The laws concerning driving licences and epilepsy in different countries are very diverse with regard to the criteria for issuance or renewal of licences, and also the methods of evaluating fitness. In 2011, a law was issued in Italy implementing the European directives on driving licences, including provisions for mandatory notification that a driver is epileptic. This was established regardless of the European rules that require compulsory notification only of patients. The Federation of Italian Boards of Physicians has made recommendations against mandated physician reporting, resulting in

To avoid loss of driving privileges, patients with epilepsy may elect not to report seizures to their physician or to the Department of Motor Vehicles (DMV). Reporting to the physician may differ depending upon the type of law in effect, i.e., mandatory physician reporting versus patient reporting to the DMV. Nondisclosure to the physician may have important consequences with regard to seizure management. We surveyed patients at two adult seizure clinics to determine the effect of driving laws on patients' reporting to their physician. One hundred fifty-eight patients completed an anonymous qu

Online or other child sexual exploitation or abuse (OCSEA) perpetration prevention programs are designed to provide services to individuals prior to offending. There is limited research on OCSEA perpetration prevention programs in jurisdictions with mandatory reporting. The present study is a mixed-method feasibility evaluation of the OCSEA perpetration prevention therapy program, Talking for Change (TFC). The feasibility study broadly addressed whether adult clients could be recruited to the program, the clinical profile of clients, and their satisfaction with the program. In total, there wer…

The concept of extensive intraductal component (EIC), currently defined by the presence of a prominent ductal carcinoma in situ (DCIS) component within an invasive tumor and extending beyond its margins, was introduced in the 1980s as a predictor of local recurrence following breast-conserving therapy for invasive breast carcinoma. At the time, surgical excision to negative margins was not the standard of care, making EIC a valuable tool for identifying patients at risk of recurrence. However, with modern oncologic and surgical advancements, its clinical relevance has diminished. Despite its c

BACKGROUND: Evidence-based, patient-specific estimates of abusive head trauma probability can inform physicians' decisions to evaluate, confirm, exclude, and/or report suspected child abuse.
OBJECTIVE: To derive a clinical prediction rule for pediatric abusive head trauma that incorporates the (positive or negative) predictive contributions of patients' completed skeletal surveys and retinal exams.
PARTICIPANTS AND SETTING: 500 acutely head-injured children under three years of age hospitalized for intensive care at one of 18 sites between 2010 and 2013.

Mortality surveillance and vital registration are limited in Sierra Leone, a country with one of the highest mortality rates among children aged <5 years worldwide, approximately 120 deaths per 1,000 live births (1,2). To inform efforts to strengthen surveillance, stillbirths and deaths in children aged <5 years from multiple surveillance streams in Bombali Sebora chiefdom were retrospectively reviewed. In total, during January 2015-November 2016, 930 deaths in children aged <5 years were identified, representing 73.3% of the 1,269 deaths that were expected based on modeled estimates. The "117

OBJECTIVES: Assessment of the relations between recorded gonorrhoea rates and clinical testing activity and disposable diagnostic tests.
METHODS: In Denmark, two sources of information on the epidemiology of gonorrhoea are available: (1) a mandatory clinical notification system (since 1867) comprising summary information about geographic distribution, season, age group and gender; in 1994, more detailed anonymous individualised epidemiological information was included; (2) a voluntary countrywide laboratory surveillance system for culture-confirmed cases (since 1957) comprising information ab

BACKGROUND: Most Pacific Island countries and territories (PICTs) provide some level of HIV testing for their populations and record demographic data for diagnosed cases of HIV and AIDS. These routine HIV data are usually held within the national Department/Ministry of Health or National AIDS Council. Summary HIV data can illustrate important trends in HIV infection over time and by age, gender and exposure. For some countries no other data are available to illustrate the extent and distribution of HIV infection in the region.
METHODS: Annual HIV and AIDS data were requested from all PICT rep

With a hepatitis B prevalence of 0.3%, Switzerland is a country with low endemicity. Unlike most other countries, Switzerland's recommendation for vaccination against hepatitis B has since 1998 focused on adolescents aged 11 to 15 years rather than on infants, in addition to risk groups since 1982. This paper describes the evolution of the incidence of acute hepatitis B virus (HBV) infection and newly reported chronic cases in Switzerland, as well as their epidemiological features, in order to discuss the implications for the control of hepatitis B through vaccination. Data from mandatory noti

Children and adolescents are at high risk for sexual assault. Early medical and mental health evaluation by professionals with advanced training in sexual victimization is imperative to assure appropriate assessment, forensic evidence collection, and follow-up. Moreover, continued research and outreach programs are needed for the development of preventative strategies that focus on this vulnerable population. In this review we highlight key concepts for assessment and include a discussion of risk factors, disclosure, sequelae, follow-up, and prevention.

Child abuse and neglect are prevalent issues that permeate all ethnic, cultural and socioeconomic segments of society. Parents of abused children frequently change physicians in order to prevent detection, but they are more likely to continue to visit the child's dentist. Most states recognize four major types of maltreatment: neglect; physical abuse; psychological maltreatment; and sexual abuse. The American Academy of Pediatric Dentistry defines dental neglect as "the willful failure of parent or guardian to seek and follow through with treatment necessary to ensure a level of oral health es

Leptospirosis, a zoonosis transmitted through contact with the urine of infected animals, either directly or through exposure to contaminated water or soil, is a frequent cause of acute febrile illness (AFI) and can be difficult to distinguish from dengue in areas where both are endemic. Approximately 5%-10% of patients with leptospirosis are affected severely; the case-fatality rate in those with severe disease is 5%-15%. Early identification of leptospirosis cases and early administration of penicillin G or doxycycline can reduce the duration and severity of illness.

Evaluating a driving licence holder's aptitude to drive will gain in significance in the coming years. In view of demographic developments and rising life expectancy, the focus will be primarily on medical examinations testing the driving ability of motorists aged over 70. Treating physicians or general practitioners will thus increasingly be confronted with questions such as how to handle patients who are no longer fit to drive, what duties of information they have vis-à-vis their patients in this respect, and whether they should or must report persons unfit for driving to the relevant road t

As computed tomographic (CT) colonography is being used increasingly in clinical practice, an effective quality improvement process must be ensured. The quality improvement process is outlined for the reader by using an adverse event during CT colonography as an example. Components of this process are the approach to a sentinel event, performance of a root cause analysis, and development of strategies for minimizing errors after a serious adverse event. Important factors include indications and contraindications for the examination, proper imaging technique, training of personnel, complication
