Thighplasty after bariatric surgery: evaluation of lymphatic drainage in lower extremities.

BACKGROUND: Bariatric surgery for morbid obesity achieves an accentuated weight loss, with skin and soft tissue redundancy in some corporal segments, including the thighs. The thigh lift is a plastic surgery that improves this redundant tissue in the medial thigh area, but this surgical procedure may have complications, such as inferior migration of the scar, delayed wound healing, vulvar distortion, and lymphatic system alterations. METHODS: To evaluate the effect of the thighplasty on the lymphatic system, 14 patients were prospectively included in this study. Lymphoscintigraphy was perform

Age-associated suppression of exploratory activity during sickness is linked to meningeal lymphatic dysfunction and microglia activation.

Peripheral inflammation triggers a transient, well-defined set of behavioral changes known as sickness behavior1-3, but the mechanisms by which inflammatory signals originating in the periphery alter activity in the brain remain obscure. Emerging evidence has established meningeal lymphatic vasculature as an important interface between the central nervous system (CNS) and the immune system, responsible for facilitating brain solute clearance and perfusion by cerebrospinal fluid (CSF)4,5. Here, we demonstrate that meningeal lymphatics both assist microglial activation and support the behavioral

Optimized lymphoscintigraphy and diagnostics of lymphatic oedema of the lower extremities.

BACKGROUND: The elementary diagnostic method for the detection of lymph circulation abnormalities in extremities is lymphoscintigraphy. This method provides information on the morphology and function of the lymphatic system. However, there are no generally acceptable standards for lymphoscintigraphy, which makes judgment of its clinical usefulness very difficult. MATERIAL AND METHODS: In this work, a discussion is presented on the usefulness of static qualitative scintigraphy of the lymphatic system of lower extremities as well as of its quantitative interpretation, depending upon: (1) time m

The Scaffold Protein Liprin β-1 (PPFIBP1) and the Intermediate Filament Synemin: Potential New Markers of Lymphatic Endothelial Cells.

There are a few molecules that are regularly used as markers for lymphatic endothelial cells (LECs) such as the adhesion molecule CD31/PEACAM1, the transcription factor PROX1, the Vascular Endothelial Growth Factor Receptor-3 (VEGFR3/FLT4), the glycoprotein podoplanin, and the hyaluronan receptor LYVE1. However, none of the molecules are exclusively expressed in LECs, and there is molecular and functional heterogeneity of LECs in initial lymphatics, lymphatic collectors and lymph nodes. Therefore, a combination of markers must be applied to identify lymphatics. This is particularly true for th

Defective lymphatic vasculature in obesity.

Lymphedema is an important, and often underdiagnosed complication of obesity and is likely due to acquired defects in the lymphatic vasculature. Study of diet-induced obesity animal models have indicated defective lymphatic vasculatures might extend to other anatomical sites, especially visceral depots. Excess mechanical pressure, metabolites, pro-inflammatory cytokines, and adipokines released during adipose tissue expansion can predispose lymphocytes to overactivation and apoptosis; compromising collecting lymphatic vessels; and triggering lymph node hypoplasia, fibrosis, and apoptosis. Cons

Deep Cervical Lymphovenous Anastomoses Improved Brain Function and Internal Neck Edema in Central Lymphatic Anomaly Associated with 13q Deletion Syndrome.

We report a case of a 21-year-old woman with 13q deletion syndrome, a rare chromosomal disorder associated with developmental delays, craniofacial dysmorphism, and multiorgan abnormalities associated with central conducting lymphatic anomaly. Her symptoms included bilateral chylothorax, pulmonary lymphangiectasis, generalized edema, and recurrent respiratory distress. Despite conservative management, her condition progressively worsened, leading to significant edema affecting the tongue and upper airway. Magnetic resonance lymphangiography revealed a severe central lymphatic flow disorder with

Platelet activation relieves liver portal hypertension via the lymphatic system though the classical vascular endothelial growth factor receptor 3 signaling pathway.

BACKGROUND: Liver cirrhosis and portal hypertension (PHT) can lead to lymphatic abnormalities and coagulation dysfunction. Because lymphangiogenesis may relieve liver cirrhosis and PHT, the present study investigated the gene expression alterations in the lymphatic system and the effectiveness of platelet-mediated lymphangiogenesis in improving liver cirrhosis and PHT. AIM: To investigate the role of lymphangiogenesis in preclinical PHT models. METHODS: Immunohistochemistry and transcriptome sequencing of bile duct ligation (BDL) and control lymphatic samples were conducted to reveal the ind

Dynamic Contrast-Enhanced Magnetic Resonance Lymphangiography and Percutaneous Lymphatic Embolization for the Diagnosis and Treatment of Recurrent Chyloptysis.

Chyloptysis, or the expectoration of triglyceride-rich sputum, is rare and typically treated with diet modification and thoracic duct ligation. This article describes 2 patients with prolonged histories of chyloptysis who failed conservative treatment and thoracic duct ligation. Dynamic contrast-enhanced magnetic resonance imaging delineated the lymphatic anatomy and identified the abnormal pulmonary lymphatic perfusion pathways in both patients. This imaging provided guidance for successful percutaneous lymphatic embolization which resulted in resolution of symptoms in both patients.

Testicular lymphangiectasis in Noonan's syndrome.

We describe a boy with testicular lymphangiectasis and Noonan's syndrome. Both testes showed seminiferous tubules with a reduced tubular diameter, containing few spermatogonia. The testicular interstitium exhibited a number of large, dilated lymphatic vessels forming irregular channels among the seminiferous tubules and surrounding them. Since there was no accompanying pathological condition to indicate an obstruction to the lymphatic flow at the level of the spermatic cord or in the regional lymph nodes, the abnormal development of testicular lymphatic vessels suggests a congenital malformati

[Extra-pulmonary lymphangiomyomatosis presenting as an exudative enteropathy with a rapid fatal course].

We report a case of lymphangiomyomatosis (LAM) which occurred in a twenty six year old woman, presenting with protein losing enteropathy and pancreatic fibrosis due to lymphostasis. Despite having no pulmonary lesion, the patient died quickly. The proliferation of smooth muscle involved the mesenteric, peripancreatic lymphatic channels but also the submucosal lymphatic vessels of the duodenum. Only one other case of LAM presenting with protein losing enteropathy has been reported, in which there were no abnormalities in the intestinal tractus or in the pancreas.

TIE1-dependent lymphatic vascular remodeling is mediated by its second tyrosine kinase domain.

Mutations in ANG2 and TIE1 are associated with primary lymphedema in humans, but the mechanisms of ANG/TIE signaling in the lymphatic vasculature remain incompletely understood. We document that TIE2 is not detected in lymphatic endothelial cells (LECs) before E14.5 but is expressed in collecting vessels from later embryonic stages, in contrast to robust TIE1 expression in all LECs from early stages. Accordingly, only LEC-specific deletion of Tie1 but not Tie2 resulted in defective lymphatic development and abnormal valve function. We discovered that defects of Tie1 lymphatic knockout mice wer

Evaluation of the degree and distribution of lymphangiectasia in full-thickness canine small intestinal specimens diagnosed with lymphoplasmacytic enteritis and granulomatous lymphangitis.

Intestinal lymphangiectasia (IL) is often observed in dogs with chronic small intestinal diseases. Hypoplasia of the lymphatic vessel due to decreased lymphangiogenesis, which has been suggested in human idiopathic IL, may contribute to the pathogenesis of canine IL. This study aimed to evaluate the diameter and number of lymphatic vessels in full-thickness small intestinal specimens of dogs with IL. Immunohistochemical labeling of lymphatic endothelial cell markers was performed on retrospectively retrieved full-thickness small intestinal specimens. Sixteen dogs with histologically confirmed

Role of high resolution ultrasound in parotid lesions in children.

PURPOSE: A pictorial review of the spectrum of sonographic abnormalities of the parotid gland in children is presented. METHODS: Two pediatric radiologists performed retrospective review of medical records and imaging findings of all parotid ultrasounds performed in 298 children in the age range of 24 days to 16 years, over a five-year period. RESULTS: The lesions varied from diffuse glandular abnormalities to discrete solid and cystic lesions. Of the 298 studies reviewed, 148 (49.6%) were normal results, while 150 (50.4%) had abnormalities of parotid gland identified at ultrasonography. The

Nodal status of malignant lymphoma in pelvic and retroperitoneal lymphatic pathways: PET/CT.

Nodal involvement of abdominal lymphatic pathways occurs in a number of histologic subtypes of malignant lymphoma. The histologic diagnosis of abnormal uptake in abdominal lymphatic pathways includes mainly non-Hodgkin lymphoma with B-cell lineage and Hodgkin lymphoma. Initial involvement of pelvic and retroperitoneal lymphatic pathways can result from a variety of underlying non-Hodgkin's lymphoma: follicular lymphoma, diffuse large B-cell lymphoma, marginal zone B-cell lymphoma of mucosa-associated lymphoid tissue (MALT) type, and mantle cell lymphoma. The diagnosis of these clinical entitie

Lymphedema secondary to limited cutaneous systemic sclerosis.

Systemic sclerosis (SSc) is an autoimmune connective tissue disease characterised by vascular abnormalities, immune system activation and fibrosis. Lymphatic involvement in SSc was described more recently and starts in early stages. This report describes a 46-year-old patient who developed over the last 2 years asymmetric lymphedema in lower extremities. Compromise in lymphatic drainage was confirmed by lymphoscintigraphy. She also presented Raynaud's phenomenon, a scleroderma pattern in nailfold capillaroscopy, cutaneous thickening and anticentromere antibodies, which together resulted in a n

Lymphatic regeneration following microvascular limb replantation: a qualitative and quantitative animal study.

Lymphatic regeneration following unilateral hindlimb autotransplantation was studied in 14 Lewis rats using Technetium-99 radiolabeled sulfur colloid (TC-99) lymphoscintigraphy and compared to the lymphatic pattern exhibited in four unoperated control rats. Control animals demonstrate a reproducible flow of lymph along the deep lymphatic system from the foot, draining into the ipsilateral inguinal nodes, and then up to the para-aortic nodes. Following replantation, lymphatic outflow from the replanted limb begins to occur within 3 to 6 days, reaching normal lymphatic clearance within 12 days.

Lymph vessel defects in patients with ovarian dysgenesis.

Lymphangiography of the lower extremities was performed in 21 patients with ovarian dysgenesis. Seventeen subjects turned out to have hypoplastic superficial lymph vessels of the lower limbs. Aplasia of the lymphatics of the legs occurred in four instances. Hypoplastic lymph pathways were also observed in the pelvis and in the retroperitoneal space. Although the impairment of the lymphatic channels occurred in all cases of ovarian dysgenesis, it appears that it is more severe in patients with streak gonads. There was no correlation between the lymphatic defect and the karyotype. Patients with

The Lymphatic System in the Fontan Patient-Pathophysiology, Imaging, and Interventions: What the Anesthesiologist Should Know.

The Fontan surgery was developed as a palliative intervention for congenital heart disease (CHD) patients with single-ventricle physiology who are not candidates for a biventricular repair. Improvements in the surgery and medical management of these patients have increased survival, yet this population remains at risk for complications and end-organ dysfunction due to Fontan failure. Lymphatic vessels maintain a fluid balance within the extracellular space, participate in fat reabsorption from the small intestine, and play an important role in the body's immune response. Altered Starling force

Use of mesenteric lymphangiography in a calf with chylothorax and chyloperitoneum.

Lymphatic abnormalities resulting in chylous effusion into a body cavity are uncommon in domestic animals. In a 6-day-old calf admitted to our hospital because of failure to suckle and abdominal distention, however, mesenteric lymphangiography revealed an obstruction of lymphatic flow. Laparoscopic examination of the abdomen was unsuccessful. Fluid accumulation was resolved in this calf by drainage. In cattle with chylothorax and concurrent chyloperitoneum in which a traumatic lesion of the thoracic duct is possible, conservative management, with drainage and supportive treatment, should be at
