BACKGROUND: The sensory innervation of the lower jaw mainly depends on the third root of the trigeminal nerve, the mandibular nerve (V3). The aim of this single-center, prospective, randomized, double-blind, placebo-controlled study was to evaluate the effectiveness of bilateral V3 block for postoperative analgesia management in mandibular osteotomies.
METHODS: 107 patients undergoing mandibular surgery (75 scheduled osteotomies and 32 mandible fractures) were randomized in two groups. A bilateral V3 block was performed in each group, either with ropivacaine 0.75% (block group, n=50) or with

Mandibular reconstruction involving the central segment after malignant tumor resection requires the achievement of both functional and aesthetic quality. Three-dimensional reproduction based on the concept of a double arc composed of a marginal arc and an occlusal arc is important. Most reports of fibula flaps applied three-segmented closed wedge fibula osteotomy (bilateral and central segments); however, the aesthetic outcome sometimes became too stout for female patients because of the large central segment. We performed four-segmented fibula osteotomy for a 78-year-old woman using a semiop

Q4: Horizontal changes occur following bilateral sagittal split osteotomy (BSSO) in skeletal class III patients. The aim of this study was to assess the ostoperative changes in intergonial (IG) width and compare them between the positional screw and miniplate fixation methods in BSSO. This study evaluated patients who had mandibular prognathism and underwent BSSO for mandibular setback. Internal fixation was performed bilaterally, either with positional screws in the lateral ramus or with a miniplate. Postero-anterior cephalograms were obtained preoperatively (T1), at 1 month postoperative (T2

PURPOSE: To analyse the possible morphologic and positional changes of the mandibular condyles after orthognathic surgery.
MATERIAL AND METHODS: A prospective cohort study was performed. Patients with mandibular retrognathism were surgically treated to advance the mandible. The study group included seventeen patients (34 condyles) treated with sagittal split osteotomies alone (4 patients) or in combination with maxillary osteotomies (13 patients). Only condyles located on the mandibular side that advance during surgery were studied, therefore only 25 condyles entered this prospective study. B

This retrospective study compared the incidence of long-term inferior alveolar nerve injury for positional screws and miniplate fixation of bilateral sagittal split osteotomies carried out in 2 hospitals in Scotland. The study involved a mixture of mandibular setback and advancement surgery. The outcome of neurosensory deficit (NSD) was solely based on subjective assessment by the surgeons and patients' reported alteration in sensation. Numbness, tingling and any alterations in sensation beyond 6 months were considered long-term inferior alveolar nerve injury. This study was conducted on 28 sa

This study aimed to evaluate the correlation between the transverse displacement of the proximal segment after bilateral sagittal osteotomy for mandibular setback and the amount and design of the mandibular setback. Patients who underwent either bilateral sagittal split ramus osteotomy (BSSRO) alone or two-jaw surgery were selected, and cephalographic postero-anterior (PA) measurements were taken pre-operatively (T1), immediately post-operatively (T2), and at follow-up (T3). The inter-gonal (IG) and inter-ramal (IR) width increased immediately after surgery, but decreased to the initial value

The currently common procedures in orthognathic surgery are widely used. Different forms of skeletal malocclusion can be treated with great outcomes in terms of bite, bone harmony, function, improved mastication, esthetics, and even decreased problems with breathing. Each type and intensity of malocclusion may affect decision-making in terms of distraction, selection of osteotomy design, or even guide clinical and surgical decisions to use a less popular osteotomy protocol. CBCT (cone-beam computed tomography) is currently a milestone in the establishment of the condition of jaw bones, bone st

BACKGROUND: Mandibular reconstruction has evolved significantly since its inception in the early 1900s. Currently, the fibula free flap (FFF) is considered the gold standard for mandibular and maxillary reconstructions, particularly for extensive defects, and the introduction of Extended Reality (XR) and virtual surgical planning (VSP) is revolutionizing maxillofacial surgery.
METHODS: This study focuses on evaluating the accuracy of using in-house cutting guides for mandibular reconstruction with FFF supported by virtual surgical planning (VSP). Planned and intraoperative osteotomies obtaine

Despite the remarkable progress in virtual surgical planning in the last few decades, which has made orthognathic surgery results more precise and predictable, there remains an unforeseeable risk of developing mandibular lower border defects after bilateral sagittal split osteotomy (BSSO). These defects can cause visible and palpable irregularities of the lower border of the mandible, thereby worsening the surgical result. The study aimed to evaluate the benefit of injectable platelet-rich fibrin (i-PRF) for contour defect modelling in orthognathic surgery in comparison to conventional orthogn

OBJECTIVES: Among the existing techniques for the correction of mandibular posterior vertical insufficiency (PVI), the intra-oral ramus vertical lengthening osteotomy (IORVLO) can be proposed as it allows simultaneous correction of mandibular height and retrusion. This study assessed the 3D morpho-anatomical changes of the ramus-condyle unit and occlusal stability after IORVLO.
MATERIALS AND METHODS: This retrospective analysis compared immediate and 1-year post-operative 3D CBCT reconstructions. The analysis focused on the condylar height (primary endpoint) and on the changes in condylar (co

BACKGROUND: Mandibular tumor surgery necessitates precise osteotomies based on tumor boundaries; however, conventional osteotomies often lack accuracy in predicting osteotomy positions and planes, potentially leading to excessive resection of normal bone tissues or residual tumors, thus compromising postoperative quality of life and clinical outcomes. Robotic-assisted surgery (RAS) augmented with artificial intelligence (AI) offers precise localization capabilities, aiding surgeons in achieving accurate osteotomy positioning. This study aimed to evaluate the feasibility and accuracy of a robot

To reduce a large amount of mandibular setback and to prevent pharyngeal airway space narrowing when correcting a severe anteroposterior skeletal discrepancy, a mandibular step osteotomy is often combined with 2-stage orthognathic surgery. This case report describes a successful 2-stage orthognathic treatment combined with a mandibular step osteotomy. A 20-year-old man had severe mandibular protrusion, facial asymmetry, and macroglossia. Phase 1 surgery included the mandibular step osteotomy and a partial glossectomy; then we performed phase 2 surgery with a 2-piece LeFort I segmental osteotom
