Assessment and Reversibility of Hepatic Abnormalities in Heart Failure: Liver in Heart Failure.

Hepatic abnormalities in non-Fontan heart failure result from congestive hepatopathy and hypoperfusion-related ischemic injury, compounded by neurohormonal activation and systemic inflammation. Assessment of liver disease severity is challenging because standard liver biochemistries, serum fibrosis markers, severity score models, elastography, and imaging modalities have limited diagnostic accuracy in congestive hepatopathy, and hepatic fibrosis is often heterogeneous on histology in this population. Management focuses on optimization of cardiac function through medical therapy or mechanical c

Late-Onset Graft Failure in Pediatric Liver Transplant Recipients: Implications of Acute-on-Chronic Liver Failure on Retransplantation Outcomes.

BACKGROUND: As short-term outcomes following pediatric liver transplantation improve, an increasing burden of morbidity arises from chronic graft dysfunction warranting retransplantation. In this single-center retrospective cohort study, we aim to describe outcomes and determine factors that influence morbidity and mortality following liver retransplantation in pediatric recipients. METHODS: Data were collected on patient, and surgical factors which may influence survival and morbidity in all recipients < 18 years of age undergoing liver retransplantation between 2010 and 2024. Retransplants

Liver Failure from Alcohol Takes on Several Forms: Decompensated Cirrhosis, Acute-on-Chronic Liver Failure, and Alcohol-Related Hepatitis.

Liver failure from alcohol can take on several forms, including decompensated alcohol-related cirrhosis (AC), acute-on-chronic liver failure (ACLF), and alcohol-related hepatitis (AH). While the definitions of decompensated AC and AH are well-established concepts in alcohol-related liver disease (ALD), ACLF is a contemporary and controversial concept of a severe form of acutely decompensated cirrhosis characterized by the existence of organ system failure(s) and a high risk of short-term mortality. While these terminologies have overlapping features, they are distinct and useful by capturing t

Effects of liver failure on branched-chain alpha-keto acid dehydrogenase complex in rat liver and muscle: comparison between acute and chronic liver failure.

BACKGROUND/AIMS: Branched-chain alpha-keto acid dehydrogenase (BCKDH) complex catalyses the committed step in the branched-chain amino acid (BCAA) catabolic pathway. In many cases of liver failure, the serum BCAAs/aromatic amino acids ratio (Fisher's ratio) decreases, and BCAAs have been administered to patients with liver failure to correct this ratio. We conducted an animal study to examine whether the effects on hepatic BCKDH complex differ between acute liver failure (ALF) and chronic liver failure (CLF). METHODS: ALF and CLF was induced in rats by a single high-dose injection and 21 week

Human liver progenitor-like cells-derived extracellular vesicles promote liver regeneration during acute liver failure.

Hepatocyte-derived liver progenitor-like cells (HepLPCs) exhibit a remarkable capacity to support liver function by detoxifying ammonia, promoting native liver regeneration, and suppressing inflammation, which leads to improvements in the recovery and survival of animals with acute liver failure (ALF). However, the mechanism through which HepLPCs promote liver regeneration is unclear. Here, we isolated HepLPC-derived extracellular vesicles (HepLPC-EVs) from conditioned media and performed microRNA sequencing analysis. Our results showed HepLPC-EVs promoted liver regeneration in mice with carbo

Acute on chronic liver failure in non-alcoholic fatty liver and alcohol associated liver disease.

Acute on chronic liver failure (ACLF) presents acutely with multiple organ failure and is precipitated by an acute event. The syndrome has high short-term mortality with a potential of returning to baseline liver function if the precipitating event is controlled and patient/s survive the acute event. With heterogeneous definition across the globe of this syndrome, there is a clinical unmet need to homogenize this definition as basis for developing pathogenesis targets, collaboration across countries and centers, and identifying new therapeutic targets. Although, the syndrome can occur in any c

Acute fatty liver of pregnancy with transient resolution preceding postpartum liver failure requiring liver transplant.

OBJECTIVES: Acute fatty liver of pregnancy (AFLP) is a rare, potentially fatal complication of unknown etiology that occurs in the third trimester or early postpartum and can be associated with adverse maternal and fetal outcomes. The purpose of this report is to share a case of AFLP in which a period of objective and symptomatic resolution preceded delayed postpartum liver failure and liver transplant. CASE PRESENTATION: A 35-year-old G3P0020 female experienced preterm premature rupture of membranes (PPROM) at 32 weeks' gestation and AFLP. She delivered vaginally and despite apparent initial

Liver transplantation for acute liver failure from drug induced liver injury in the United States.

Studies of acute liver failure from drugs have included cases mostly attributed to acetaminophen (APAP) but have reported limited data on other drugs. We used the United Network for Organ Sharing (UNOS) liver transplant database from 1990 to 2002 to identify recipients and estimate a U.S. population-based rate of liver transplantation due to acute liver failure from drugs. Patients were identified if their diagnosis was acute hepatic necrosis from an implicated drug at the time of transplant. Liver transplantation for drug hepatotoxicity accounted for 15% of liver transplants for acute liver f

Primary Small Cell Carcinoma of the Liver Presenting With Acute Liver Failure.

Primary small cell carcinoma of the liver is an exceptionally rare malignancy and must be distinguished from metastatic pulmonary small cell carcinoma by excluding a pulmonary primary. We report a 55-year-old man with suspected alcohol-related cirrhosis and diabetes who presented for elective lithotripsy and was found to have jaundice, hepatomegaly, and acute liver dysfunction. Laboratory evaluation demonstrated marked hyperbilirubinemia and transaminitis with normal alpha-fetoprotein. Imaging revealed interval hepatomegaly with diffuse hepatic involvement compared to CT performed three months

ABO incompatible auxiliary partial orthotopic liver transplantation for paediatric acute liver failure.

Although a successful auxiliary partial orthotopic liver transplantation (APOLT) demands technical expertise, it provides an opportunity for a post-liver transplant (LT) immunosuppression-free life. With the advent of newer medications and an implicit understanding of the post-LT immunopathophysiology, clinicians have successfully breached the ABO barrier. However, given the time constraints among other factors, the immunological outcomes of this ABO incompatible LT (ABOiLT) remain suboptimal in the setting of an acute liver failure (ALF). Here, we report an urgent ABOi-APOLT in a child with A

Hepatitis A Liver Failure in Children: Native Liver Survival Despite Poor Prognosis.

BACKGROUND: Hepatitis A virus (HAV) infection is the commonest cause of pediatric acute liver failure (PALF) in developing countries. Literature has shown good outcomes of HAV-induced PALF as compared to other etiologies. The advanced critical care and use of extracorporeal liver assist devices (ELAD) have improved the survival with native liver in PALF and overall outcomes. Various liver transplant listing criteria have been proposed in PALF, however none of them is specific enough to predict the outcome. The timing of liver transplant in living donor setting has never been straightforward. D

[Management of liver transplantation perioperative period in acute-on-chronic liver failure].

Acute-on-chronic liver failure (ACLF) is a potentially reversible entity that occurs in patients with chronic liver disease accompanied with or without cirrhosis and is characterized by extrahepatic organ failure and high short-term mortality. Currently, the most effective treatment method for patients with ACLF is liver transplantation; therefore, admission timing and contraindications must be emphasized. The function of vital organs such as the heart, brain, lungs, and kidneys should be actively supported and protected during the liver transplantation perioperative period in patients with AC

Gut Microbiota and Neuroinflammation in Acute Liver Failure and Chronic Liver Disease.

Acute liver failure and chronic liver disease are associated with a wide spectrum of neurological changes, of which the best known is hepatic encephalopathy (HE). Historically, hyperammonemia, causing astrocyte swelling and cerebral oedema, was considered the main etiological factor in the pathogenesis of cerebral dysfunction in patients with acute and/or chronic liver disease. However, recent studies demonstrated a key role of neuroinflammation in the development of neurological complications in this setting. Neuroinflammation is characterized by activation of microglial cells and brain secre

Outcomes Analyses of Pediatric Acute Liver Failure Subjects Listed for Liver Transplantation.

BACKGROUND: We characterized recent outcomes in US pediatric acute liver failure (PALF) subjects listed for liver transplantation (LT) using the Scientific Registry of Transplant Recipients (SRTR) database. METHODS: Pediatric subjects listed for LT from 2002 to 2015 were assigned to the "PALF" group based on status 1/1A listing, INR >2, no hepatic artery thrombosis, and no primary graft nonfunction (N = 397). Subjects were assigned to the "non-PALF" group if listed with any status other than 1/1A (N = 4509). RESULTS: The PALF group had more infants <3 months of age and males at listing for L

Acute-on-chronic liver failure in patients with alcohol-related liver disease.

The spectrum of alcohol-related liver diseases (ALD) includes steatosis, steatohepatitis, progressive liver fibrosis, and cirrhosis. Acute-on-chronic liver failure (ACLF) is a recently defined entity that occurs in patients with chronic liver diseases and is characterised by acute decompensation, organ failures and a high risk of short-term mortality. Active alcohol consumption, alcoholic hepatitis and bacterial infections are the most frequent events precipitating the development of ACLF in the context of ALD (ALD-ACLF). The specific management of this entity remains unknown and the place of

Gut-Liver Axis Links Portal Hypertension to Acute-on-Chronic Liver Failure.

Acute-on-chronic liver failure (ACLF) is considered a distinct syndrome in patients with liver disease, with systemic inflammation playing a central role. Portal hypertension (PHT) is also aggravated by inflammation and may subsequently impact the course of ACLF. PHT is more than just an increase in portal pressure in the portal venous system; it aggravates the course of liver disease and, thus, also facilitates the development of acute decompensation and ACLF. A critical mechanistic link between PHT and ACLF might be the gut-liver axis, which is discussed in this review.

Live donor liver transplantation for acute liver failure: A single center experience.

INTRODUCTION: Acute liver failure (ALF) is an indication for emergency liver transplantation (LT). Although centers performing only deceased donor liver transplants (DDLT) have shown improved outcomes in this situation, they still have relatively long waiting lists. An alternative would be living donor liver transplantation (LDLT), which has shown equivalent outcomes in the elective situation but there is limited evidence of its results in ALF. AIM: The purpose of this study was to assess the outcomes in patients with ALF undergoing emergency LDLT in our center in Delhi, India. METHODS: We p
