Robotic assistance in total hip arthroplasty: a systematic review and meta-analysis of leg length, cup orientation, and early outcomes.

This review examined whether robotic assistance alters postoperative leg-length discrepancy (LLD), acetabular cup orientation, or early hip-specific outcomes relative to conventional total hip arthroplasty (THA). We searched PubMed and Web of Science through May 2026 for comparative English-language reports. Study eligibility, data extraction, and methodological appraisal were undertaken independently by two reviewers. Mean differences (MDs) and 95% confidence intervals (CIs) were calculated in Review Manager 5.4. Model selection was based on the target estimand and anticipated clinical and me

Advancements in Managing Bone Defects with Limb Length Discrepancies with a Motorized Intramedullary Combined Transport and Lengthening Nail.

Large bone defects that coexist with limb length discrepancy present a challenging problem in limb reconstruction. Historically, management has required complex circular fixation constructs, staged procedures, or separate devices to independently address each problem. However, the development of motorized intramedullary nails capable of performing both bone transport and limb lengthening with a single implant represent a paradigm shift. This review traces the evolution of internal bone transport technology from the initial foundational principles of distraction osteogenesis through first gener

Growth disturbance in paediatric anterior cruciate ligament reconstruction.

UNLABELLED: Anterior cruciate ligament (ACL) reconstruction in skeletally immature patients represents a significant challenge due to the risk of growth disturbance, including limb length discrepancy (LLD) and angular deformity (AD). Although rare, the consequences of such complications can be substantial. Contributing factors include inadequate clinical and radiological preoperative assessment and insufficient postoperative monitoring. METHODS: This review synthesizes the current understanding of growth disturbances after paediatric ACL reconstruction, examining their incidence, pathophysiol

Differential Effects of Lower Extremity Length Discrepancy on Functional and Structural Scoliosis: Coronal and Sagittal Plane Analysis.

BACKGROUND: Lower extremity length discrepancy (LELD) is closely associated with spinal imbalances such as pelvic tilt and functional scoliosis. While existing studies have focused on LELD-induced coronal compensations, its impact on sagittal spinal balance remains poorly understood. The aim of the current study was to investigate the differential effects of LELD on coronal and sagittal spinal balance in adolescents with functional versus structural scoliosis. METHODS: A retrospective analysis of 211 patients was conducted. Participants were stratified into functional (n=165) or structural sc

Management of a rare fixed knee flexion deformity in adulthood following childhood osteoarticular infection: A case report and literature review.

RATIONALE: Severe fixed knee flexion deformities and significant limb shortening resulting from childhood osteoarticular infections are uncommon in clinical adult orthopedics and present considerable surgical challenges. PATIENT CONCERNS: A 23-year-old female presented with a 130° fixed knee flexion deformity and severe limb shortening. Despite recommendations for amputation from multiple hospitals, the patient insisted on pursuing limb-preserving surgical options. DIAGNOSES: The patient was diagnosed with a severe fixed knee flexion deformity and significant limb shortening due to childhood

Confounding Association of Lower Limb Hypertrophy With Retarded Long Bone Growth in Mosaic Somatic Neurofibromatosis 1-A Genetic Review and Femoral Lengthening Treatment.

This presentation describes a 15-year-old adolescent boy with neurofibromatosis type 1 with regional somatic mosaicism in the right half of his body. The unique clinical features are highlighted by generalized hemihypertrophy of the lower extremity in association with progressive growth retardation of long bones, which results in limb shortening. The patient successfully underwent limb lengthening. To our knowledge, this is the first neurofibromatosis type 1 article to report of a limb-lengthening procedure in a patient with two major and opposite growth phenomena on the same limb caused by a

Arthroplasty for femoral neck fractures is at risk for under restoration of lateral femoral offset.

PURPOSE: The aim of the study was to determine the restoration of hip biomechanics through lateral offset, leg length, and acetabular component position when comparing non-arthroplasty surgeons (NAS) to elective arthroplasty surgeons (EAS). METHODS: 131 patients, with a femoral neck fracture treated with a THA by 7 EAS and 20 NAS, were retrospectively reviewed. 2 blinded observers measured leg-length discrepancy, femoral offset, and acetabular component position. Multivariate logistic regression models examined the association between the surgeon groups and restoration of lateral femoral, ace

Lower back kinetic demands during induced lower limb gait asymmetries.

BACKGROUND: Gait asymmetries are common in many clinical populations (e.g., amputation, injury, or deformities) and are associated with a high incidence of lower back pain. Despite this high incidence, the impact of gait asymmetries on lower back kinetic demands are not well characterized due to experimental limitations in these clinical populations. Therefore, we artificially and safely induced gait asymmetry during walking in healthy able-bodied participants to examine lower back kinetic demands compared to their normal gait. RESEARCH QUESTION: Are lower back kinetic demands different durin

A smaller femoral stem is needed for asian females.

BACKGROUND: Current femoral implants are manufactured based on Western anatomical structures and may be too large for smaller physiques, such as those of Asian females. This study reviewed the femoral stem size distribution used in a high-volume total hip arthroplasty (THA) practice and evaluated malalignment in patients receiving a size one implant. MATERIALS AND METHODS: A consecutive cohort of female patients self-reported as Asian (257 patients, 331 hips) or Caucasian (158 patients and 190 hips) were retrospectively evaluated. A single femoral stem type was used in all cases, performed by

Litigation after primary total hip and knee arthroplasties in France: review of legal actions over the past 30 years.

INTRODUCTION: Since the law of March 4, 2002, several modifications have impacted medical practices and their medico-legal implications. In case of an infectious complication not related to a fault of the practitioners (surgeons, anesthetists), the patient has the right to compensation assumed by the care structures. Moreover, the lack of preoperative information is no longer just a failing of the ethical standards but a breach of medical and legal obligations. Then, aims of this study were: (1) to describe the reasons for a lawsuit in France following total primary hip (THA) and total knee (T

Extracorporal noninvasive acute retraction of STRYDE® for continued lengthening in cases with limited nail stroke: a technical less invasive solution to reload the STRYDE®.

With STRYDE® nails (NuVasive Specialized Orthopedics, San Diego, CA), lengthening nails for full weight-bearing during callus distraction have been available CE-certified since February 2019 in Europe. At present only antegrade femur nails and tibia nails with various lengths and diameters are available. Due to a mismatch of bone length and realizable (implantable) nail length e.g., in cases of skeletal dysplasia or achondroplastic dwarfism, it may happen that a desired lengthening of 6-8 cm cannot be realized because of a nail stroke from only 5 cm. Retrograde usage of antegrade piriformis na

Walking quality after surgical treatment of developmental dysplasia of the hip in children.

We assessed the quality of life of children with developmental dysplasia of the hip (DDH) treated surgically, through analysis of leg length discrepancy, quality of walking and presence of pain in 39 children with DDH between 1991 and 2011 at the University Children's Hospital in Belgrade. Salter's innominate osteotomy combined with derotation and femoral bone shortening was performed. Patients were divided into 3 groups based on their age at operation: the first group included participants operated at age up to 24 months, the second group between 24 and 48 months and the third group above 48

The correction of a 140 mm lower limb difference as a late complication of a childhood meningococcal sepsis with a fully implantable system.

Meningococci (Neisseria meningitidis) are the typical agents of a severe septic shock with fulminate diseases. Many children survive the initial phase of shock today; there are several late complications such as limb differences, large scar areas and impaired soft tissue. The current case report deals with the impaired bone growth and a resulting lower limb difference as a late complication of meningococcal sepsis. Usually, these cases are corrected by external fixator systems. In the present case, a lower leg-length difference of 140 mm was uneventfully corrected in two operative steps by ful

Segmentally arranged basaloid follicular hamartomas with osseous, dental and cerebral anomalies: a distinct syndrome.

A 39-year-old man presented with multiple basaloid follicular hamartomas involving the right side of his body in a systematized pattern following Blaschko's lines. His right leg was 22.5 cm shorter than the left, and rudimentary pre-axial polydactyly was noted on the left hand and the right foot. The teeth of the right maxilla were hypoplastic. DNA analysis of blood lymphocytes and fibroblasts from lesional skin did not reveal any mutation in the Patched gene. On account of this case and of 8 similar cases found in th e literature, the spectrum of a distinct syndrome is delineated. Ipsilateral

Leg-length discrepancy and scoliosis in Marfan syndrome.

Leg-length discrepancy (LLD) greater than 2 cm is rare in the general population and is associated with a lumbar postural scoliosis; it has not been studied in the setting of Marfan syndrome. Thirteen LLDs of 2 cm or more were recorded from a group of 250 Marfan patients visiting a medical genetics clinic for complete care. Diagnosis was confirmed by standing anteroposterior pelvic radiograph and/or scanogram. Records and anteroposterior radiographs of the spine were reviewed to obtain Cobb angles for spinal curvatures. A mean discrepancy of 3.2 +/- 1.0 cm and a Cobb angle of 37.0 degrees +/-
