Human esophageal pressures and chest wall configuration in upright and head-down posture.

Pressures were measured at two levels in the esophagus in 14 young healthy subjects performing slow inspiratory and expiratory vital capacity (VC) maneuvers in upright and head-down posture (180 degrees body tilt). In both postures, a gravitational pressure gradient was found, which increased very slightly with decreasing lung volumes (0.006 cmH2O X % VC-1 X cm descent-1) except for upright expiratory curves above 60% VC. The expiratory pressure gradient tended to be larger in head-down than in upright posture; however, during inspiration the opposite was true. In both postures the pressure ch

Lung volume dependence of esophageal pressure in the neck.

There is conflicting evidence in the literature regarding tissue pressure in the neck. We studied esophageal pressure along cervical and intrathoracic esophageal segments in six healthy men to determine extramural pressure for the cervical and intrathoracic airways. A balloon catheter system with a 1.5-cm-long balloon was used to measure intraesophageal pressures. It was positioned at 2-cm intervals, starting 10 cm above the cardiac sphincter and ending at the cricopharyngeal sphincter. We found that esophageal pressures became more negative as the balloon catheter moved from intrathoracic to

[Effect of potent analgesics on the ventilatory CO2 response and on the mouth occlusion pressure (author's transl)].

The depressant effects of four potent analgesics on the respiration were studied in 9 healthy young persons by determining the CO2 response (Read's method) and the mouth occlusion pressure (Whitelaw's method) before and after administration of the drugs and with or without interposition of additional inspiratory resistance (Rin). The analgesics tested were: Mr. 1268 = M1; pentazocine = M2; tramadol = M3; Mr. 1353 = M4. M1 and M2 induced a marked reduction in the ventilatory CO2 response (56.2 per cent +/- 21.8 for M1 and 53.8 per cent +/- 17.1 for M2). Interposition of Rin reduced the response

Lung hypoplasia in congenital pulmonary valve stenosis.

The pulmonary function of ten adult patients with congenital pulmonary valvular stenosis was investigated. The patients clearly showed smaller lungs than healthy control subjects of equivalent age and height; lung elastic recoil pressure was normal at any given percentage of measured total lung capacity, indicating that postnatal parenchymal damage is not the cause of the small lungs. The lung diffusing capacity for carbon monoxide was reduced, reflecting the anatomical alterations of the pulmonary vascular bed. Finally, the maximal flow-static recoil curves showed a fixed (not dynamic) reduct

Understanding the experiences of lung volume recruitment among boys with Duchenne muscular dystrophy: A multicenter qualitative study.

BACKGROUND: Despite recommendations for regular lung volume recruitment (LVR) use in clinical practice guidelines for children with neuromuscular disease, adherence to LVR is poor. We aimed to describe the experience of LVR by boys with Duchenne muscular dystrophy (DMD), their families, and healthcare providers (HCPs), as well as to identify the barriers and facilitators to LVR use. METHODS: This multicenter, qualitative study evaluated boys with DMD (n = 11) who used twice-daily LVR as part of a randomized controlled trial, as well as their parents (n = 11), and HCPs involved in the clinical

[A case of asthma relieved by aspirin--the first case in Japan and investigation of its mechanism].

We report a case of asthma that was relieved by aspirin and other cyclooxygenase inhibitors. The patient, a 51-year-old man, was admitted to our hospital because of an asthmatic attack. Onset of asthma had occurred at the age of 40 years, after a flu-like infection, and was preceded for several years by perennial rhinitis and loss of the sense of smell. The course was perennial, and unrelated to the seasons. These clinical features resembled those of aspirin-induced asthma (AIA). Therefore, suspecting AIA, we performed a aspirin-DL-lysine iv challenge test. After aspirin-DL-lysine injection, F

Influence of airway resistance on hypoxia-induced periodic breathing.

We studied the effects of changing upper airway pressure on the variability of the dynamic response of ventilation to a hypoxic disturbance in 11 spontaneously breathing dogs. Supralaryngeal pressure, instantaneous inspiratory flow, end-expiratory lung volume, and the inspiratory and expiratory O2 and CO2 concentrations were continuously recorded at baseline and after a 1.5-min hypoxic stimulus (abrupt normoxic recovery). Arterial blood gases were obtained at baseline, at the end of the hypoxic period, and after 1 min of recovery. Airway resistances were modified during the recovery by changin

Acute respiratory failure following pharmacologically induced hyperventilation: an experimental animal study.

The pulmonary effects of hyperventilation following infusion of sodium salicylate into the cisterna magna was studied in 16 spontaneously breathing adult sheep. We found a fall in PaO2, a decrease in the static compliance of the respiratory system, abnormal chest roentgenographic films, and grossly abnormal lungs following 3.5 to 13 h of hyperventilation. A control group of 15 sheep (10 sheep similarly injected with sodium salicylate, but then sedated and paralyzed and ventilated at normal tidal volume and respiratory rate on a mechanical ventilator, and 5 sheep infused with saline alone and b

Proximal airway function 8 to 16 years after laryngomalacia: follow-up using flow-volume loop studies.

Twenty children who had endoscopically confirmed laryngomalacia were reviewed at 8 to 16 years of age. All had developed stridor in the first 6 weeks of life; stridor had stopped or largely resolved by age 4 years. Proximal airway function was assessed by expiratory and inspiratory flow volume loops and compared with control values. These children as a group had variable extrathoracic obstruction to inspiratory airflow, as indicated by a significantly low maximal inspiratory flow at 50% of vital capacity and a high mean ratio of maximal expiratory to maximal inspiratory flow at 50% vital capac

Characterizing extravascular lung water-A dual-contrast agent extracellular volume approach by cardiovascular magnetic resonance.

BACKGROUND: Pathological extravascular lung water is a facet of decompensated congestive heart failure that current cardiovascular magnetic resonance (CMR) methods fail to quantify. CMR can measure total lung water density, but cannot distinguish between intravascular and extravascular fluid, and thus is not diagnostic. Therefore, we develop and evaluate a novel method to measure extravascular lung water by distinguishing intravascular from extracellular fluid compartments using two different contrast agents, extracellular gadolinium chelates and iron-based intravascular ferumoxytol. METHODS:

Lung recruitability determines the impact of PEEP on mechanical power in ARDS.

BACKGROUND: Mechanical power increases with positive end-expiratory pressure (PEEP). However, its injurious potential may depend on the available lung gas volume, which can be modified by alveolar recruitment. We investigated how PEEP-induced recruitment affects mechanical power. METHODS: We analyzed previously collected data on 20 patients with acute respiratory distress syndrome who underwent a decremental PEEP trial (15-5 cmH₂O). End-expiratory lung volume and respiratory mechanics were measured to quantify recruited volume, functional residual capacity (FRC), and the recruitment-to-inflat

A new method for bedside determination of effective lung volume and functional residual capacity.

Established methods of measuring functional residual capacity (FRC) involve sophisticated equipment and elaborate procedures. Here we present a new method based on CO2 rebreathing that has a simple fast procedure and only requires end-tidal CO2 monitoring. Ten healthy subjects with diverse anthropometric and respiratory parameters were studied in the sitting position. Reference FRC (RefFRC) and tidal volume (TV) were measured with a Cosmed Quark PFT/DLCO unit using the single-breath methane dilution technique in combination with spirometry. Rebreathing through an external dead space of precise

Machine-learning-based prediction of respiratory flow and lung volume from real-time cardiac MRI using MR-compatible spirometry.

BACKGROUND: Cardiac real-time MRI (RT-MRI) in combination with MR-compatible spirometry (MRcS) offers unique opportunities to study heart-lung interactions. In contrast to other techniques that monitor respiration during MRI, MRcS provides quantitative respiratory data. Though MRcS is well tolerated, shortening of the scanning time with MRcS would be desirable, especially in young and sick patients. PURPOSE: The aim of the study was to predict airflow and lung volume based on RT-MR images after a short learning phase of combined RT-MRI and MRcS to provide respiratory data for subsequent short

Capnodynamic determination of end-expiratory lung volume in a porcine model of hypoxic pulmonary vasoconstriction.

PURPOSE: The capnodynamic method, End Expiratory Lung Volume CO2 (EELV-CO2), utilizes exhaled carbon dioxide analysis to estimate End-Expiratory Lung Volume (EELV) and has been validated in both normal lungs and lung injury models. Its performance under systemic hypoxia and variations in CO2 elimination is not examined. This study aims to validate EELV-CO2 against inert gas wash in/wash out (EELV- SF6, sulfur hexafluoride) in a porcine model of stable hemodynamic conditions followed by hypoxic pulmonary vasoconstriction and inhaled nitric oxide (iNO). METHODS: Ten mechanically ventilated pigl

Airway function in healthy subjects and patients with left heart disease.

1. In forty non-smoking healthy subjects and seventy-two patients with left heart diseases measurements were made of the volume expired in the first second of a forced expiration (FEV1) and the total volume expired in a forced expiration (FVC) before and after inhalation of salbutamol. Before and after salbutamol the healthy subjects and patients also inhaled maximally an inspirate, the first part of which contained 133Xe and, during controlled expiration, the radioactivity of the expirate was measured and plotted against its volume. the resulting curves were divided into phases of different s
