Nipple-Sparing Mastectomy and Adequate Margins for Patients With Ductal Carcinoma In Situ.

BackgroundFor patients with ductal carcinoma in situ (DCIS) undergoing breast conservation surgery (BCS), guidelines advise a margin width of at least 2 mm, with studies demonstrating decreased recurrence risk compared to narrower margins. However, limited data exist establishing if this margin is appropriate in mastectomies, and specifically for nipple-sparing mastectomy (NSM). Consequently, we evaluated the margins of DCIS patients undergoing NSM and resulting oncologic outcomes.MethodsA single-institution retrospective review was performed in patients with DCIS or DCIS with microinvasion (D

Sentinel lymph node biopsy following prior augmentation mammaplasty and implant rupture.

We report the case of a 44 year old lady with bilateral cosmetic silicone breast implants who had previously undergone a change of her right implant following extracapsular rupture. She presented 4 years later with a new lump in her right breast and underwent subcutaneous mastectomy for a grade 3 invasive ductal carcinoma. Sentinel lymph node biopsy demonstrated axillary silicone lymphadenopathy but nil evidence of metastatic disease. We present this as the first described case of successful sentinel lymph node biopsy in the context of prior augmentation mammaplasty and ipsilateral implant rup

Robotic Nipple-Sparing Mastectomy with Immediate Prosthetic Breast Reconstruction: Surgical Technique.

Robotic nipple-sparing mastectomy could be a significant advancement in the treatment and prophylaxis of selected breast cancers. Motion-scaling, high-resolution, three-dimensional optics; tremor elimination; and instruments with enhanced precision with 7 degrees of freedom have allowed surgeons to overcome the limitations experienced with the endoscopic approach in breast surgery. Advantages of this procedure, in comparison with the open technique, are a shorter and more acceptable scar located in the lateral thoracic region, and greater respect for the vascularization of the mastectomy skin

Risk-reducing mastectomy.

Breast cancer is a malignant proliferation of epithelial cells lining the ducts or lobules of the breast. Excluding skin cancer, breast cancer is the most common cancer in women. Only lung cancer accounts for more cancer deaths in women. Breast cancer may exist for a long period either as an invasive or noninvasive disease, but not as a nonmetastatic disease. Consequently, timely diagnosis and appropriate management are lifesaving. Approximately 10% of human breast cancers are linked to germline mutations, such BRCA1 and BRCA2. Correct staging of breast cancer patients is critical. It permits

Mucocele-like tumor of the breast caused by ductal carcinoma in situ: a case report.

A mucocele-like tumor (MLT) of the breast caused by clinging and micropapillary carcinoma in situ is reported. The tumor was a smooth-contoured mass located near the nipple in the lower inner quadrant of the left breast. Ultrasonography showed a cystic tumor and distended ducts. An aspiration specimen contained abundant mucinous material and a small amount of epithelial cell components, but was insufficient for cytological diagnosis. An excisional biopsy specimen disclosed multiple cysts containing mucin with extravasation into the stroma. Additional pathologic diagnosis using consecutive micr

[Giant fibroadenoma of the breast. Its clinical picture and differential diagnosis. A report of a clinical case].

An 18-year-old girl came to our Institute with a large tumefaction at the right breast, that during the last year had risen slowly but continuously. She underwent some clinical and instrumental exams that allow us to diagnose a giant adenofibroma or cystosarcoma phyllodes without excluding either of them. Then she undergoes a subcutaneous mastectomy with an application of a prosthesis. From the histologic point of view it is a giant adenofibroma. Giant adenofibroma is a rare breast pathology, always benign. Nowadays we have some difficulties distinguishing it from cystosarcoma phyllodes, which

Mastectomy without drain at pectoral area: a randomized controlled trial.

OBJECTIVES: Mastectomy is still one of the standard alternative procedures for the management of female breast cancer. Axillary node dissection is also performed to establish the accurate staging. After operation, the axilla must be drained because of lymphatic leakage. Whether the raw surface at the pectoral area should be drained or not is an interesting controversial point. The authors conducted a randomized controlled trial to compare outcomes after modified radical mastectomy (MRM) with and without drainage at the pectoral area. METHODS: Sixty patients who agreed to be treated with MRM a

Minimally invasive endoscopic nipple-sparing mastectomy with immediate reconstruction with breast implant for ductal carcinoma in situ.

The standard of care for a large area of ductal carcinoma in situ within the breast is conventional mastectomy with or without immediate reconstruction. This case report describes Ireland's first minimally invasive endoscopic nipple-sparing mastectomy with immediate reconstruction with breast implant. While it has been practised in Europe and elsewhere, this was Ireland's first minimally invasive endoscopic nipple-sparing mastectomy. The reported benefits in published literature include higher wound-related satisfaction and aesthetics, improved post-operative pain and equivocal or better compl

Complications After Deep Inferior Epigastric Perforator Flap Breast Reconstruction for Nipple-Sparing Mastectomy: A Series of 380 Flaps.

BACKGROUND: Nipple-sparing mastectomy (NSMs) and the deep inferior epigastric perforator (DIEP) flap for breast reconstruction represent the latest evolutions of breast and reconstructive surgery, with resemblance to the aesthetics and feel of the natural breast without needing additional nipple reconstruction or tattooing. Despite clinical adoption of these advances, current literature of autologous reconstruction after NSM remains limited. Herein, we classify the incidence and predictors for the most common complications in patients undergoing NSM and DIEP flap reconstruction over a 10-year

Nipple-Sparing Mastectomy After Neoadjuvant Chemotherapy: Definitive Results with a Long-Term Follow-Up Evaluation.

BACKGROUND: Nipple-sparing mastectomy (NSM) is increasingly used for women with breast cancer who are not candidates for conservative surgery. The authors previously reported satisfying results with NSM after neoadjuvant chemotherapy (NACT). METHODS: From 2010 to 2020, 1072 women underwent mastectomy at the authors' institution. In this group, 433 NSMs were performed (40%). The only contraindications to NSM were close proximity to the nipple-areola complex (NAC), bloody discharge, and Paget disease. RESULTS: In 112 cases involving 111 women, NSM followed NACT (group 1), whereas it was perfor

Residual Oncological Risk After Chest Masculinization Surgery: Invasive Breast Cancer 2 Years After Surgery.

Chest masculinization surgery (gender-affirming mastectomy [GAM]) is increasingly performed to achieve a masculine chest contour. Unlike oncological mastectomy, GAM prioritizes aesthetic outcomes and may leave residual glandular tissue. Although breast cancer after GAM is uncommon, its occurrence raises important clinical considerations regarding residual risk and surveillance. We report a case of invasive ductal carcinoma arising 2 years after subcutaneous chest masculinization surgery in a transgender patient without testosterone exposure or known hereditary cancer predisposition. Clinical,

Combined cold atmospheric plasma and photobiomodulation is associated with improved early macroscopic and ultrasonographic wound-healing parameters after unilateral mastectomy in female dogs: an exploratory randomized intra-individual controlled clinical study.

BACKGROUND: Postmastectomy complications remain relevant in veterinary surgery. Although cold atmospheric plasma (CAP) and photobiomodulation (PBM) may improve tissue repair, evidence on their combined use is limited. This exploratory pilot study evaluated CAP-PBM therapy in nine female dogs undergoing unilateral mastectomy. METHODS: A prospective, randomized, blinded, intra-individual controlled clinical study was conducted in nine female dogs with mammary neoplasia undergoing clinically indicated unilateral mastectomy. In each dog, one half of the surgical wound was randomly assigned to rec

Improving Surgical Treatment for Severe Gynecomastia and Avoiding Skin Resection: Skin Redistribution and Fixation Approach for Aesthetic Excellence.

BACKGROUND: Gynecomastia, a common condition involving benign breast tissue enlargement in men, poses challenges for plastic surgeons, especially in severe cases. Current surgical methods for severe gynecomastia often require complex, invasive procedures with skin resection, resulting in significant scarring. Conversely, techniques avoiding skin resection frequently leave skin redundancy and an unsatisfied chest contour. OBJECTIVES: The authors of this study introduce a skin redistribution and fixation approach, which help to improve aesthetic outcomes and eliminate the need for skin excision

[Retrospective studies on the relation between fibrocystic disease and cancer of the breast with therapeutic conclusions].

On the basis of 597 cases of benign and malignant tumoral, respectively dysplastic diseases of the breast, operated at the IInd Surgical Clinic of Tirgu-Mureş during the period 1977-1983, the authors consider the fibrocystic disease (FCD) of the II and III degree as a facultative precancerous lesion. The extremely high frequency of FCD associated to the mammary cancer in young women, as well as of intense epithelial proliferations and intermediate forms, of transition from typical epithelial hyperplasias to the atypical forms observed in some of these cases, is pointed out. The 4 cases (1.94%)

Nipple-sparing mastectomy in modern breast practice.

Breast cancer management has evolved from the conventional radical Halsted to a fully integrated multidisciplinary approach. Nipple sparing mastectomy (NSM) is an innovative technique that preserves native breast envelope without mutilation of nipple-areola complex (NAC), thus providing a reasonable alternative for early cancers without NAC involvement and prophylactic high risk patients and avoids multiple surgical procedures required for reconstruction. This article aims to critically review indications, intra-operative protocols, radiotherapy planning and limitations of NSM. Patient selecti

[Reduction-plasty of the breast in symmastia].

Medial confluence of the breasts, known as symmastia, has received little attention in plastic surgery literature. By reporting on one case, we present our surgical technique. The chosen method of the reduction mammoplasty is considered to be particularly important. Liposuction is integrated into the surgical procedure. Suggestions made by other authors are discussed.

Influence of some operative and postoperative factors on capsular contracture around breast prostheses.

In a follow-up of 490 patients with breast implants, neither the size of the prosthesis nor the nature of the surgical procedure (simple augmentation mammaplasty or after subcutaneous mastectomy) increased the incidence of capsular contracture. Hematoma or infection were followed by a higher incidence of capsular contracture. Drainage with suction and the instillation of steroid around the prostheses seemed to be effective in reducing the incidence of capsular contracture. In the steroid-treated group, the need for open surgical treatment was reduced. The recurrence rate after closed capsuloto

A new circumareolar approach combined with power assisted liposuction in the management of advanced gynecomastia.

AIM: Gynecomastia is a benign proliferation of the glandular tissue of the male breast that causes considerable emotional distress due to restriction of daily activity, especially in young men. In patients with severe gynecomastia, a new approach is described with liposuction combined with circumareolar subcutaneous mastectomy with minimal periareolar scarring, including the elimination of the excess skin. Prospectively recorded data in a period of fifteen-month were evaluated retrospectively for aesthetic outcomes and early and late complications. MATERIAL-METHOD: A total of 18 patients (36

Case Report: Lateral Chest Placement of IPG for Hypoglossal Nerve Stimulator Implantation.

Hypoglossal nerve stimulation for the treatment of obstructive sleep apnea is an increasingly common procedure. The hypoglossal nerve is stimulated by an implanted pulse generator that is placed in a subcutaneous pocket in the upper chest over the pectoralis muscle. A pressure sensor is placed through a separate incision between two ribs to detect respiratory effort. This case reports an alternative implantation of the device and pressure sensor through a single shared incision lateral to the breast in a thin patient with a previous history of breast cancer, mastectomy, and subpectoral breast
