACL injury outcomes in young athletes

Is Pain in the Uninjured Leg Associated With Unhelpful Thoughts and Distress Regarding Symptoms During Recovery From Lower Extremity Injury?

BACKGROUND: Patients recovering from lower extremity injuries often interpret discomfort associated with increased use of the uninjured leg as a potential indication of harm. If expressed concerns regarding contralateral leg pain are associated with unhelpful thinking regarding symptoms, they can signal orthopaedic surgeons to gently reorient these thoughts to help improve comfort and capability during recovery. QUESTIONS/PURPOSES: We asked: (1) Among people recovering from isolated traumatic lower extremity injury, is pain intensity in the uninjured leg associated with unhelpful thoughts and

Muscle oxygenation during hybrid arm and functional electrical stimulation-evoked leg cycling after spinal cord injury.

This study compared muscle oxygenation (StO2) during arm cranking (ACE), functional electrical stimulation-evoked leg cycling (FES-LCE), and hybrid (ACE+FES-LCE) exercise in spinal cord injury individuals. Eight subjects with C7-T12 lesions performed exercises at 3 submaximal intensities. StO2 was measured during rest and exercise at 40%, 60%, and 80% of subjects' oxygen uptake (VO2) peak using near-infrared spectroscopy. StO2 of ACE showed a decrease whereas in ACE+FES-LCE, the arm muscles demonstrated increasing StO2 from rest in all of VO2) peak respectively. StO2 of FES-LCE displayed a dec

Electrically-evoked control of the swinging leg after spinal cord injury: open-loop or motion sensor-assisted control?

In paraplegics, gait can be restored by means of functional electrical stimulation (FES). Because the electrophysiological responses of the lower limbs to the neuromuscular stimulus are not completely deterministic, several stimulation strategies have been reported in an attempt to refine stepping motion. In open-loop (OL) systems, the electrical stimulation sequences applied over the leg muscles are often tuned for each patient in order to improve the quality of gait. Our aim was to contrast this traditional technique against variable stimulation sequences based on motion sensors (MS) data fe

[Primary shortening--secondary lengthening. A new treatment concept for reconstruction of extensive soft tissue and bone injuries after 3rd degree open fracture and amputation of the lower leg].

The main problem in major limb replantation--especially of the lower extremity--is an extensive bone- and soft-tissue loss. The traditional replantation concept tries to preserve the initial limb length; only a small shortening is accepted. To avoid a more extensive shortening, often insufficient debridement at the time of replantation is carried out. After successful revascularisation, bone and soft-tissue defects will be reconstructed according to the principles of staged reconstruction. Especially segmental nerve defects of more than one major peripheral nerve and severe skin and muscle los

Effectiveness of Single Leg Isometric Bridge and Nordic Hamstring Exercise Testing for Prediction of Hamstring Injury Risk in Professional Baseball Players.

BACKGROUND: Several investigations have examined various methods of measuring hamstring strength to determine the effectiveness of these tests in predicting hamstring injury (HSI) risk in athletes. However, no previous studies have examined the effectiveness of hamstring strength and interlimb asymmetry utilizing a newly developed modified single leg isometric bridge test (SLIBT) versus a standard Nordic hamstring exercise (NHE) to predict HSI in professional baseball players. PURPOSE: To determine whether preseason hamstring strength and interlimb asymmetry measured using a modified SLBIT ve

One-leg rise performance and associated knee kinematics in ACL-deficient and ACL-reconstructed persons 23 years post-injury.

BACKGROUND: Research indicates reduced knee function and stability decades after anterior cruciate ligament (ACL) injury. Assessment requires reliable functional tests that discriminate such outcomes from asymptomatic knees, while providing suitable loading for different populations. The One-leg rise (OLR) test is common in clinics and research but lacks scientific evidence for its implementation. Our cross-sectional study compared performance including knee kinematics of the OLR between ACL-injured persons in the very long term to controls and between legs within these groups, and assessed th

Closed-loop control of trunk posture improves locomotion through the regulation of leg proprioceptive feedback after spinal cord injury.

After spinal cord injury (SCI), sensory feedback circuits critically contribute to leg motor execution. Compelled by the importance to engage these circuits during gait rehabilitation, assistive robotics and training protocols have primarily focused on guiding leg movements to reinforce sensory feedback. Despite the importance of trunk postural dynamics on gait and balance, trunk assistance has comparatively received little attention. Typically, trunk movements are either constrained within bodyweight support systems, or manually adjusted by therapists. Here, we show that real-time control of

Delayed leg paraplegia associated with hyperextension injury in patients with diffuse idiopathic skeletal hyperostosis (DISH): case report and review of the literature.

Diffuse idiopathic skeletal hyperostosis (DISH) is characterized by calcifications and ossification of ligaments and tendon insertions. The patients with DISH are susceptible to spinal column injury with trivial trauma due to immobilization of vertebrae and this easily leads to spinal cord injury. However, only few reports on hyperextension injuries of the thoracolumbar spine with DISH exist. Here we report three cases of those who developed delayed leg paraplegia after hyperextension injuries of the thoracolumbar spine with DISH. All cases complained only lower back pain without neurological

Effects of foot rotation positions on knee valgus during single-leg drop landing: Implications for ACL injury risk reduction.

BACKGROUND: Non-contact anterior cruciate ligament (ACL) injuries commonly occur when athletes land in high risk positions such as knee valgus. The position of the foot at landing may influence the transmission of forces from the ankle to the knee. Using an experimental approach to manipulate foot rotation positions, this study aimed to provide new insights on how knee valgus during single-leg landing may be influenced by foot positions. METHODS: Eleven male recreational basketball players performed single-leg drop landings from a 30-cm high platform in three foot rotation positions (toe-in,

Muscle morphometric effect of anterior cruciate ligament injury measured by computed tomography: aspects on using non-injured leg as control.

BACKGROUND: Anterior cruciate ligament (ACL) tears are common, functionally disabling, and predispose to subsequent injuries and early onset of osteoarthritis in the knee. Injuries result in muscular atrophy and impaired muscular activation. To optimize surgical methods and rehabilitation strategies, knowledge of the effects of ACL injuries on muscles size and function is needed. Asymmetry due to limb dominance implies that the effect of ACL-injury might be different in right-sided and left-sided injuries which, should be taken in account when evaluating the effect of an injury.
