[Everyday suspicion: our life is delusional].

Suspicious thinking in given situations can be useful and helps the adaptation as events occurring in the world also corroborate this. Factors participating in the development of paranoia can be listed as psychodynamic (projection), salience attributional, neurobiological (dopamine), impaired perceptual (hearing loss), sociocultural (minority, pseudocommunity), self-esteem (worrying, depression, mania), and cognitive (jump-to-conclusion) mechanisms. Along the spectrum of thinking, from the mild to the severe, enhanced worrying, salience attribution, overvalued concepts, suspicion/mistrust, par

Associations among impulsivity, trauma history, and alcohol misuse within a young adult sample.

OBJECTIVE: Young adult alcohol misuse is associated with numerous long-term adverse outcomes. Given the link between impulsivity and alcohol use, we examined whether three impulsivity-related traits differentially predicted number of drinks per drinking day (DDD). We also examined whether these effects varied for those with different trauma histories. METHOD: The current study (n=254) examined motor, non-planning, and attentional impulsivity as predictors of DDD. It also examined whether impulsivity was differentially predictive of DDD across individuals in: a control group (non-trauma expose

WAYS OF ACQUIRING FLYING PHOBIA.

BACKGROUND: The few studies that have explored how flying phobia is acquired have produced contradictory results. We hypothesized that classical conditioning plays a role in acquiring flying phobia and investigated if vicarious (model) learning, informational learning through media, and experiencing stressful life events at the time of onset of phobia also play a role. METHOD: Thirty patients with flying phobia and thirty healthy controls matched on age, sex, and education were interviewed with the Mini-DIPS, the short German version of the Anxiety Disorders Interview Schedule (DSM-IV diagnos

Recurrent factitious subcutaneous emphysema: report of a complex case in a young woman and a literature review.

Subcutaneous emphysema (SE) can be caused by antecedent trauma, surgery, rupture of visceral organs or serious infection from gas-forming microorganisms. Factitious SE is very rare; only about a dozen cases have been reported. Most patients had an underlying psychiatric condition or relevant history. The diagnosis and management of factitious SE are challenging. We report a new case of factitious SE with a very complex clinical course. A 29-year-old woman was seen at consultation for recurrent attacks of painful soft tissue swelling, affecting her chest wall, abdomen and neck over several mont

A test of the integration of the hopelessness and response styles theories of depression in middle adolescence.

The current study examined several theories of depression in a sample of middle adolescents. At Time 1, 367 ninth graders completed measures assessing depressogenic inferential styles, the tendency to ruminate in response to depressed mood, and depressive symptoms. Six weeks later, participants completed measures assessing negative events and depressive symptoms. In line with the hopelessness theory, a depressogenic weakest link interacted with negative events to predict increases in hopelessness depression symptoms but not in more general depressive symptoms. Further, providing partial suppor

The link between judgments of comparative risk and own risk: further evidence.

Individuals typically believe that they are less likely than the average person to experience negative events, a phenomenon termed "unrealistic optimism". The direct method of assessing unrealistic optimism employs a question of the form, "Compared with the average person, what is the chance that X will occur to you?". However, it has been proposed that responses to such a question (direct-estimates) are based essentially just on estimates that X will occur to the self (self-estimates). If this is so, any factors that affect one of these estimates should also affect the other. This prediction

Bereavement: helping a patient with a learning disability to cope.

Our case study is of a 55-year-old man who has a moderate learning disability. He has been diagnosed as having bipolar disorder and is currently in remission. At Christmas six years ago while Dave Croft was on home leave from an NHS unit, his father died suddenly. His mother, acting in what she felt were Mr Croft's best interests, arranged the funeral and did not inform her son until after the ceremony had taken place. Mr Croft appeared to have adjusted to his loss, until November 2001 when he became increasingly upset and agitated at the mention of death. A multidisciplinary team approach and

Amnestic state in a Holocaust survivor patient: psychogenic versus neurological basis.

Differentiation between psychogenic and organic amnesia is sometimes quite difficult. This paper focuses on the psychogenic and organic components of a complex case of amnesia rooted in remote and prolonged traumatic stress and manifested under circumstances evoking dissociated memories. The Transient Global Amnesia (TGA) of a concentration camp survivor who developed sudden amnesia during a psychiatric intake interview was clearly triggered by the pressure of repressed Holocaust memories. The importance of distinguishing between TGA and dissociative amnesia is emphasized, and the role of psyc

Causes of child abuse and neglect.

This paper is a study of child abuse and neglect from the perspective of the child. Generally, the mistreatment of children was associated with "poor care" from parents, attributed mainly to immaturity, marital problems, alcohol abuse, unemployment, drug abuse and lack of money. Differences in attribution are noted between males and females, and some differences are noted by the age of the child. When factors other than the causes given by the children were taken into account, mistreatment was significantly related to family break-up, as well as long-term disinterest and lack of affection from

The birthday: lifeline or deadline?

This study of deaths from natural causes examined adult mortality around the birthday for two samples, totalling 2,745,149 people. Women are more likely to die in the week following their birthdays than in any other week of the year. In addition, the frequency of female deaths dips below normal just before the birthday. The results do not seem to be due to seasonal fluctuations, misreporting on the death certificate, deferment of life-threatening surgery, or behavioral changes associated with the birthday. At present, the best available explanation of these findings is that females are able to

Biological correlates of panic disorder with agoraphobia: practice perspectives for nurses.

The literature on panic disorder is summarized with regard to epidemiological, environmental, cultural, psychological, family, and genetic factors. Laboratory provocation of anxiety is reviewed with emphasis on the noradrenergic model of panic from a biological perspective. Positron emission tomography results involving brain asymmetries in lactate-sensitive panic patients are discussed, as are the psychopharmacological agents currently effective in the treatment of panic disorder. It is suggested that psychiatric nurses base their clinical practice on a holistic understanding of treatment imp

Mental and social problems of families with handicapped child.

Studies were carried out in a series of 100 mothers of children with facial clefts who were treated up to the age of 13 to 15 years at the Department for Plastic Surgery. Clefts were used as a model situation for studies into the response of parents--and especially of mothers--to the birth of a child with an inborn malformation. The main results showed: that three quarters of mothers designated the birth of a handicapped child as a mental shock followed by a variety of neurotic symptoms. A certain proportion of mothers suffered from reactive depression which was not diagnosed and therefore not

Anxiety, panic and phobic disorders: an overview.

This paper reviews anxiety, panic, and phobic disorders as they were described in landmark works, along with more recent epidemiologic studies of the disorders. The author discusses clinical syndromes of anxiety as outlined in the DSM-III: agoraphobia, social phobia, generalized anxiety disorder, panic disorder, simple phobic states, and obsessive-compulsive disorder, relating them to Phobic Anxiety-Depersonalization Syndrome and to earlier descriptions by Westphal and Benedict. The paper addresses the problem of delineating anxiety and phobic states from depressive disorders, with regard to d

Depressive disorder associated with physical illness. The impact of stroke.

The consultation-liaison psychiatrist is confronted time after time with depression in the context of physical illness, and the link between the two is of practical and theoretical interest. In addressing this topic, current classification systems are ambiguous. The advent of research diagnostic criteria and the primary/secondary depression dichotomy have not clarified the problem as was hoped. The conventional view that physical illness only precipitates depression in the genetically predisposed has been challenged by recent studies in patients with stroke. These studies point to a specific r

Life stress and psychopathology: progress on research begun with Barbara Snell Dohrenwend.

This is a progress report on some of the research that was planned and begun with Barbara Dohrenwend before she died in 1982. The main focus is on two of the studies. One was conducted in New York City; the other is still underway in Israel. The New York study is a retrospective case/control study of social and psychological factors that may put people at risk for developing schizophrenic episodes and episodes of major depression. The Israel research consists of epidemiological, case/control, and family studies of these two disorders together with other types of psychopathology that are invers
