Stories about self-perceived quality of life from adults with Down syndrome.

This study explores the self-perceived quality of life among five adults with Down syndrome in Norway. Data were collected through qualitative semi-structured interviews with five adults with Down syndrome, supplemented by the Personal Wellbeing Index - Intellectual Disability, and analysed using reflexive thematic analysis. The findings highlight four themes contributing to their quality of life: (1) Adulthood, autonomy and independence, (2) Health experiences, self-care, and navigating barriers, (3) Pursuing dreams and ambitions, and finding joy in daily life, (4) Freedom and leisure: person

The adolescent celiac experience: a holistic perspective on self-care among the "invisible" segment of society.

PURPOSE: Strict adherence to a gluten-free diet, combined with the developmental needs of adolescence, can profoundly impact the lives of adolescents with celiac disease. This study aims to explore the life experiences of such individuals, a population often overlooked in society. MATERIALS AND METHODS: The study utilized Interpretative Phenomenological Analysis (IPA) in semi-structured interviews with 14 adolescents (11 females and 3 males) aged 14-19 years. RESULTS: Four experiential themes were identified: (1) The Role of Celiac Disease in Life Dimensions; (2) Strategies for Self-Care Mai

Siblings in families of children with chronic disorders: a model of risk and protective factors.

OBJECTIVE: We empirically tested a family systems model's ability to explain variance in psychosocial adjustment among siblings of children with chronic developmental and physical disorders (CDs). The model included the impact of CD severity, family social support, parental mental health, and parent-sibling communication quality. We hypothesized that family social support would moderate the relationship between CD severity and parent mental health, and that sibling-reported communication quality with their parents would mediate the relationship between parent mental health and sibling psychoso

The expectations we build: reframing chronic pain outcomes as a systems level public health issue.

Chronic pain is a public health issue that affects a substantial proportion of adults and limits life, work, and social participation. Although pain is understood as a multidimensional experience shaped by biological, psychological, social, and contextual factors, chronic pain care is often organized around the expectation that pain can be identified, treated, and resolved through targeted clinical intervention. This perspective argues that chronic pain outcomes are often misattributed to individual factors such as adherence, motivation, or resilience, when they may also reflect expectations g

In vitro cultivation methods for coccidian parasite research.

The subclass Coccidia comprises a large group of protozoan parasites, including important pathogens of humans and animals such as Toxoplasma gondii, Neospora caninum, Eimeria spp., and Cystoisospora spp. Their life cycle includes a switch from asexual to sexual stages and is often restricted to a single host species. Current research on coccidian parasites focuses on cell biology and the underlying mechanisms of protein expression and trafficking in different life stages, host cell invasion and host-parasite interactions. Furthermore, novel anticoccidial drug targets are evaluated. Given the v

When Technology Meets Its Limits: Integrating Medical Aid in Dying With Withdrawal of a Left Ventricular Assist Device.

Many patients in the United States live with left ventricular assist devices - surgically implanted mechanical pumps that support circulation in individuals with advanced heart failure. The use of these devices has grown steadily, with new implants performed each year and many patients maintained with ongoing longitudinal care. As outcomes improve and patients live longer with this form of mechanical support, clinicians across disciplines are increasingly confronted with the complex clinical, ethical, and practical challenges that arise when life-sustaining therapy is dependent on implanted te

Rethinking Symptom Fluctuation in Multimorbidity: A Dynamic Systems Framework From Primary Care.

BACKGROUND: Symptom fluctuation is a common but poorly understood feature of multimorbidity in primary care. Conventional disease-centred frameworks provide limited explanation for day-to-day variability in symptoms such as fatigue, reduced activity tolerance and dyspnoea. AIM: To propose a conceptual framework that explains symptom fluctuation in multimorbidity as a dynamic systems phenomenon. CONCEPTUAL APPROACH: Drawing on clinical observation and theoretical integration, we conceptualise symptom expression as emerging from dynamic configurations of interacting physiological systems withi

Considerations in designing trauma-focused interventions for displaced Afghan women.

In light of the 2021 United States military withdrawal from Afghanistan, as well as the humanitarian crises of mass displacement and subsequent health system strain that have ensued, practitioners worldwide will need to develop a more nuanced understanding of the adverse life experiences that women from Afghanistan frequently endorse. As they bear a disproportionate impact of constraints within Afghan society, and as patriarchal systems affect most of their life domains, women from Afghanistan may present with high levels of baseline trauma upon resettlement, and health systems may seek to att

Death revisited: rethinking death and the dead donor rule.

Traditionally, people were recognized as being dead using cardio-respiratory criteria: individuals who had permanently stopped breathing and whose heart had permanently stopped beating were dead. Technological developments in the middle of the twentieth century and the advent of the intensive care unit made it possible to sustain cardio-respiratory and other functions in patients with severe brain injury who previously would have lost such functions permanently shortly after sustaining a brain injury. What could and should physicians caring for such patients do? Significant advances in human o

Review article: the current management of acute liver failure.

BACKGROUND: Acute liver failure is a devastating clinical syndrome with a persistently high mortality rate despite critical care advances. Orthotopic liver transplantation (OLT) is a life-saving treatment in selected cases, but effective use of this limited resource requires accurate prognostication because of surgical risks and the requirement for subsequent life-long immunosuppression. AIM: To review the aetiology of acute liver failure, discuss the evidence behind critical care management strategies and examine potential treatment alternatives to OLT. METHODS: Literature review using Ovid

Active health behaviors in patients after thyroid cancer surgery: a qualitative study of barriers and facilitators.

AIM: To identify the factors that promote or hinder active health behaviors in patients following thyroid cancer surgery, and to provide evidence to support the development of relevant interventions for this population. BACKGROUND: In recent decades, the incidence of thyroid cancer has surged globally, nearly doubling in some developed countries. China has similarly witnessed an upward trend in thyroid cancer rates. In the management of patients following thyroid cancer surgery, active health behaviors play a pivotal role. Under the current healthcare model, patients' engagement and self-mana

Barriers for access and utilization of dementia care services in Europe: a systematic review.

BackgroundDementia is a group of chronic diseases characterised by cognitive impairment that progressively disrupts daily functioning and requires increasing levels of healthcare, social support, and long-term care. Support for people with dementia can be provided by formal support systems although most of the care process relies upon informal care givers. Despite the availability of formal support systems and healthcare workers, the utilization of dementia care services remains suboptimal. Factors such as non-compliance, lack of awareness, and poor care coordination contribute to this issue.

Full recovery with left ventricular unloading via venoarterial extracorporeal membrane oxygenation and use of emapalumab in severe metapnomovirus myocarditis presenting with cardiogenic shock.

Fulminant myocarditis is a life-threatening event that can present as cardiogenic shock. Human metapneumovirus (hMPV)-associated myocarditis is exceptionally uncommon, particularly in the pediatric population. Treatment may require mechanical ventilation, inotropic agents, vasopressors, and advanced life support systems. In this article, we report an 18-month-old previously healthy infant who presented with severe metabolic acidosis, elevated lactate, and profound biventricular systolic dysfunction secondary to hMPV infection. Despite mechanical ventilation, inotropic support, and initial immu…

Effects of a clinical decision support system and patient portal for preventing medication-related falls in older fallers: Protocol of a cluster randomized controlled trial with embedded process and economic evaluations (ADFICE_IT).

BACKGROUND: Falls are the leading cause of injury-related mortality and hospitalization among adults aged ≥ 65 years. An important modifiable fall-risk factor is use of fall-risk increasing drugs (FRIDs). However, deprescribing is not always attempted or performed successfully. The ADFICE_IT trial evaluates the combined use of a clinical decision support system (CDSS) and a patient portal for optimizing the deprescribing of FRIDs in older fallers. The intervention aims to optimize and enhance shared decision making (SDM) and consequently prevent injurious falls and reduce healthcare-related co

Optimal treatment for severe neurogenic bowel dysfunction after chronic spinal cord injury: a decision analysis.

BACKGROUND: When conservative management fails in patients with chronic spinal cord injury (SCI) and neurogenic bowel dysfunction, clinicians have to choose from a variety of treatment options which include colostomy, ileostomy, Malone anterograde continence enema (MACE) and sacral anterior root stimulator (SARS) implantation. This study employed a decision analysis to examine the optimal treatment for bowel management of young individuals with chronic refractory constipation in the setting of chronic SCI. METHODS: A decision analysis was created to compare the four surgical strategies using
