Linezolid compared with teicoplanin for the treatment of suspected or proven Gram-positive infections.

The efficacy, safety and tolerability of linezolid was compared with teicoplanin in a randomized, controlled, open-label, multicentre study of 430 patients with suspected or proven Gram-positive infection. Patients received intravenous (iv) +/- oral linezolid 600 mg every 12 h (n = 215) or iv or intramuscular teicoplanin (n = 215) for up to 28 days. Clinical outcomes in the intent-to-treat (ITT) and clinically-evaluable populations and microbiological success rates in microbiologically evaluable patients were assessed at follow-up (test of cure). Investigator assessed clinical cure rates at en…

Linezolid for treatment of ventilator-associated pneumonia: a cost-effective alternative to vancomycin.

OBJECTIVES: To determine the incremental cost-effectiveness of linezolid compared with vancomycin for treatment of ventilator-associated pneumonia due to Staphylococcus aureus. DESIGN: Decision model analysis of the cost and efficacy of linezolid vs. vancomycin for treatment of ventilator-associated pneumonia. The primary outcome was the incremental cost-effectiveness of linezolid in terms of cost per added quality-adjusted life-year gained. Other outcomes were the marginal costs per hospital survivor and per year of life saved generated by using linezolid. Model estimates were derived from p…

Lactate abnormalities during intravenous linezolid therapy in hospitalized children: a brief report.

Linezolid is commonly used in children for the treatment of severe gram-positive infections; however, data on metabolic adverse effects remain limited. We evaluated lactate abnormalities in pediatric patients receiving intravenous linezolid. Among 215 children, hyperlactatemia/lactic acidosis occurred in 55.8%. Hyperlactatemia and/or lactic acidosis tended to develop early after treatment initiation and was more frequently observed in younger children and in those with complex underlying conditions, including neurometabolic and hemato-oncologic disorders. These findings suggest that host-relat…

Vancomycin versus linezolid for treatment of staphylococcal-associated central nervous system infections.

BACKGROUND: Linezolid and vancomycin are both recommended for the treatment of staphylococcal-associated central nervous system (CNS) infections. However, to date, no data are available comparing the outcomes of patients treated with vancomycin or linezolid for these infections. The aim of this study was to compare the incidence of treatment failure and adverse events (AEs) associated with vancomycin and linezolid in staphylococcal-associated CNS infections. METHODS: This retrospective monocentric observational study was conducted between 01/01/2015 and 31/12/2023. All patients with a confirm…

Linezolid use in post-neurosurgical central nervous system infections: A systematic review.

Post-neurosurgical central nervous system (CNS) infections are associated with prolonged hospital stays and poor neurological outcomes. Given the predominance of Gram-positive bacteria as causative pathogens, vancomycin is the first-line treatment. However, its use is limited by nephrotoxicity, the global emergence of resistant strains, and unfavorable pharmacokinetics with poor cerebrospinal fluid penetration. Therefore, linezolid may be a promising alternative. This systematic review assesses the current evidence regarding its efficacy and safety in the treatment of post-neurosurgical CNS in…

Linezolid induced toxic optic neuropathy in drug resistant tuberculosis-A case series.

In developing countries like India, Linezolid is widely used for the treatment of Multi drug resistant tuberculosis (MDR-TB) and extensively drug resistant tuberculosis (XDR-TB). Long-term administration of Linezolid is reported to cause toxic optic neuropathy causing bilateral, progressive visual loss in patients. We report case details of three patients on anti-tubercular therapy presented to us with sudden, progressive, painless blurring of vision of both eyes the cause of which was confirmed to be toxic optic neuropathy due to linezolid. Subsequently, cessation of the drug resulted in comp…

Activity of vancomycin combined with linezolid against clinical vancomycin-resistant Enterococcus strains.

INTRODUCTION: Because multi-drug-resistant Gram-positive bacteria have been isolated frequently worldwide and are difficult to treat, alternative treatment choices are required. Combination antibiotherapies have a distinct advantage over monotherapies in terms of their broad spectrum and synergistic effect. In the present study, it was aimed to investigate the in vitro activity of vancomycin combined with linezolid against clinical vancomycin-resistant enterococci (VRE) strains with high-level aminoglycoside resistance. MATERIAL AND METHODS: A total of 30 randomly selected clinical VRE strain…

Emergence of optrA-mediated linezolid resistance in enterococci from France, 2006-16.

OBJECTIVES: To describe the epidemiological trend of linezolid-resistant enterococci (LRE) collected in France from 2006 to 2016 and to extensively characterize LRE isolates. METHODS: The National Reference Center for Enterococci (NRC-Enc) received enterococcal isolates suspected to be VRE and/or LRE from all French hospitals between 2006 and 2016. LRE isolates were phenotypically characterized and their genomes were entirely sequenced by Miseq (Illumina). Transfer of linezolid resistance was attempted by filter mating experiments. RESULTS: Out of 3974 clinical isolates of enterococci receiv…

Predictors of Inadequate Linezolid Concentrations after Standard Dosing in Critically Ill Patients.

Adequate linezolid blood concentrations have been shown to be associated with an improved clinical outcome. Our goal was to assess new predictors of inadequate linezolid concentrations often observed in critically ill patients. Fifty-two critically ill patients with severe infections receiving standard dosing of linezolid participated in this prospective observational study. Serum samples (median, 32 per patient) were taken on four consecutive days, and total linezolid concentrations were quantified. Covariates influencing linezolid pharmacokinetics were identified by multivariate analysis and…

Epidemiological and Genomic Insights into Linezolid-Non-Susceptible Enterococci in Pediatric Patients.

Enterococci are major opportunistic pathogens causing healthcare-associated infections in children. Linezolid, a WHO-designated critically important antibiotic for multidrug-resistant Gram-positive infections, is increasingly challenged by linezolid-non-susceptible enterococci (LNSE). Yet pediatric LNSE epidemiology and genomics data remain scarce, hindering targeted control. We analyzed 26 LNSE strains isolated from Children's Hospital, Zhejiang University School of Medicine (June 2020-July 2024) using MALDI-TOF MS, Vitek2 Compact, micro-broth dilution (for linezolid MIC), MLST, resistance/vi…

Continuous linezolid infusion: the new way to dose, the old risks to fear.

Continuous infusion (CI) of linezolid has been proposed to stabilize exposure and improve pharmacokinetic/pharmacodynamic (PK/PD) target attainment compared with conventional intermittent dosing. By maintaining concentrations above inhibitory thresholds, CI may optimize antibacterial efficacy and reduce inter- and intra-patient variability. However, this 'new way of dosing' revisits 'old risks' associated with linezolid, including nonlinear PK, metabolic saturation, and concentration-dependent toxicities such as lactic acidosis, myelosuppression and neurotoxicity. Rather than eliminating these…

Effects of florfenicol on linezolid resistance in Enterococcus faecium from waterfowl farms.

This study aimed to investigate the antimicrobial resistance profiles of Enterococcus faecium (E. faecium) in waterfowl farms across selected provinces in China and to explore the impact of florfenicol on linezolid resistance in E. faecium. Between 2022 and 2025, 490 E. faecium strains were isolated from samples collected from duck and goose farms in 11 Chinese provinces. These isolates exhibited high resistance rates to multiple antibiotics, with a linezolid resistance rate of 69.2% (339/490). Transferable linezolid resistance genes were identified as the primary mechanism underlying linezoli…

Adverse effects of linezolid in the treatment of drug-resistant tuberculosis: a scoping review.

BACKGROUND: Linezolid (LZD) has emerged as a key drug for multidrug-resistant (MDR) and extensively drug-resistant (XDR) tuberculosis (TB) treatment. However, its clinical use is limited by substantial toxicity. This scoping review aimed to synthesise evidence on LZD-associated adverse effects (AEs) to support safer and more effective TB management clinical decisions. METHODS: A systematic search of PubMed, Scopus, Web of Science, and EMBASE databases (conducted in October 2024) identified 103 primary studies published between 2005 and 2024 that reported AEs related to LZD use in MDR/XDR-TB. …

Therapeutic drug monitoring of linezolid in a case of pulmonary nocardiosis: a case report.

This clinical case discusses a man between 60 and 70 years old with lung adenocarcinoma who developed a Nocardia cyriacigeorgica infection. Because it is an intracellular bacterium, we need to start antibiotic treatment and maintain for a year. For initial therapy, sulfamethoxazole/trimethoprim (SMX/TMP) is the preferred antibiotic, but it can cause side effects, such as liver damage. Linezolid is another valid alternative.Pharmacokinetic monitoring of linezolid was essential in managing the patient because the drug can cause significant side effects like myelosuppression. The pharmacist track…

A Unified Approach to Synthesizing Four Linezolid Metabolites That May Cause Thrombocytopenia.

Background/Objectives: Linezolid is a first-in-class oxazolidinone antibiotic that exhibits activity against Gram-positive pathogens, including methicillin-resistant Staphylococcus aureus and vancomycin-resistant enterococci. However, its clinical use is often restricted because of hematological toxicities, particularly thrombocytopenia, in patients with renal impairment. That side effect is thought to result from the systemic accumulation of pharmacologically inactive metabolites generated by oxidative degradation and ring-opening of the morpholine, but the details remain unclear. In this stu…

Population pharmacokinetics and thrombocytopenia risk assessment of linezolid in liver transplant recipients.

BACKGROUND: Linezolid is a commonly prescribed antibiotic for multidrug-resistant enterococcal infections in liver transplant recipients (LTRs). However, changes in pharmacokinetics due to fluctuations in liver and renal functions, combined with the increased risk of thrombocytopenia, complicate its clinical use. This study aimed to characterize the exposure-thrombocytopenia risk relationship of linezolid in LTRs, and to identify safe dosing thresholds to promote rational drug use. METHODS: A retrospective analysis was conducted on adult LTRs treated with linezolid at Zhongshan Hospital betwe…

Linezolid and levofloxacin: an uncommon pairing that suppresses evolution of resistance in E. faecalis.

Drug combinations offer one potential strategy for slowing the evolution of antibiotic resistance. In this work, we investigate the evolution of drug resistance (strictly, the decrease in susceptibility) in populations of Enterococcus faecalis, an opportunistic human pathogen, exposed to different concentrations of the rarely used combination of linezolid (LZD) and levofloxacin (LEV). Using continuous culture bioreactors, we measured the two-dimensional dose-response surface of ancestral populations of E. faecalis, revealing that the LZD-LEV combination is strongly antagonistic, resulting in i…
